Herb-induced liver injury (HILI) is a global concern due to the uptrend in Complementary and Alternative Medicine (CAM). The authors add to the current literature by reporting a case of a 61-year-old man with recent travel to Haiti. His past medical history include hepatitis C virus treated in 2021 with a sustained virologic response (SVR). He presented with profound weakness and abnormal liver transaminases in the thousands. It was initially unclear what the etiology of the patient's hepatocellular necrosis was, however, the level of abnormality was most consistent with either toxic metabolic injury or vascular ischemic injury. We initiated N-acetylcysteine and vitamin K and had a positive outcome. Upon further questioning, he admitted to consuming an herbal product cleansing tea called "asowosi" in large quantities. We searched the botanical name of the extract and found the active ingredient was Momordica charantia. The team utilized the updated Roussel Uclaf Causality Assessment Method (RUCAM), and the results demonstrated a highly probable relationship with M. charantia.

Objetive: To assess the impact of %PEP on liver indicators and lipid profile two years after BS. Background: The prevalence of weight gain in the adult population continues to increase, 57.8% of the world's adult population will be overweight or obese by 2030. Methods: This is a retrospective cohort and descriptive study, performed by consulting the database of an Obesity and Digestive Surgery Clinic in the city of Santa Maria - (Rio Grande do Sul, Brazil). The study included 351 patients (284 women, 67 men), aged at least 18 years, who underwent bariatric surgery from March 2014 to March 2016. The following data were obtained from the patients' medical records: Weight, height, age, sex, associated morbidities, biochemical parameters. The data were described by mean and standard deviation, median and interquartile range, count, and percentages. Results: The results showed a significant reduction in excess loss, triglycerides (TG) and low-density lipoprotein (LDLc) cholesterol variables in the prospective period, while high-density lipoprotein (HDLc) cholesterol levels increased, thus minimizing the hepatic changes. There was a reduction in LDLc and an increase in HDLc at 24 months in both groups 1 and 2. Between 6 and 12 months, total cholesterol (TC) increased in group 2, however, in the period between 12 and 18 months, only G1 had triglycerides reduced. In the period of 18 and 24 months, there was a significant reduction in blood glucose in group 1. The variables AST and ALT were within the normal range, without significance. However, 84.3% of patients had grade I hepatic steatosis. Conclusion: Bariatric surgery is effective in reducing the %EWL, modifying the lipid profile and liver markers up to 24 months after the bypass, reducing associated comorbidities. More research is needed to clarify the impact of %PEP on liver indicators and lipid profile two years after SB.

BACKGROUND: Overweight and obesity are associated with deaths and diseases worldwide. Cordia ecalyculata is a plant marketed as a slimmer. METHODS: The study evaluated the anti-obesity effects of the dry extract from C. ecalyculata in rats fed with a standard diet (STD) or cafeteria diet (CD) receiving the dry extract from C. ecalyculata at 500, 1000, and 2000 mg/kg for 40 days. Furthermore, it evaluated the slimming effect on diet-induced obese rats by the treatment with the same doses for 30 days. The bodyweight of the rats, as well as the intake of food, was measured. Blood samples were collected to determine the liver function (albumin, alanine transaminase (ALT), alkaline phosphatase (ALP), glucose), renal function (urea and creatinine), and lipid profile (cholesterol, triglycerides). RESULTS: The genotoxic effect in peripheral blood was assessed through the comet assay. A lower C. ecalyculata dose significantly prevented the weight gain in rats fed with STD and CD and decreased body weight and intake food of obese rats. The biochemical parameters were not altered, except to increase the serum albumin. Only the higher dose induced DNA damage when evaluated in rats fed with CD in the slimming evaluation model used. CONCLUSIONS: These results reinforce the extract as an anti-obesity and slimming supplement.

The aim of this study was to evaluate the metabolic, inflammatory, and hepatic aspects, as well as the milk yield in heifers submitted to protocol for induction of lactation compared to primiparous cows. Sixty Holstein heifers were selected and enrolled into two groups: Control (n= 30), pregnant heifers and Induction heifers (n= 30), non-pregnant femeales, submitted to a lactation induction protocol. Blood samples were collected at: pre-lactation period (weeks -3, -2 and -1) and post-lactation period (weeks 1, 2 and 3), aiming to evaluate glucose, non-esterified fatty acids, paraoxonase-1, albumin, ALT, GGT and cortisol. The protocol efficiently induced lactation in all the heifers, which produced 74.54% of the total production of milk from primiparous cows. In the pre-lactation period, induced animals presented higher concentrations of non-esterified fatty acids than the Control heifers, and the opposite was observed in the post lactation period. In both moments albumin and ALT were lower in the Induction group, and paraoxonase-1 activity and GGT concentrations were higher, compared to the Control. Thus, lactation induction protocol is efficient to initiate milk production in dairy heifers with no considerable changes in energetic, metabolic and hepatic profile when compared to heifers in physiological lactation.(AU)
O objetivo deste estudo foi avaliar os perfis metabólico, inflamatório, hepático e a produção de leite de novilhas induzidas à lactação comparadas a primíparas. Sessenta novilhas da raça Holandês foram selecionadas e alocadas em grupos: controle (n=30), novilhas prenhas, e indução (n=30), novilhas vazias submetidas a um protocolo de indução de lactação. As amostras de sangue foram coletadas nas semanas -3, -2 e -1 (pré-lactação) e nas semanas 1, 2 e 3 (pós-início de lactação) para avaliação de glicose, ácidos graxos não esterificados, paraoxonase-1, albumina, ALT, GGT e cortisol. O protocolo induziu eficientemente a lactação em todas as novilhas, que produziram 74,54% da produção total de leite do controle. No período pré-lactação, o grupo indução apresentou maiores concentrações de ácidos graxos não esterificados que o controle, e o oposto foi observado pós-lactação. Em ambos os momentos, albumina e ALT foram menores no grupo indução, e a atividade da paraoxonase-1 e as concentrações de GGT foram maiores, em comparação ao controle. Assim, o protocolo de indução de lactação foi eficiente para iniciar a produção de leite em novilhas induzidas, além de terem sido observadas alterações nos perfis energético, metabólico e hepático em comparação a novilhas em lactação fisiológica.(AU)

Most populations of wild felines are now considered endangered or near extinction due to the indiscriminate hunting and the reduction of their natural habitat, caused by man. Due to the limited number of animals within a population, these species are especially vulnerable to the impacts of diseases and endogamy. In addition, wild felines are also susceptible to metabolic and behavioral changes related to a change in their natural conditions, habitat, and feeding, when the animal enters into captivity. There are few descriptions in the literature of the serum biochemistries of lions and tigers. This study presents the biochemical parameters of 23 captive lions (Panthera leo) and 10 captive Siberian tigers (Panthera tigris altaica) from the Zoological Park of the Sao Paulo Foundation. Serum urea, creatinine, total protein, albumin, total, direct and indirect bilirubin were measured. In addition, serum levels of ALT, AST, ALP, GGT, glucose, CK, lactate dehydrogenase, cholesterol, and triglycerides were determined, as well as calcium, phosphorus, chlorine, sodium, and potassium. Biochemical parameters did not differ significantly among males and females lions, but higher albumin values were observed in male Panthera tigris altaica in comparison to females. The values obtained in this study may be used as reference for captive individuals of Panthera leo and Panthera tigris altaica.(AU)
A maioria das populações de felinos selvagens é considerada ameaçada ou em extinção devido à caça indiscriminada e à redução de seu habitat natural causada pelo homem. Devido ao número limitado de animais, essas espécies são vulneráveis às doenças e endogamia, além das alterações metabólicas e comportamentais relacionadas à mudança das condições naturais, habitat e alimentação e, ainda, à condição de cativeiro. Há poucos relatos na literatura sobre a bioquímica sérica em leões e tigres. Este estudo apresenta os parâmetros bioquímicos de 23 leões de cativeiro (Panthera leo) e 10 tigres siberianos de cativeiro (Panthera tigris altaica) do Fundação Parque Zoológico de São Paulo. Ureia; creatinina; AST; ALT; ALP; GGT; bilirrubinas totais, diretos e indiretos; proteína total; albumina; CK; lactato desidrogenase; os níveis de triglicérides, de colesterol e de glicose séricas foram determinados; e também os eletrólitos cálcio, fósforo, cloro, sódio e potássio. Não foram encontradas diferenças de parâmetros bioquímicos relacionados ao sexo de Panthera leo, mas para Panthera tigris altaica foram observados valores mais elevados de albumina em machos. Os valores obtidos neste estudo podem ser usados como referência para os indivíduos em cativeiro de Panthera leo e Panthera tigris altaica.(AU)
