Aim: The main purpose of this study was to conduct a narrative review investigating the possible relationship between permanent maxillary anterior teeth and anthropometric facial parameters in different populations. Methods: Searches were performed in the PubMed, BVS (Biblioteca Virtual em Saúde) and SciELO databases to identify relevant scientific articles using the following search terms: "maxillary anterior teeth", "facial measurements" and "anthropometry", in such a way that 218 publications were found. After application of inclusion and exclusion criteria, 13 publications remained for full-text reading. Results: All studies involved male and female samples and it was notorious that male measurements were unanimously higher than those obtained in the opposite sex. The age of the analyzed participants ranged from 17 to 60 years; however, a mean age of 18 to 25 years was the most investigated in literature. In addition, facial measurements including the bizygomatic width, interpupillary distance, intercanthal distance, interalar width and intercommissural width have been proposed to help determine the correct size of anterior teeth. Conclusion: It was concluded that despite the limited number of studies, some factors that influence dental and facial dimensions, such as sex and age, can be identified. However, there is no standardization of the facial or dental parameters used in the studies, a fact that makes it difficult to establish a universal ratio for clinical dental practice

OBJECTIVE: The aim of this study was to systematically review observational studies assessing the association between socioeconomic status (SES) and traumatic dental injuries (TDI) in permanent dentition. METHODS: Electronic searches were performed in PubMed, EMBASE, Web of Science, LILACS, CINAHL, COCHANE Library and ScoINDEX databases for articles published up to February 2023. Two independent reviewers performed the search and critical appraisal of the studies. The inclusion criteria were observational studies that evaluated the association between individual-level socioeconomic indicators and TDI (clinically examined) in permanent teeth. Quality assessment of included articles was conducted using the Newcastle-Ottawa Scale. Global meta-analysis was performed with all studies and different subgroup analysis based on socioeconomic indicators (household income, educational level or any other indicator), age (children, early adolescents, late adolescents or young adults) and economic classification of the country (high, upper-middle or lower-middle). A random-effects model was used to estimate pooled prevalence ratios (PR) and respective 95% confidence intervals (CI) for each study. RESULTS: The search strategy retrieved 11 315 publications. According to eligibility criteria, 17 articles were included in the meta-analysis. Individuals with low SES were 17% more likely to have TDI (PR 1.17; 95% CI 1.05-1.30). The subgroup analysis also revealed that the indicator (household income, PR 1.16; 95% CI 1.00-1.34) and the economic classification of the country (upper-middle, PR 1.19; 95% CI 1.07-1.33) influenced the association of SES with TDI occurrence. CONCLUSIONS: Individuals with lower SES were more likely to present with TDI in permanent dentition than those with higher SES.

The aim of this study was to assess the erosive tooth wear (ETW) at early and mature adulthood in subjects with natural normal occlusion. The sample consisted of 23 untreated subjects with normal occlusion. Dental models were taken longitudinally from the same subjects at 13 (T0), 17 (T1) and 60.9 years of age (T2) at a university. Evaluation of ETW was performed using a modified Basic Erosive Wear Examination (BEWE) index. Interphase changes were evaluated using Friedman and Dunn's test. Ordinal logistic regression was used to assess the influence of sex, dental arch, tooth and dental surfaces on the erosive tooth wear. Linear regression was used to evaluate whether the ETW degree at T1 could discriminate the degree of ETW at T2. The significance level adopted was 5%. ETW showed a significant increase with aging. The median ETW index at T0, T1 and T2 was 2, 4 and 7, respectively. ETW was greater in males in the incisors and canines and on the incisal/occlusal and lingual tooth surfaces. No significant differences were found between the maxillary and mandibular arches. Subjects with severe ETW at mature adulthood had greater tooth wear at age 17. In conclusion, ETW significantly increased during aging in subjects with normal occlusion. The greater the degree of tooth wear at early adulthood, the greater the tooth wear at mature adulthood. Preventive care should be recommended during early adulthood in patients demonstrating erosive tooth wear in order to avoid worsening with aging.

The aim of this prospective cohort study was to assess the radiographic progression of underlying dentin shadows (UDS) on the occlusal surfaces of permanent posterior teeth of adolescents and young adults over 1-2 years and to identify possible risk factors. A total of 149 UDS lesions (from 101 individuals) were included at baseline. Each participant had to present at least one UDS to be considered eligible for the study. Data collection included the application of a questionnaire, clinical examination, and bilateral bitewing radiographs, performed at baseline and after 1-2 years. The association between possible predictors and UDS progression (defined radiographically as an increase in the radiographic score from baseline to follow-up) was assessed using Weibull regression models. Hazard ratios (HRs) and their 95% confidence intervals (CIs) were estimated. A total of 81 individuals (mean age: 24.0, standard deviation: 8.03) were reexamined after 1-2 years (742 occlusal surfaces, of which 118 were UDS). The overall progression rate was 8.6% after 1-2 years, being 12.6% for UDS without baseline radiolucency and 20% for UDS with baseline radiolucency. The risk analysis showed that UDS without radiolucency at baseline had a similar likelihood of progression (adjusted HR = 1.71, 95% CI = 0.68-4.32, p = 0.26) while UDS with radiolucency at baseline were more likely to progress (adjusted HR = 2.96, 95% CI = 1.06-8.26, p = 0.04) than the reference category (sound occlusal surfaces without radiolucency). These estimates were adjusted for caries prevalence, tooth type, and arch. This study showed low progression rates of UDS after 1-2 years. The presence of radiolucency at baseline was found to predict UDS progression.

OBJECTIVES: Answer the PICO question: Do class I and II posterior restorations in permanent teeth placed with high-viscosity glass-ionomer cement (HV-GIC) fail more than composite resin (CR) restorations? DATA: The study was registered in the PROSPERO database (CRD42020138290). Randomized and controlled clinical trials, comparing the performance of HV-GIC and CR in load bearing cavities of posterior permanent teeth were included. Cochrane risk of bias tool and GRADE were used to assess the quality and certainty of the evidence. Meta-analyses were performed for clinical outcomes on USPHS and FDI criteria for 12-, 24- and 36-months follow-ups. SOURCES: PubMed, Scopus and Web of Science were last searched on April 2, 2022, without language or date restrictions. Reference lists of primary studies and their related article link in PubMed were manually searched. STUDY SELECTION: Ten studies were included, while data from 8 were used for the meta-analyses. A total of 849 HV-GIC and 800 CR restorations were followed. The primary outcome was the fracture/retention of the restoration, with a comparable performance for both materials on all follow-ups. The 36 months follow-up for class I restorations (longest) showed risk difference of -0,00 (95%CI -0,03 to 0,03; p = 0,98) and no heterogeneity (p = 0,98, I2=0%). The certainty of the evidence is moderate, as all included studies were at an uncertain risk of bias. CONCLUSIONS: HV-GIC and CR presented comparable clinical performance in posterior permanent teeth up to 36 months. HV-GIV wear in class I restorations followed by 24 months was the only poorer result compared to CR. CLINICAL SIGNIFICANCE: Conservative load bearing cavities in permanent posterior teeth can be restored with HV-GIC with comparable clinical performance to CR expected at least up to 3 years. HV-GIC is a valuable direct restorative option for posterior teeth in high caries risk patients, in which CR is frequently associated with failure.

This 2.5-year cohort study investigated whether patient's caries activity is independently associated with caries increment among adolescents, regardless of previous caries experience, in a sample of 801 adolescents from South Brazil. Caries examination was performed at baseline (12 y) and at follow-up (14-15 y). Caries activity was significantly associated with caries increment even after adjustment for sex, socioeconomic status, type of school, and previous caries experience at both cavity and non-cavitated levels. Caries-active adolescents had approximately 2-fold higher risk of caries increment than those without caries activity (cavity level, incidence risk ratio [IRR] = 1.90, 95% confidence interval [CI] = 1.45-2.49, p < 0.001; non-cavitated level, IRR = 2.16, 95% CI = 1.63-2.86, p < 0.001).

The aims of this study were to estimate the risk of caries in the permanent teeth at 12 years of age and to describe the diagnostic accuracy of caries patterns in the primary dentition at age 4 years to predict caries at age 12 years. A prospective cohort study followed children from birth to age 12 years in the city of São Leopoldo, Brazil. Sociodemographic variables were collected at birth, and dental caries was measured at 4 and 12 years of age (n = 204). At 4 years, children were classified according to the presence of caries (cavitated and non-cavitated lesions), number of lesions, affected segment (anterior or posterior), and affected surface (occlusal, smooth, or proximal). Prediction of permanent dentition caries occurrence (DMFT ≥1) (primary outcome) involved Poisson regression with robust variance and standard diagnostic accuracy measures. The prevalences of caries at age 4 years (including non-cavitated lesions) and 12 years were 61.8% and 42.2%, respectively. All caries patterns in the primary dentition were associated with caries in the permanent dentition. In multivariable analysis, the strongest associations were carious lesions on the primary posterior teeth (RR 2.2; 95% CI 1.5-3.2) and occlusal surfaces (RR 2.1; 95% CI 1.4-3.0). Among patterns evaluated, the presence of any tooth with caries (cavitated or non-cavitated) had the highest sensitivity (73%), but any tooth with cavitated decay had the highest accuracy (67%). In conclusion, any dental caries experience in early childhood is strongly predictive of dental caries experience in early adolescence. Primary dentition carious lesions on the posterior teeth or occlusal surfaces and the presence of cavitated lesions were stronger predictors.

OBJECTIVES: To evaluate the validity of partial protocols (PP) to assess the prevalence of developmental defects of enamel (DDE) in permanent teeth and identify the strength of the association between DDE and some risk factors, using PP compared to the full-mouth (FM) exam. MATERIALS AND METHODS: This study was conducted in a population-based birth cohort of children born in 2004 in Pelotas, Southern Brazil. Socioeconomic, demographic, pre-, per-, and post-birth variables were collected. A subsample of 994 children was clinically examined for DDE in 2017, using the modified DDE index, using the "full- mouth" (FM) protocol. After FM had been performed, a dataset was created. Two different partial protocols (PP) were simulated from FM data: "only buccal surfaces (BS)" and "incisive and molars only (IM)." Sensitivity, absolute and relative bias, and inflation factors were calculated. RESULTS: For any DDE, FM had prevalence of 40.8%. The prevalence of DDE was 38.8% and 36.0%, for BS and IM protocols, respectively. When tested for any DDE, PP "BS" and "IM" showed high sensitivity. The underestimation of the true prevalence did not exceed 6.9% for PP "BS" and 16.1% for PP "IM." All protocols showed similar magnitude of association with the selected risk factors. CONCLUSION: Both PP "BS" and "IM" can be used to estimate the prevalence of DDE in epidemiological studies. CLINICAL RELEVANCE: Oral health surveys now have the option of using PP to collect DDE prevalence and investigate their association with risk factors, being less time-consuming, expensive, and labor intensive.

Objective Dental caries in Mexico continues to be a public health problem, indicated by it is high prevalence and incidence in children. This disease is associated with pain in preschool and school children, with large-scale consequences for the individual, society, and health systems. The objective of the present study was to determine the experience (mean of caries indices), prevalence, and severity of caries in children 2-12 years of age who sought dental care at a dental school in Mexico. Material and methods A cross-sectional study was carried out on 826 clinical records of patients ages 2-12 years. The dependent variable was caries, in terms of experience (mean primary teeth (dmft) and permanent teeth (DMFT) indices), prevalence (dmft and DMFT greater than 0), and severity (cutoffs of caries indices at various levels). The independent variables were age and sex. The data were analyzed in Stata 14 (StataCorp LLC, College Station, Texas). Results The average age was 7.2±2.3; 52.4% were boys. In the primary dentition, the caries experience (mean dmft) was 3.01±3.30, the prevalence of caries was 65.8%, and the severity dmft>3=37.3% and dmft>6=15.6%. In the permanent dentition, the caries experience (mean DMFT) was 0.99±1.88, the prevalence of caries was 31.5%, and the severity DMFT>3=12.5% ​​and DMFT>6=1.6%. The percentage of caries-free children in both dentitions was 26.1% (n=216/826). The experience, prevalence, and severity of caries were statistically different by age (p<0.001). Only in the severity of dmft>3 were differences observed across sex (p<0.05). Among children with mixed dentition, primary teeth were more affected than permanent teeth (2.46±2.87 vs 0.88±1.61; p<0.0001). Conclusions About seven out of 10 children were affected by cavities in either or both dentitions. It was observed that age was positively associated with dental caries, equally affecting girls and boys. In this sample, the primary dentition was impacted more than the permanent dentition. Despite being preventable, dental caries continues to be a health problem in children.

Although fluoride (F) is required for the normal growth and development of several human organs and tissues, excessive exposure to it may be potentially toxic. Groundwater may present ranging levels of F; however, the appearance, taste, and smell are not altered. Thus, the present study aimed to evaluate dental fluorosis in children from a Brazilian slave-descendent community, as well as to assess F levels in the drinking water supplies available in that area. For that, 21 children aged from 6 to 14 years living in Rincão dos Martimianos were invited to participate. Thylstrup and Fejerskov index (a 0­9 scale) was used to assess dental fluorosis on permanent teeth. Furthermore, the F concentration of two water samples (from the artesian well and a tank) was determined by using a potentiometric method. All children presented dental fluorosis to some degree, ranging from score 2 to 9. About 81% of them had dental fluorosis scores equal to or over 5. Scores lower than 5 were observed only in children younger than 8 years; on the other hand, all children older than 8 years presented scores higher than 5. Moreover, artesian well water had an F concentration of 5 mg/L and tank water 0.8 mg/L. It is suggested that the F-rich groundwater supply was most probably responsible for dental fluorosis in that area. Brazilian slave-descendent communities, therefore, should receive constant attention from their local authorities in order to guarantee a proper water supply for consumption, as well as to provide public health education(AU)


Embora o flúor (F) seja necessário para o crescimento e desenvolvimento normal de vários órgãos e tecidos humanos, a exposição excessiva pode ser potencialmente tóxica. As águas subterrâneas podem apresentar níveis variados de F; no entanto, a aparência, o sabor e o cheiro não são alterados. Desta forma, o presente estudo teve como objetivo avaliar a fluorose dentária em crianças de uma comunidade brasileira descendente de escravos, bem como avaliar os níveis de F na água potável disponível nessa área. Para isso, 21 crianças de 6 a 14 anos residentes no Rincão dos Martimianos foram convidadas a participar da pesquisa. O índice de Thylstrup e Fejerskov (uma escala de 0 a 9) foi usado para avaliar a fluorose dentária em dentes permanentes. Além disso, a concentração de F de duas amostras de água (do poço artesiano e de um tanque) foi determinada pelo método potenciométrico. Todas as crianças apresentaram algum grau de fluorose dentária, variando de 2 a 9. Cerca de 81% delas apresentaram escores de fluorose dentária iguais ou superiores a 5. Escores inferiores a 5 foram observados apenas em crianças menores de 8 anos; por outro lado, todas as crianças maiores de 8 anos apresentaram escores superiores a 5. Além disso, a água do poço artesiano tinha concentração de F de 5 mg/L e a água do tanque 0,8 mg/L. Desta forma, acredita-se que a fonte de água subterrânea rica em F foi provavelmente responsável pela fluorose dentária na região avaliada. As comunidades quilombolas brasileiras, portanto, devem receber atenção constante de suas autoridades locais, a fim de garantir o abastecimento adequado de água para consumo, bem como proporcionar educação em saúde pública(AU)

Developmental defects of enamel (DDEs) are widely observed in children and are related to the appearance of dental caries, malocclusion, tooth sensitivity, and unfavorable esthetic conditions. The objective of this cross-sectional study was to determine the prevalence and distribution of enamel defects present in children aged 6 to 12 years in the provinces located in southern Ecuador. A total of 1606 schoolchildren were examined under the World Health Organization criteria for diagnosis of DDEs. The results are presented using percentage frequency measures and chi-square associations. Some types of DDEs were presented by 50% of the schoolchildren, mainly diffuse opacity, with no statistical differences according to place of residence and/or environment, sex, and age (p > 0.05). In Ecuador, it is necessary to carry out studies on the factors that trigger enamel defects, since they may be associated with the high prevalence of caries already reported in other studies in the country.

Background: Removing plaque with toothbrush and toothpaste from proximal cavities in primary molars without restoring them follows sound cariological principles. But does this treatment affect space for and alignment of their permanent successors negatively? Hypothesis: There is no difference in impaction and displacement of the premolars, as well as in the D+E space in quadrants with three different statuses of the proximal surface of primary molars over a 4-year period. Methods: A total of 936 quadrants (466 maxillary and 470 mandibular quadrants) in 233 children were assessed. Treatment of cavities in the proximal surfaces of the primary molars consisted of amalgam and ART restorations using high-viscosity glass-ionomer cement, and cleaning of open large- and medium-sized cavities with toothbrush and toothpaste (UCT) under supervision for 220 days per year over 3 years. Dental casts were made at baseline, and after two, three, and 4 years. The D+E spaces were measured digitally. Status of the proximal surface of the primary molars was assessed by two calibrated examiners, and quadrants were grouped into normal anatomy, defective restoration, and proximal cavity. ANCOVA, ANOVA and LSD tests were applied. Results: There was a statistically significant difference between groups (p <= 0.001) and between evaluation times (p < 0.001), for the D+E space in both the maxilla and mandible. A sex difference related to the D+E space in the maxilla was found (p = 0.007). For boys, quadrants in the maxilla of the group 'proximal cavity' showed a significant shorter D+E space when compared to quadrants of the group 'normal anatomy' at the 3- and 4-year evaluation time. For girls the difference between the two groups was only present at the 3-year evaluation time. There was no significant difference between the D+E space in quadrants with defective restorations and those with normal anatomy in the mandible and in the maxilla. Displacement and impaction of the premolars showed no significant difference between groups. Conclusion: Primary molars with open proximal cavities that are cleaned with toothbrush and toothpaste do not result in displacement and impaction of the successor teeth, neither do primary molars with defective restorations in proximal tooth surfaces.

RESUMEN Objetivo: Identificar los factores asociados a caries severa del primer molar permanente en adolescentes de Acapulco, Guerrero, México. Material y métodos: Estudio transversal en una muestra de 173 adolescentes de primer a tercer grado de una secundaria pública. Se usó un cuestionario autoadministrado que obtuvo datos sociodemográficos, nivel socioeconómico, salud oral, hábitos tóxicos y dietéticos. Se identificó la caries severa con base en los códigos combinado 5-6 del Sistema Internacional de Clasificación y Gestión de Caries. Se realizó un análisis multivariado con el software estadísitico CIETmap, el cual identificó factores asociados a caries severa con la razón de prevalencia y su intervalo de confianza del 95% como estimación de la fuerza de asociación. Resultados: Fueron revisados 692 primeros molares permanentes. El 54% de los adolescentes (94/173) presentó caries severa en al menos un molar. Se encontraron dos factores asociados, consumo de 6 o más cigarros al día (RPa: 7,18, IC95%= 2,07 - 24,22) y el uso de auxiliares en la higiene oral (RPa: 0,41, IC95%= 0,34 - 0,78). Conclusión: La prevalencia de caries severa fue similar a la reportada a otros estudios. Se deben realizar acciones de promoción de la salud que ayuden a disminuir la afección en esta población.


ABSTRACT Objective: To identify the factors associated with severe caries of the first permanent molar among adolescents from Acapulco, Guerrero, Mexico. A self-administered questionnaire was used to obtain sociodemographic data, socioeconomic level, oral health, toxic and dietary habits. Severe caries was identified based on the combined codes 5-6 of the International Caries Classification and Management System. A multivariate analysis was performed with CIETmap statistical software, which identified factors associated with severe caries with the prevalence ratio and its 95% confidence interval as an estimate of the strength of association. Results: A total of 692 permanent first molars were examined. The 54% of the adolescents (94/173) presented severe caries in at least one molar. Two associated factors were found, consumption of 6 or more cigarettes per day (RPa: 7.18, IC95%= 2.07 - 24.22), and use of oral hygiene aids (RPa: 0.41, IC95%= 0.34 - 0.78). Conclusion: The prevalence of severe caries was similar to that reported in other studies. Health promotion actions should be carried out to help reduce caries in this population.

This case report outlines a rare complete intrusion of a permanent, maxillary central incisor (tooth 11) into the nasal cavity, with a late diagnosis and treatment. An eight-year-old boy was referred to an oral and maxillofacial surgery service with absence of tooth 11 after an episode of a fall from his own height. Approximately 50 days after the trauma, the patient presented with a complaint of obstruction of the right nostril. The clinical examination and tomographic evaluation showed that the tooth had been intruded into the nasal cavity on the right side. The tooth in question was removed under general anesthesia by direct approach through the right nostril. The patient was followed up for approximately one year without complications. This case demonstrates the importance of a detailed evaluation during the first examination after intrusive luxation, so the correct diagnosis is made and the correct treatment is performed to avoid greater morbidity and complications for the patient.

Resumo O presente estudo teve como objetivo avaliar o conhecimento dos ortodontistas inscritos na Associação Brasileira de Ortodontia e Ortopedia Facial ­ Seção Minas Gerais (ABOR/MG) sobre a movimentação ortodôntica de dentes traumatizados. A pesquisa foi realizada por meio de um questionário eletrônico desenvolvido na plataforma Google Forms® e enviado via e-mail. O questionário foi baseado no trabalho de Tondelli et al.1 (2010), composto por 21 questões objetivas, o qual foi dividido em 3 partes: a parte 1 abordou os dados demográficos dos participantes; a parte 2 coletou informações a respeito do conhecimento sobre traumatismo dentoalveolar e a parte 3 buscou avaliar o conhecimento dos profissionais em relação à movimentação ortodôntica de dentes traumatizados. A amostra consistiu de 93 (32%) de 289 ortodontistas registrados na ABOR/MG que responderam voluntariamente ao questionário, sendo 52,7% do sexo masculino e 47,3% do sexo feminino. Quanto à formação, a maioria (58,1%) apresentava o curso de Especialização concluído, 34,4% tinham concluído Mestrado ou Doutorado. A maior parte da amostra tinha mais de 10 anos de exercício da Ortodontia (50,5%). Do total, 53,8% afirmaram que praticam exclusivamente Ortodontia em seu consultório e 91,4% responderam que já atenderam pacientes com traumatismo dentário. No entanto, 21,5% afirmaram que apenas às vezes perguntam sobre história prévia de dentes traumatizados. O tipo de trauma com respostas mais assertivas foi avulsão (95,7%), enquanto subluxação, foi a que teve menos respostas adequadas (75,3%). A maioria dos ortodontistas possui um nível de conhecimento considerado satisfatório em relação às classificações de traumas dentários. Porém, a conduta em relação à movimentação ortodôntica destes dentes ainda é inadequada e uma maior capacitação desses profissionais torna-se necessária para levar ao correto tratamento e ao melhor prognóstico do paciente com traumatismo dentário (AU)


Abstract The aim of the present study was to evaluate the knowledge of orthodontists of the Brazilian Orthodontics and Facial Orthopedics Association ­ Minas Gerais section (ABOR/MG) in relation to orthodontic movement of traumatized teeth. The research was conducted with an electronic questionnaire answered through the Google Forms® platform and sent to the participants by e-mail. A number of 21 objective questions, based on Tondelli's et al.1 (2010) work, were divided into 3 parts: the profile data of the professionals composed part 1; part 2 collected information on the knowledge of dentoalveolar trauma; and part 3 evaluated the professional's knowledge in relation to the orthodontic movement of traumatized teeth. The sample of the study was a total of 289 orthodontists registered in the ABOR/MG association: 93 of these answered the questionnaire (32%), in which 51% were men and 49% were women. In terms of professional qualification and education, most participants (58,1%) had Specialization and 53,8% had more than 10 years of experience in the area of Orthodontics. In total, 53,8% declared having practiced exclusively Orthodontics in their clinics and 91,4% have already treated patients with dental trauma. However, 21,5% affirmed that only sometimes they ask the patients about the previous trauma history. The type of trauma with the most correct answers was avulsion (95,7%) and subluxation was the lesion with the lowest number of adequate response (75,3%). Most orthodontics has a level of knowledge in regard to dental trauma considered to be satisfactory. On the other hand, their conduct in orthodontic movement is inadequate and more capacitation and training of these professionals are necessary for the correct treatment and prognosis of the traumatized patient. Objective: Traumatic dental injuries (TDIs) affect the aesthetics and function of primary teeth and can cause sequelae in their permanent successors. This study assessed the distribution of TDIs and sequelae in the primary dentition and the respective permanent dentition in children treated at the Dental Trauma Centre in Brazil. Material and Methods: This cross-sectional study was conducted by collecting data from 256 dental records over 7 years. Descriptive statistical analyses and the Fisher exact test were used (p=0.05). Results: Sixty-two patients presented with trauma in the deciduous teeth. TDIs were observed mostly in boys (64.50%) and in the 2 to 4-year age group (44.10%); most injuries occurred from fall from the child's own height (40.30%) and at home (71.20%). The most common type of trauma was subluxation (22.90%). The most prevalent clinical and radiographic sequelae were mobility (45.60%) and periapical lesion (31.80%), respectively. Regarding the permanent dentition, the most frequent sequela was hypomineralization (33.30%). Conclusion:According to this study, TDIs in deciduous teeth made up 29.24% of all the cases in the Dental Trauma Program. The major type of trauma in children was subluxation, with sequelae in both dentitions. Follow-up after trauma is important to the maintenance of primary dentition and to prevent and treat possible sequelae in the permanent dentition.(AU)


Objetivo: Lesões dentárias traumáticas (TDIs) afetam a estética e a função dos dentes decíduos e podem causar sequelas em sucessores permanentes. Este estudo avaliou a distribuição das TDIs e sequelas na dentição decídua e sua respectiva dentição permanente em crianças atendidas no Centro de Trauma Odontológico no Brasil. Material e Métodos: Trata-se de um estudo transversal realizado por meio da coleta de dados de 256 prontuários odontológicos ao longo de 7 anos. Foram utilizadas análises estatísticas descritivas e teste exato de Fisher (p <0,05). Resultados:Sessenta e dois pacientes apresentaram trauma na dentição decídua. As TDIs foram observadas principalmente em meninos (64,50%) e na faixa etária de 2 a 4 anos (44,10%); a maioria das lesões ocorreu por queda da própria altura (40,30%) e em casa (71,20%). O tipo de trauma mais comum foi a subluxação (22,90%). As sequelas clínicas e radiográficas mais prevalentes foram mobilidade (45,60%) e lesão periapical (31,80%), respectivamente. Em relação à dentição permanente, a sequela mais frequente foi a hipomineralização (33,30%). Conclusão: De acordo com este estudo, as TDIs em dentes decíduos foram de 29,24% de todos os casos do Programa de Trauma Dental. O principal tipo de trauma em crianças foi a subluxação, com sequelas em ambas as dentições. O acompanhamento após o trauma é importante para a manutenção da dentição decídua e para prevenir e tratar possíveis sequelas na dentição permanente. (AU)

Traumatic dental injuries (TDIs) are a public health concern that requires special attention in primary dentition due to the sequel that can be originated in permanent dentition. This paper aims to report a dental trauma injury highlighting the importance of follow-up the traumatized primary tooth until the eruption of its permanent successor. A 3-year-old female patient referred to a Dental Trauma Care Program (DTCP) in a Brazilian Public University after falling from her own height with the involvement of the deciduous maxillary left central incisor. The child's mother did not seek immediate care. The patient did not have any complaint and the tooth presented an enamel fracture during the clinical examination without radiographic alterations. The procedure performed was occlusal wear and insertion of the patient into the DTCP. After a 6-months clinical and radiographic follow-up, the primary maxillary left central incisor presented crown discoloration and periapical bone rarefaction. The treatment was pulpectomy followed by restoration of the tooth. In the 3-year follow-up, prolonged retention occurred and tooth extraction was the treatment of choice. After a 6-year follow-up, the permanent teeth erupted without any sequelae. TDIs are frequent in the primary dentition and, in some circumstances may interfere with the normal development of the permanent tooth causing irreversible sequelae. This case report reinforces the importance of seeking immediate care whenever dental traumas occur. Besides, a long-term follow-up of the traumatized tooth is essential for successful outcomes. Clinical and radiographic monitoring is fundamental to help dental professionals to decide the best treatment and to minimize potential complications.

Introdução: Traumatismos dentoalveolares acometem uma parcela considerável da população e ocorrem por diversas causas, como quedas, acidentes automobilísticos e agressões físicas. As lesões podem variar desde simples fraturas de esmalte até avulsão dental, estando associadas a danos de caráter estético, funcional, psicológico e social. Luxações intrusivas, que por lesionarem ao máximo os tecidos periodontais e pulpares, possuem um dos piores prognósticos, tendo como consequência a necrose pulpar na quase totalidade dos casos com rizogênese completa. Os possíveis tratamentos baseiam-se no desenvolvimento radicular e severidade da lesão, incluindo aguardar re-erupção espontânea, reposicionamento cirúrgico, tracionamento ortodôntico e exodontia do dente intruído. O presente estudo tem por finalidade relatar um caso clínico expondo o tratamento tardio da intrusão traumática grave do incisivo central superior permanente. Relato de caso: paciente do sexo feminino, 10 anos de idade, sofreu traumatismo bucofacial em 2009 e foi atendida na emergência, contudo nenhum atendimento odontológico foi realizado, somente no ano de 2011 procurou atendimento especializado na clínica odontológica da Universidade Estadual de Feira de Santana-BA. Introduction: Dentoalveolar traumas affect a considerable portion of the population and occur due to several causes, such as falls, car accidents and physical aggressions. Lesions can range from simple enamel fractures to dental avulsion, and are associated with aesthetic, functional, psychological and social damage. Intrusive luxation, which, due to maximum damage to the periodontal and pulp tissues, have one of the worst prognoses and result in pulp necrosis in almost all cases of teeth with complete rhizogenesis. Possible treatments are based on root development and severity of lesion and include waiting for spontaneous re eruption, surgical repositioning, orthodontic traction and extraction of intruded tooth. The present study aims to report a clinical case exposing the late treatment of severe traumatic intrusion of the permanent upper central incisor. Case report: a 10-year-old female patient suffered a bucofacial trauma in 2009 and was attended to at an emergency service, however no dental care was provided, only in 2011 she sought specialized care at the dental clinic of the State University of Feira de Santana - BA. Final Considerations: The most appropriate conduct in the face of dentoalveolar traumatisms is urgent care, aiming to minimize sequelae to preserve the remaining tooth and adjacent structures... Introducción: Los traumatismos dentoalveolares afectan a una parte considerable de la población y ocurren por diversas causas, como caídas, accidentes automovilísticos y agresiones físicas. Las lesiones pueden variar desde simples fracturas de esmalte hasta avulsión dentaria, estando asociadas con daños estéticos, funcionales, psicológicos y sociales. Las luxaciones intrusivas, que causan el máximo daño a los tejidos periodontales y pulpares, tienen uno de los peores pronósticos, resultando en necrosis pulpar en casi todos los casos con rizogénesis completa. Los posibles tratamientos se basan en el desarrollo de la raíz y la gravedad de la lesión, incluida la espera de una nueva erupción espontánea, el reposicionamiento quirúrgico, la tracción ortodóncica y la extracción del diente intruido. El presente estudio tiene como objetivo reportar un caso clínico que muestra el tratamiento tardío de la intrusión traumática severa del incisivo central maxilar permanente. OBJECTIVE: To assess the radiographic pattern of inactive enamel caries lesions (IECL) in the occlusal surfaces of permanent molars and to investigate whether the presence of radiolucency at baseline could predict lesion progression over 4-5 years. METHODS: A prospective cohort study followed 193 schoolchildren from southern Brazil. At baseline, clinical and radiographic examinations were performed. After 4-5 years, clinical examination was repeated following the same protocol. Logistic regression models were used to investigate the relationship between the presence of radiolucency at baseline and caries progression. RESULTS: This study included 916 permanent molars, classified as sound occlusal surfaces (n = 434), non-cavitated IECL (n = 438), or cavitated IECL (n = 44) at baseline. The proportion of lesions with radiolucency increased as lesion severity increased (p<0.001, chi-square test). The proportion of cases that progressed was significantly higher among teeth showing radiolucency at baseline than among teeth without radiolucency (p<0.001, chi-square test). The presence of radiolucency at baseline was found to be a predictor of caries progression (activity criteria, adjusted OR=3.37, 95%CI=1.66-6.82, p<0.001; severity criteria, adjusted OR=4.01, 95%CI=1.85-8.72, p<0.001). CONCLUSION: Occlusal surfaces (either sound or with IECL) presenting radiolucency at baseline were more likely to progress over 4-5 years of monitoring and should be monitored more closely. CLINICAL RELEVANCE: Bitewing radiographs, when available, may be used to identify which surfaces/lesions are more likely to progress and thus, to define proper recall intervals for patient monitoring.

AIM: To evaluate the active tactile sensitivity in individuals with complete natural dentition, determining the smallest thickness detected by the participants, and clarifying if there is a difference between the thicknesses analyzed. MATERIALS AND METHODS: Active tactile sensitivity was evaluated in 40 research participants. Inclusion criteria included participants with complete natural dentition, without active or history of periodontal disease, absence of temporomandibular disorders, bruxism, and restorations in the evaluated area. Exclusion criteria included age below 18 years. The active tactile perception threshold was evaluated by using carbon sheets of different thicknesses (0, 12, 24, 40, 80, 100, and 200 µm), which were inserted in the participants' premolars, bilaterally. The carbon sheet was inserted so as not to come into contact with the oral soft tissues. Subsequently, the participant occluded and was asked about the perception of the intraocclusal object 20 times in each occlusal contact. The collected data were tabulated considering the amount of positive and negative responses for each carbon thickness. Values of p < 0.05 were considered significant. RESULTS: The results showed that there was linearity in perception, on both sides, besides, the natural dentition was able to perceive difference in thickness from 12 µm. CONCLUSION: We conclude that the 12 µm thickness is noticeable in occlusion and can be differentiated from other thicknesses in natural dentition and that there is no difference between the tactile sensitivity of the right and left sides. CLINICAL SIGNIFICANCE: A better understanding of active oral tactile sensitivity will contribute to numerous clinical applications in dentistry, including occlusal adjustment in dental rehabilitation, dental implants prosthesis design, and survival of prosthetic rehabilitation.
