INTRODUCTION: The availability of a clinical decision algorithm for diagnosis of chronic lymphocytic leukemia (CLL) may greatly contribute to the diagnosis of CLL, particularly in cases with ambiguous immunophenotypes. Herein we propose a novel differential diagnosis algorithm for the CLL diagnosis using immunophenotyping with flow cytometry. METHODS: The hierarchical logistic regression model (Backward LR) was used to build a predictive algorithm for the diagnosis of CLL, differentiated from other lymphoproliferative disorders (LPDs). RESULTS: A total of 302 patients, of whom 220 (72.8%) had CLL and 82 (27.2%), B-cell lymphoproliferative disorders other than CLL, were included in the study. The Backward LR model comprised the variables CD5, CD43, CD81, ROR1, CD23, CD79b, FMC7, sIg and CD200 in the model development process. The weak expression of CD81 and increased intensity of expression in markers CD5, CD23 and CD200 increased the probability of CLL diagnosis, (p < 0.05). The odd ratio for CD5, C23, CD200 and CD81 was 1.088 (1.050 - 1.126), 1.044 (1.012 - 1.077), 1.039 (1.007 - 1.072) and 0.946 (0.921 - 0.970) [95% C.I.], respectively. Our model provided a novel diagnostic algorithm with 95.27% of sensitivity and 91.46% of specificity. The model prediction for 97.3% (214) of 220 patients diagnosed with CLL, was CLL and for 91.5% (75) of 82 patients diagnosed with an LPD other than CLL, was others. The cases were correctly classified as CLL and others with a 95.7% correctness rate. CONCLUSIONS: Our model highlighting 4 markers (CD81, CD5, CD23 and CD200) provided high sensitivity and specificity in the CLL diagnosis and in distinguishing of CLL among other LPDs.

BACKGROUND: Previous studies have observed that psychosocial factors are associated with an increase in work-related musculoskeletal disorders (WMSDs) in European countries. However, the influence of psychosocial factors on the WMSD symptoms has not been studied in detail. Additionally, working conditions differ between developing and developed countries. OBJECTIVE: In this study, the influence of psychosocial factors on WMSD symptoms among men and women in the footwear industry in northeastern Brazil is investigated. METHODS: A questionnaire survey was conducted to evaluate workers' perceptions of psychosocial factors and WMSD symptoms. The influence of psychosocial factors on WMSD symptom trends was observed through ordinal logistic regression models. RESULTS: It was found that different psychosocial factors are related to WMSD symptom intensification. Factors such as "job insecurity" and "low social support," which are not associated with the worsening of WMSD symptoms in developed countries, showed a noticeable influence in the sample analyzed. CONCLUSION: These findings may be associated with differences in working conditions and economic problems in developing countries, suggesting that some psychosocial factors have a different effect on workers' perceptions in developing countries.

Introduction: Perinatal asphyxia is one of the main causes of morbidity and mortality in newborns. It generates high costs, both social and economic, and presents modifiable risk factors. Objective: To determine the biological and psychosocial factors and risk behaviors associated with the development of perinatal asphyxia (Sarnat II-III) in newborns from low socioeconomic status in a tier III university hospital in the city of Cali, Colombia. Materials and Methods: With a case and control design, 216 patients were studied (54 cases/162 controls) (1 case/3 matched controls). The cases were defined as newborns with modified or severe perinatal asphyxia (Sarnat II-III) between 2012 and 2014, with gestational age ≥ 36 weeks, with neurological signs not attributable to other causes, multiorgan compromise, advanced reanimation, and presence of a sentinel event. For the analysis, conditional logistic regression models were developed to evaluate association (OR), considering that the cases and controls had been paired by the birth and gestational age variables. Results: The final model showed that, from the group of biological variables, meconium amniotic fluid was identified as a risk factor (OR 15.28, 95%CI 2.78-83.94). Induction of labor lowered the risk of perinatal asphyxia by 97% (OR 0.03, 95%CI 0.01-0.21), and monitoring of fetal heart rate was associated with lower odds by 99% (OR 0.01, 95%CI 0.00-0.31) of developing perinatal asphyxia in the newborn. Regarding social variables, the lack of social support was identified as a risk factor for the development of perinatal asphyxia (OR 6.44, 95%CI 1.16-35.66); in contrast, secondary education lowered the odds of developing perinatal asphyxia by 85% when compared with pregnant women who only had primary school education (OR 0.15, 95%CI 0.03-0.77). Conclusion: Assessment of biological and psychosocial factors and social support is important in pregnant women to determine the risk of developing perinatal asphyxia in a low-income population.

Postharvest diseases and disorders are two of the most important parameters associated with the quality of avocado fruit. The aim of this study was to identify postharvest diseases and disorders in Hass avocado plots and to evaluate their relationships with different preharvest agronomical practices. This work was developed in 20 commercial plots of Hass avocado dedicated to production for national and export markets. The first part of this work was associated with the identification and characterization of diseases and disorders related with postharvest of avocado. In addition, it was determined the incidence of each disease and disorder based on simulation of postharvest scenarios for the national and export markets. Using a multinomial logistic regression model, it was possible to determine that the presence of each disease and disorder were related to crop management practices, soil and leaf and fruit nutrients levels. Most relevant postharvest disease and disorders were anthracnose, stem-end rot, chilling injury, and lenticel damage. Additionally, variables such as dry matter, plant pruning, and tissue concentration of Ca+2 were related with some pathologies and disorders. This work presents an advance in the recognition of postharvest diseases and disorders in avocado under tropical conditions, besides determining the main edaphic and anthropogenic associated factors.

In low-endemic areas for malaria transmission, asymptomatic individuals play an important role as reservoirs for malarial infection. Understanding the dynamics of asymptomatic malaria is crucial for its efficient control in these regions. Genetic host factors such as Toll-like receptor (TLR) polymorphisms may play a role in the maintenance or elimination of infection. In this study, the effect of TLR polymorphisms on the susceptibility to malaria was investigated among individuals living in the Atlantic Forest of São Paulo, Southern Brazil. A hundred and ninety-five Brazilian individuals were enrolled and actively followed up for malaria for three years. Twenty-four polymorphisms in five toll-like receptor (TLR) genes were genotyped by RFLP, direct sequencing or fragment analysis. The genotypes were analyzed for the risk of malaria. Ongoing Plasmodium vivax or P. malariae infection, was identified by the positive results in PCR tests and previous P. vivax malaria, was assumed when antiplasmodial antibodies against PvMSP119 were detected by ELISA. An evaluation of genomic ancestry was conducted using biallelic ancestry informative markers and the results were used as correction in the statistical analysis. Nine SNPs and one microsatellite were found polymorphic and three variant alleles in TLR genes were associated to malaria susceptibility. The regression coefficient estimated for SNP TLR9.-1237.T/C indicated that the presence of at least one allele C increased, on average, 2.3 times the malaria odds, compared to individuals with no allele C in this SNP. However, for individuals with the same sex, age and household, the presence of at least one allele C in SNP TLR9.-1486.T/C reduced, on average, 1.9 times the malaria odds, compared to individuals with no allele C. Moreover, this allele C plus an S allele in TLR6.P249S in individuals with same sex, age and ancestry, reduced, on average, 4.4 times the malaria odds. Our findings indicate a significant association of TLR9.-1237.T/C gene polymorphism with malarial infection and contribute to a better knowledge of the role of TLRs in malaria susceptibility in an epidemiological setting different from other settings.

Truck drivers have been involved in a significant number of road fatalities in Colombia. To identify variables that could be associated with crashes in which truck drivers are involved, a logistic regression model was constructed. The model had as the response variable a dichotomous variable that included the presence or absence of a crash during a specific trip. As independent variables the model included information regarding a driver's work shift, with variables that could be associated with driver's fatigue. The model also included potential confounders related with road conditions. With the model, it was possible to determine the odds ratio of a crash in relation to several variables, adjusting for confounding. To collect the information about the trips included in the model, a survey among truck drivers was conducted. The results suggest strong associations between crashes (i.e., some of them statistically significant) with the number of stops made during the trip, and the average time of each stop. Survey analysis allowed us to identify the practices that contribute to generating fatigue and unhealthy conditions on the road among professional drivers. A review of national regulations confirmed the lack of legislation on this topic.

O ambiente urbano influencia a saúde e os comportamentos humanos, sendo necessário um melhor entendimento dos determinantes da saúde das populações que vivem nas cidades. A partir dos dados da Pesquisa Nacional por Amostra de Domicílios (PNAD) 2008, usando modelos logísticos ordinais incorporando o plano amostral, o presente artigo avalia a associação entre a área de localização do domicílio (urbana e rural) e o estado de saúde autorreferido da população adulta brasileira, controlando para um conjunto de fatores individuais e do ambiente intradomiciliar e extradomiciliar. Os resultados indicam que ao controlar por fatores individuais e ambientais, a associação entre a área de localização do domicílio e o estado de saúde autorreferido dos adultos se modifica (passando de OR=1,51 para OR=0,96) e perde a sua significância estatística (p-valor=0,208). Entretanto, foram observadas interações estatisticamente significantes entre a área de localização do domicílio e as seguintes variáveis: sexo, cor/raça, morbidade autorreferida, posse de bens básicos e percentual de domicílios adequados quanto a qualidade de moradia.
The urban environment influences human health and behavior, and a clearer understanding of the health determinants of populations living in cities is needed. Using data from the National Household Sampling Survey (PNAD) 2008, with ordinal logistic models incorporating the sampling plan, this study assessed the association between the location of the home (urban or rural) and the self-reported state of health of the adult Brazilian population, taking into account a set of individual and environmental factors inside and outside the home. The results indicated that after allowing for individual and environmental factors, the association between the location of the home and the self-reported state of health is changing (going from OR = 1.51 to OR = 0.96) and losing its statistical significance (p-value = 0.208). However, statistically significant interactions were observed between the location of the home and the following variables: sex, color/race, self-reported morbidity, possession of basic goods and percentage of households with adequate living conditions.
