BACKGROUND: Multimorbidity frequently occurs in patients with acute heart failure (AHF). The co-occurrence of comorbidities often follows specific patterns. OBJECTIVES: This study investigated multimorbidity subtypes and their associations with clinical outcomes. METHODS: From the prospective RELAX-AHF-2 (Relaxin for the Treatment of Acute Heart Failure-2) trial, 6,545 patients (26% with HF with preserved ejection fraction, defined as LVEF ≥50%) were classified into multimorbidity groups using latent class analysis. The association between subgroups and clinical outcomes was examined. Validation of these findings was conducted in the RELAX-AHF trial, which comprised 1,161 patients. RESULTS: Five distinct multimorbidity groups emerged: 1) diabetes and chronic kidney disease (CKD) (often male, high prevalence of CKD and diabetes mellitus); 2) ischemic (ischemic HF); 3) elderly/atrial fibrillation (AF) (oldest, high prevalence of AF); 4) metabolic (obese, hypertensive, more often HF with preserved ejection fraction); and 5) young (fewest comorbidities). After adjusting for confounders, patients in the diabetes and CKD (HR: 1.80; 95% CI: 1.50-2.20), elderly/AF (HR: 1.42; 95% CI: 1.20-1.70), and metabolic (HR: 1.40; 95% CI: 1.20-1.80) groups had higher rates of the composite outcome than patients in the young group, primarily driven by differences in rehospitalization. Treatment allocation (placebo or serelaxin) modified these associations (Pinteraction <0.001). Serelaxin-treated patients in the young group were associated with a lower risk for all-cause mortality (HR: 0.59; 95% CI: 0.40-0.90). Similarly, patients from the RELAX-AHF trial clustered in 5 multimorbidity groups. The clinical characteristics and associations with outcomes could also be validated. CONCLUSIONS: Comorbidities naturally clustered into 5 mutually exclusive groups in RELAX-AHF-2, showing variations in clinical outcomes. These data emphasize that the specific combination of comorbidities can influence adverse outcomes and treatment responses in patients with AHF.

Vertebral fracture (VF) is one of the most common injuries seen in individuals with osteoporosis, especially in post-menopausal females. There is an increase in bone resorption rate, leading to the destruction of the microarchitecture of bone. A 67-year-old female patient diagnosed with wedge compression fracture of the D12 vertebra, mild compression of the spinal cord, and bilateral foot drop came to a tertiary care hospital, where she underwent spinal fusion at the D11-L1 level and posterior decompression, after which she was referred to physiotherapy, where a patient-tailored treatment protocol was made and implemented over three weeks. Outcome measures like the visual analog scale (VAS), functional independence measure (FIM), and Oswestry's low back disability questionnaire were recorded before and after rehabilitation, and improvement in pain and activities of daily living (ADL) was found. The patient needed mild assistance. There was also improvement in the range and strength of the lower limb muscles. This case report aims to provide a comprehensive treatment protocol for a post-operative spinal fusion and bilateral foot drop patient.

Emerging research indicates that yoga is a promising adjunct to psychological trauma treatment. The current pilot study examined the associations between psychophysiological stress, diaphragmatic breathing (DB), and a trauma-sensitive yoga (TSY) regimen developed specifically for trauma-exposed service members in alignment with recent calls for precision in reporting therapeutic yoga protocols. Participants were 31 service members enrolled in a trauma-focused intensive outpatient program (IOP). Service members participated in a brief diaphragmatic breathing (DB) session using the Breathe2Relax (B2R) app followed by the TSY session. Heart rate (HR) and perceived stress were measured at baseline and after both the DB practice and the TSY session. We assessed Yoga and DB expectancies at baseline and post TSY. Participants also rated the acceptability and usability of the B2R app. Results of linear mixed effects regression models showed decreases in HR and perceived stress, compared to baseline, following DB (HR, b = -8.68, CI 95% = -13.34, -4.02; perceived stress, b = -1.77, CI 95% = -2.35, -1.18) and TSY (HR, b = -12.44, CI 95% = -17.15, -7.73; perceived stress b = -3.69, CI 95% = -4.29, -3.08). Higher levels of expectancies, compared to lower levels, related to stronger decreases in HR and perceived stress, particularly after TSY. Overall, participants rated the B2R usability as high; virtually all participants reported that "most would learn to use the app quickly," and 76.6% reported that they would use it frequently.

AbstractDisentangling different types of selection is a common goal in molecular evolution. Elevated dN/dS ratios (the ratio of nonsynonymous to synonymous substitution rates) in focal lineages are often interpreted as signs of positive selection. Paradoxically, relaxed purifying selection can also result in elevated dN/dS ratios, but tests to distinguish these two causes are seldomly implemented. Here, we reevaluated seven case studies describing elevated dN/dS ratios in animal mitochondrial DNA (mtDNA) and their accompanying hypotheses regarding selection. They included flightless lineages versus flighted lineages in birds, bats, and insects and physiological adaptations in snakes, two groups of electric fishes, and primates. We found that elevated dN/dS ratios were often not caused by the predicted mechanism, and we sometimes found strong support for the opposite mechanism. We discuss reasons why energetic hypotheses may be confounded by other selective forces acting on mtDNA and caution against overinterpreting singular molecular signals, including elevated dN/dS ratios.

Revenue management for passenger rail transportation has a vital role in the profitability of public transportation service providers. This study proposes an intelligent decision support system by integrating dynamic pricing, fleet management, and capacity allocation for passenger rail service providers. Travel demand and price-sale relations are quantified based on the company's historical sales data. A mixed-integer non-linear programming model is presented to maximize the company's profit considering various cost types in a multi-train multi-class multi-fare passenger rail transportation network. Due to market conditions and operational constraints, the model allocates each wagon to the network routes, trainsets, and service classes on any day of the planning horizon. Since the mathematical optimization model cannot be solved time-efficiently, a fix-and-relax heuristic algorithm is applied for large-scale problems. Various real numerical cases expose that the proposed mathematical model has a high potential to improve the total profit compared to the current sales policies of the company. BACKGROUND: Shortly after the first outbreak of COVID-19 in Wuhan, the disease spread rapidly around the world. Previous findings reported an increase in mental health problems among Chinese medical staff, but there was a lack of research following changes in COVID-19 prevention and control policies. METHODS: Medical staff were recruited separately in China from 15 to 16 December 2022 (N = 765, wave 1) and from 5 to 8 January 2023 (N = 690, wave 2). All participants completed the assessments of Generalized Anxiety Disorder-7, the Patient Health Questionnaire-9 and the Euthymia Scale. Network analysis was used to explore the relationships between symptoms both within and across depression, anxiety and euthymia. RESULTS: Medical staff showed worse anxiety, depression and euthymia at wave 2 than at wave 1. Depression, motor, restlessness and uncontrollable worrying showed high centrality (i.e., strength, expected influence, closeness) at wave 1, but higher at peak. Meanwhile, motor symptoms and restlessness showed the strongest connection between different mental disorders at both wave 1 and wave 2. The network structure was stable over time after the relaxation of the infection policy. LIMITATIONS: Our participants were not a random sample and the assessments were based on self-reports. CONCLUSIONS: This study indicated the changes in central and bridging symptoms in medical staff at different stages after lifting of restrictions and the withdrawal of testing requirements, which provided management suggestions for the Chinese government and hospitals, as well as clinical guidance for psychological interventions.

Introduction and Objective: Previously, various stretching techniques were compared to study their effects on the different physiological parameters of hamstring muscles in the elderly population. There is no research that compares the immediate and long-term effects of proprioceptive neuromuscular facilitation-contract-relax (PNF-CR) and static stretching (SS) techniques on knee range of motion (ROM), hamstring flexibility, and knee flexor muscle EMG activity in the elderly. This study intends to compare the same. Methods: A total of 30 males aged 55-75 years were randomly assigned into the PNF-CR group (n = 10), SS group (n = 10), and control group (n = 10). The PNF-CR group received four trials of the contract-relax technique, the SS group received passive stretching of an 80 s duration by the therapist, and the control group received no intervention. A total of 12 sessions were given during the four-week period. Knee range of motion, electromyographic activity of the biceps femoris, and the sit-and-reach test were taken for the dominant side thrice: pre-intervention, immediately after stretching, and after the training period. Results: A statistically significant difference was observed in the maximum voluntary isometric contraction (MVIC) of biceps femoris between the PNF and the control groups (p = 0.01) after four weeks of intervention. The knee ROM and hamstring flexibility for the PNF group showed significant improvement immediately post-test (p = 0.01) and after four weeks of training (p = 0.07 and p = 0.001). SS showed significant results for both ROM and flexibility after four weeks of intervention (p = 0.001), and significant immediate post-test improvements were seen for ROM only (p = 0.007). Conclusions: PNF stretching has an immediate, as well as long-term, effect on knee ROM and hamstring flexibility, whereas it has only a long-term effect on muscle electromyographic activity. SS has an immediate, as well as long-term, effect on knee ROM and only a long-term effect on hamstring flexibility, without any immediate or long-term effects on muscle electromyographic activity.

Stretching is considered a clinically effective way to prevent muscle contracture development in children with spastic cerebral palsy (CP). Therefore, in this study, we assessed the effects of a single session of proprioceptive neuromuscular facilitation (PNF) or static stretching (SS) on ankle joint range of motion (RoM) and gastrocnemius muscle-tendon behavior in children with CP. During the SS (n = 8), the ankle joint was held in maximum dorsiflexion (30 s). During the PNF stretching (n = 10), an isometric contraction (3-5 s) was performed, followed by stretching (~25 s). Ten stretches were applied in total. We collected data via dynamometry, 3D motion capture, 2D ultrasound, and electromyography, before and after the stretching sessions. A mixed ANOVA was used for the statistical analysis. Both ankle RoM and maximum dorsiflexion increased over time (F(1,16) = 7.261, p < 0.05, η² = 0.312; and F(1,16) = 4.900, p < 0.05, η² = 0.234, respectively), without any difference between groups. An interaction effect (F(1,12) = 4.768, p = 0.05, η² = 0.284) was observed for muscle-tendon unit elongation (PNF: -8.8%; SS: +14.6%). These findings suggest a positive acute effect of stretching on ankle function. However, SS acutely increased muscle-tendon unit elongation, while this decreased after PNF stretching, indicating different effects on the spastic muscles. Whether PNF stretching has the potential to cause positive alterations in individuals with CP should be elucidated in future studies.

Numerical stabilization is often used to eliminate (alleviate) the spurious oscillations generally produced by full order models (FOMs) in under-resolved or marginally-resolved simulations of convection-dominated flows. In this article, we investigate the role of numerical stabilization in reduced order models (ROMs) of marginally-resolved, convection-dominated incompressible flows. Specifically, we investigate the FOM-ROM consistency, that is, whether the numerical stabilization is beneficial both at the FOM and the ROM level. As a numerical stabilization strategy, we focus on the evolve-filter-relax (EFR) regularization algorithm, which centers around spatial filtering. To investigate the FOM-ROM consistency, we consider two ROM strategies: (i) the EFR-noEFR, in which the EFR stabilization is used at the FOM level, but not at the ROM level; and (ii) the EFR-EFR, in which the EFR stabilization is used both at the FOM and at the ROM level. We compare the EFR-noEFR with the EFR-EFR in the numerical simulation of a 2D incompressible flow past a circular cylinder in the convection-dominated, marginally-resolved regime. We also perform model reduction with respect to both time and Reynolds number. Our numerical investigation shows that the EFR-EFR is more accurate than the EFR-noEFR, which suggests that FOM-ROM consistency is beneficial in convection-dominated, marginally-resolved flows.

Introducción: La psicoprofilaxis quirúrgica permite poner en funcionamiento mecanismos defensivos capaces de amortiguar el impacto psicoafectivo y emocional que, potencialmente, implica la cirugía. Objetivo: Constatar la efectividad de un programa de psicoprofilaxis quirúrgica basada en la magia en la mejora de diversos indicadores de confort pre y postquirúrgicos. Método: Tras la realización de soporte psicoprofiláctico con un programa de pocos minutos basado en la magia, con grupo control equivalente, se han examinado diferentes aspectos relacionados con el dolor el estrés y la ansiedad pre y postquirúrgica en la infancia durante el ingreso e intervención quirúrgica en una muestra de 52 niños y sus correspondientes padres. Resultados y conclusión: Se han hallado evidencias de una menor presencia de estrés y ansiedad en los niños del grupo experimental frente al grupo control, así como el aumento en la transmisión de emociones positivas con los padres durante su ingreso y transcurso de la cirugía, y una significativa disminución de la ansiedad pre-quirúrgica. Por último, el grupo de psicoporfilaxis basado en la magia presenta, de manera estadísticamente significativa, menos dolor y menos ansiedad postoperatoria que el grupo control. (AU)


Introduction: the surgical psycho-prophylaxis alows us to put into action defensive mechanisms which are able to soften the psychoaffective and emotional impact which, potentially, implies the surgery. Objective: To prove the effectiveness of a surgical prophylactic programme based on the magic and in the improvement of several pre and post surgical comfort indicators. Method: After performing psycho-prophilactic support through a programme of a few minutes based on the magic, with an equivalent control group, we have examined different aspects related to pre and post surgical pain, stress and anxiety in children during their hospitalization and surgical intervention in a sample of 52 children and their parents. Results and conclusion: Evidences of lower levels of stress and anxiety have been observed in the experimental group of children in comparison with those of the control group,together with an improvement in the sharing of positive emotions with their parents during the hospitalization and surgery, and also less pre-surgical anxiety. Finally, the group of children whose prophilaxis is based on the magic shows, in a significant statistical way, less pain and less post-surgical anxiety than the control group. (AU)

To overcome a possible drop in performance following longer stretch durations (>60 s), post-stretching dynamic activities (PSA) can be applied. However, it is not clear if this is true for isolated proprioceptive neuromuscular facilitation (PNF) stretching of different muscle groups (e.g., triceps surae and quadriceps). Thus, 16 participants performed both interventions (triceps surae PNF + PSA; quadriceps PNF + PSA) in random order, separated by 48 h. Jump performance was assessed with a force plate, and tissue stiffness was assessed with a MyotonPro device. While no changes were detected in the countermovement jump performance, the PNF + PSA interventions resulted in a decrease in drop jump performance which led to a large magnitude of change following the triceps surae PNF + PSA and a small-to-medium magnitude of change following the quadriceps PNF + PSA. Moreover, in the triceps surae PNF + PSA intervention, a decrease in Achilles tendon stiffness was seen, while in the quadriceps PNF + PSA intervention, a decrease in the overall quadriceps muscle stiffness was seen. According to our results, we recommend that especially triceps surae stretching is avoided during warm-up (also when PSA is included) when the goal is to optimise explosive or reactive muscle contractions.

BACKGROUND: Since the outbreak of COVID-19 in Wuhan in December 2019, lifestyle has been changing to an online-based learning and working environment rather than on-site, and improvisation training is no exception. However, no research compares the efficacy of online versus on-site training. Although we believed that the most effective way to learn improvisation is an on-site format, it is important to explore how format differences can affect learners. METHOD: We offer three types of training such as on-site training (n = 6) (Consisting of 1 female with age ≥40 and <50, and 5 males with ages ≥20 and <50), hybrid training (Instructor participates from online and learners participate on-site) (n = 120) (Consisting of 55 female with age ≥15 and <20, and 65 males with ages ≥15 and <50), and online training (n = 20) (Consisting of 4 female with age ≥20 and <30, and 16 males with ages ≥20 and <50) We collected pretest, test, and posttest data by using the Kansei Analyzer, a simplified electroencephalograph (EEG) and Profile of Mood States (POMS) questionnaire. RESULTS: All formats of training displayed an increase in vigor and a decrease in depression, confusion, tension, anger, and fatigue. The online training displayed better results than the on-site training. Regardless of the format, all training displayed an increase in stress during the activities and a decrease in stress after the activity without changes in other indexes. Additionally, on-site training displayed an increase in sleepiness and stress during the activities. Some participants were tested twice but no significant differences were found between the initial results and the secondary results. CONCLUSION: In this study, we found evidence that online improvisation can lead to the prevention of depressive symptoms and can function as a method for the reduction of stress in conjunction with the increase of individual vigor. However, a future study is required due to the low number of participants and the absence of POMS data for the on-site training. Any future studies should account for these factors while examining other data such as blood pressure, blood sugar, and pulse.

Digital video forensics plays a vital role in judicial forensics, media reports, e-commerce, finance, and public security. Although many methods have been developed, there is currently no efficient solution to real-life videos with illumination noises and jitter noises. To solve this issue, we propose a detection method that adapts to brightness and jitter for video inter-frame forgery. For videos with severe brightness changes, we relax the brightness constancy constraint and adopt intensity normalization to propose a new optical flow algorithm. For videos with large jitter noises, we introduce motion entropy to detect the jitter and extract the stable feature of texture changes fraction for double-checking. Experimental results show that, compared with previous algorithms, the proposed method is more accurate and robust for videos with significant brightness variance or videos with heavy jitter on public benchmark datasets.

BACKGROUND: Four ohnologous genes (sox1, sox2, sox3, and sox15) were generated by two rounds of whole-genome duplication in a vertebrate ancestor. In eutherian mammals, Sox1, Sox2, and Sox3 participate in central nervous system (CNS) development. Sox15 has a function in skeletal muscle regeneration and has little functional overlap with the other three ohnologs. In contrast, the frog Xenopus laevis and zebrafish orthologs of sox15 as well as sox1-3 function in CNS development. We previously reported that Sox15 is involved in mouse placental development as neofunctionalization, but is pseudogenized in the marsupial opossum. These findings suggest that sox15 might have evolved with divergent gene fates during vertebrate evolution. However, knowledge concerning sox15 in other vertebrate lineages than therian mammals, anuran amphibians, and teleost fish is scarce. Our purpose in this study was to clarify the fate and molecular evolution of sox15 during vertebrate evolution. RESULTS: We searched for sox15 orthologs in all vertebrate classes from agnathans to mammals by significant sequence similarity and synteny analyses using vertebrate genome databases. Interestingly, sox15 was independently pseudogenized at least twice during diversification of the marsupial mammals. Moreover, we observed independent gene loss of sox15 at least twice during reptile evolution in squamates and crocodile-bird diversification. Codon-based phylogenetic tree and selective analyses revealed an increased dN/dS ratio for sox15 compared to the other three ohnologs during jawed vertebrate evolution. CONCLUSIONS: The findings revealed an asymmetric evolution of sox15 among the four ohnologs during vertebrate evolution, which was supported by the increased dN/dS values in cartilaginous fishes, anuran amphibians, and amniotes. The increased dN/dS value of sox15 may have been caused mainly by relaxed selection. Notably, independent pseudogenizations and losses of sox15 were observed during marsupial and reptile evolution, respectively. Both might have been caused by strong relaxed selection. The drastic gene fates of sox15, including neofunctionalization and pseudogenizations/losses during amniote diversification, might be caused by a release from evolutionary constraints.

BACKGROUND: Although the instrument-assisted manual (IM) technique has been widely utilised to improve soft tissue and joint mobility, its therapeutic benefits and underlying neuromechanical mechanisms remain unknown compared to those of conventional static stretching (SS) and hold-relax (HR) manual techniques. OBJECTIVE: This study aimed to compare the effects of the SS, HR, and IM techniques on muscle activity, kinematics, and strength during deep squatting in limited ankle dorsiflexion (DF) syndrome. METHODS: This was a cross-sectional randomised controlled study including 39 adults divided into three groups: SS group: 13, HR group: 13, IM group: 13. Outcome measures were the tibialis anterior (TA): gastrocnemius (GCM) balance ratio and ankle, knee, hip and thoracolumbar junction angles. TA muscle strength was analysed to evaluate the limited ankle DF. Analysis of variance was performed, with P< 0.05. RESULTS: The TA:GCM balance ratio, ranges of motion of ankle DF and knee flexion, and TA muscle strength in the IM group improved significantly compared to that in either the SS group or HR group (P< 0.05). CONCLUSIONS: Our novel results demonstrated that IM was most effective in normalising TA:GCM balance, ankle DF range of motion, and TA muscle strength during deep squatting in adults with limited ankle DF.

Antagonist contract-relax stretching and contract-relax stretching is commonly used in sports practice and rehabilitation settings. To date, no study has compared these modalities regarding muscle stiffness and stretch tolerance. This study aimed to investigate the effects of contract-relax and antagonist contract-relax stretching on dorsiflexion range of motion (ROM), stretch tolerance, and shear elastic modulus. Forty healthy participants (24 men and 16 women) took part in the study. Participants were randomly assigned to perform either contract-relax stretching or antagonist contract-relax stretching for 2 min. Outcomes were assessed on ROM, stretch tolerance, and shear elastic modulus before and after stretching. The ROM and stretch tolerance significantly increased after both contract-relax stretching (+ 5.4 ± 5.8°, p < 0.05; + 3.5 ± 8.0 Nm, p < 0.05) and antagonist contract-relax stretching (+ 6.1 ± 4.9°, p < 0.05; + 4.2 ± 6.4 Nm, p < 0.05); however, no significant difference was found between the two groups. Alternatively, the shear elastic modulus significantly decreased after both contract-relax (-31.1 ± 22.6 kPa, p < 0.05) and antagonist contract-relax stretching (-11.1 ± 22.3 kPa, p < 0.05); however, contract-relax stretching (-41.9 ± 19.6%) was more effective than antagonist contract-relax stretching (-12.5 ± 61.6%). The results of this study suggest that contract-relax stretching instead of antagonist contract-relax stretching should be conducted to decrease muscle stiffness. However, either contract-relax or antagonist contract-relax stretching can increase ROM.

Synthetic aperture radar (SAR) is a widely used remote sensing observation technique. However, SAR raw echo data may be lost during the process of data acquisition by radar platform. In this paper, the imaging problem of SAR echo signal with periodically missing data along the azimuth is analyzed and a novel imaging method is proposed. Firstly, the problem of artificial artifact targets caused by periodically missing data is explained in detail, and the corresponding mathematical model is established. Then, the recovery method based on the RELAX algorithm with periodic notches data is proposed. In addition, when the size of two-dimensional (2D) echo data are large, block restoration along the azimuth is proposed to reduce the amount of calculation. Finally, the advantages of the algorithm proposed in this paper is demonstrated by the points target simulated SAR echo data processing and the real raw SAR echo data processing. When the azimuth periodically missing data rate is 50%, the SAR echo data can be recovered and the well-focused image can be obtained. Comparing the image entropy value and structural similarity index (SSIM) of the focused image, it proves the superiority of the proposed algorithm in solving the imaging problem of SAR azimuth periodically missing data.

OBJECTIVE: To analyze the clinical effect of Zhuang medicine tendon therapy combined with chiropractic manipulation in treating sacroiliac joint dislocation. METHODS: From January 2017 to May 2018, 60 patients with sacroiliac joint dislocation were divided into treatment group and control group according to the order of admission. There were 19 males and 11 females in the treatment group, aged from 23 to 52 (38.97±3.23) years old, with a course of 2 h to 5.1 months, with an average of (2.19±1.12) months. There were 14 males and 16 females in the control group, aged from 26 to 50 (39.07±3.30) years old, with a course of 3 h to 6 months, with an average of(2.41±1.05) months. The treatment group was treated with Zhuang medicine tendon therapy combined with chiropractic manipulation, while the control group was treated with conventional acupuncture and massage. Before treatment, the main clinical symptoms of the patients were lumbosacral pain, posterior superior iliac spine not at the same level and accompanied with dyskinesia. The pelvic separation test and the "4" test were positive. After treatment, the curative effect was evaluated according to the improved Macnab standard and the "score of treatment of lumbar diseases". RESULTS: Sixty patients were followed up for an average of 8 months. At the latest follow-up, the clinical effect of modified Macnab was better in the treatment group than in the control group(P<0.01). After treatment, the treatment group was better than the control group on lumbar function score (P<0.01). CONCLUSIONS: The treatment of sacroiliac joint dislocation by Zhuang medicine tendon therapy combined with chiropractic manipulation has good clinical effect and is worth further application and development.
