PURPOSE: To analyze the upper eyelid contour after Müller's muscle conjunctiva resection (MMCR) performed by four different surgeons. METHODS: Comparative cross-sectional analysis of the pre- and postoperative contours of a control group and four groups of upper lids (n = 88) of 65 patients who underwent MMCR at four international centers. The procedure employed was essentially the same as described by Putterman but performed with different instruments to entrap the posterior lamella. Multiple medial and lateral margin lid distances were measured on Bézier lines expressing the pre- and postoperative lid contours. RESULTS: Preoperatively, two groups had significant lateral and medial ptosis. After MMCR, the lateral segment of the lid's contour was corrected in all groups. In the two groups with more pronounced ptosis, the nasal lid contour was undercorrected. CONCLUSIONS: In MMCR, regardless of the instrument used to entrap the posterior lamella, the amount of medial tissue resection is essential to avoid postoperative nasal undercorrection. RESUMO
PURPOSE: To assess the clinical characteristics of tarsal buckling after ptosis correction and its management with margin rotation techniques. METHODS: Multicenter retrospective review of ten patients who developed upper eyelid entropion following ptosis correction. In all cases the tarsal deformity was corrected with margin rotational procedures with either a lid crease anterior approach or a traditional posterior approach. Data collection included patient demographics, type of ptosis surgery, and photographic documentation of the affected eyelids. RESULTS: Entropion occurred after a variety of different ptosis surgery techniques, including frontalis sling, levator advancement and supramaximal levator resection. A horizontal tarsal fold was detected in all eyelids, being in the upper third of the tarsus in 70% and in the central tarsus in 20% of the cases. Tarsal buckling was corrected in all cases with rotational surgery, with nine cases being operated through an anterior lid crease approach and 1, through the traditional posterior approach. The most reported complication was minimal residual ptosis. CONCLUSION: Tarsal buckling following ptosis surgery is associated with folds located in the upper part of the tarsus. Margin rotation techniques are effective in restoring the natural position of the eyelid margin in these cases.

Objetivo: Describir los resultados funcionales de la blefaroplastia superior. Métodos: Se realizó un estudio descriptivo longitudinal prospectivo de 99 pacientes (198 párpados) con diagnóstico de dermatochalasis de párpado superior, algunos asociados a ptosis palpebral y de la ceja, en el Instituto Cubano de Oftalmología "Ramón Pando Ferrer", en el período comprendido de febrero del año 2019 a enero de 2020. Resultados: El 85,8 por ciento fueron del sexo femenino y el 43,5 por ciento eran mayores de 62 años. Predominó la dermatochalasis del párpado superior y la ptosis palpebral en el 68,4 por ciento, así como la dermatochalasis y la ptosis palpebral severa, las cuales representaron el 47,5 y el 50 por ciento respectivamente. En el grupo de 56 a 61 años sobresalió la dermatochalasis del párpado superior y la ptosis de la ceja, con el 61 por ciento. En el 98 por ciento de los casos intervenidos se obtuvo la corrección total de la dermatochalasis del párpado superior, la corrección de la ptosis en el 86 por ciento y de la ceja en el 88 por ciento. No se presentaron complicaciones en el 94 por ciento de los párpados intervenidos. La complicación más frecuente fue el sangramiento, con el 2,5 por ciento. Conclusiones: El abordaje de la ptosis palpebral y de las cejas transblefaroplastia superior logra en la mayoría de los pacientes la corrección total de estas afecciones y de la dermatochalasis. Este procedimiento constituye una herramienta importante para el rejuvenecimiento facial(AU)
Objective: Describe the functional results of upper blepharoplasty. Methods: A prospective longitudinal descriptive study was conducted of 99 patients (198 eyelids) diagnosed with upper eyelid dermatochalasis, in some cases associated to eyelid and eyebrow ptosis. The study was carried out at Ramón Pando Ferrer Cuban Institute of Ophthalmology from February 2019 to January 2020. Results: Of the total patients studied, 85.8 percent were female and 43.5 percent were aged over 62 years. Upper eyelid dermatochalasis and palpebral ptosis prevailed with 68.4 percent, whereas severe dermatochalasis and palpebral ptosis represented 47.5 percent and 50 percent, respectively. A predominance was found of upper eyelid dermatochalasis and eyebrow ptosis in the 56-61 age group (61 percent). Total correction of upper eyelid dermatochalasis was achieved in 98 percent of the cases intervened, eyelid ptosis correction in 86 percent and eyebrow ptosis correction in 88 percent. No complications appeared in 94 percent of the eyelids intervened. The most common complication was bleeding with 2.5 percent. Conclusions: In most cases, management of palpebral ptosis and upper eyebrow transblepharoplasty achieves total correction of these disorders and dermatochalasis. The procedure is an important tool for facial rejuvenation(AU)

BACKGROUND: Facial care treatments have grown a remarkable demand for effective and minimally invasive techniques with fast recovery time. Plasma technology is a nonsurgical alternative technique for skin rejuvenation. AIMS: We assessed patient satisfaction and symptoms after upper eyelid blepharoplasty with plasma technology. PATIENTS/METHODS: Observational study including 16 patients submitted to upper eyelid blepharoplasty using plasma technology to treat dermatochalasis. Patient satisfaction, symptoms, and quality of life were assessed using 2 questionnaires at follow-up days 7 and 30. Also, the answers were correlated with age, Fitzpatrick skin type, and quantity of eyelid skin treated with plasma. RESULTS: All 16 patients were treated and completed the survey. Fourteen (87.5%) were female, and the mean age was 50.5 years. Physical appearance was the most relevant factor impacting on quality of life at first week postoperative. Regarding satisfaction with results, most patients stated higher level of satisfaction at day 7 follow-up analysis (P = .038). Less impact on quality of life and higher satisfaction was associated with eyelid-treated area (P = .044 and P = .036) and Fitzpatrick skin type (P = .043) at 7 and 30 days after procedure, respectively. Eyelid edema and itching were the symptoms most reported at 7 and 30 days, respectively. CONCLUSIONS: Upper blepharoplasty with plasma is a minimally invasive treatment with low impact on quality of life. However, overall patient satisfaction is questionable when considering less willing of undergoing procedure again and decreased expectation with results over postoperative period. Symptoms are reported mainly at the first week after procedure.

Resumo O carcinoma de glândulas sebáceas (CGS) é a terceira neoplasia maligna mais frequente nas pálpebras. Acomete geralmente mulheres idosas e pode simular blefarite ou calázio. A reconstrução da pálpebra superior é sempre desafiadora. O objetivo deste é relatar um caso inusitado de CGS na pálpebra superior de indivíduo jovem e do sexo masculino. Foi realizada a ressecção total da pálpebra superior direita e o exame histológico revelou margens livres. A reconstrução foi feita com dois procedimentos cirúrgicos em tempos diferentes. Devido ao fato do CGS apresentar alto potencial de metástases, o paciente continua em acompanhamento para reavaliação funcional e estética.
Abstract Sebaceous gland carcinoma (SGC) is the third most frequent malignant neoplasm in the eyelids. It usually affects elderly women and can simulate blepharitis or chalazion. Upper eyelid reconstruction is always challenging. The purpose of this is to report an unusual case of SGC in the upper eyelid of a young and male patient. Total resection of the right upper eyelid was performed and histological examination revealed free margins. The reconstruction was done with two surgical procedures at different times. Due to the fact that SGC presents a high potential for metastasis, the patient continues to be followed for functional and aesthetic reevaluation.

RESUMEN Objetivo: Evaluar los resultados quirúrgicos de la técnica de elevación de la ceja transblefaroplastia y las complicaciones presentadas. Métodos: Se realizó un estudio descriptivo, retrospectivo, de corte longitudinal, de 12 pacientes (24 párpados) con ptosis de la ceja asociada a dermatochalasis del párpado superior desde mayo a septiembre del año 2018, en la consulta de Cirugía Plástica Ocular del Instituto Cubano de Oftalmología "Ramón Pando Ferrer". Fueron excluidos los que ya tenían una cirugía anterior del tercio facial superior. Resultados: El 83,3 por ciento de los pacientes fueron del sexo femenino. En cuanto a los grupos de edades resultó más frecuente el grupo de mayores de 62 años de edad. Hubo un predominio de la raza blanca, para un 58,3 por ciento. En el 60 por ciento de los casos del sexo femenino (6 pacientes), la dermatochalasis del párpado superior, asociada a la ptosis de la ceja, fue moderada. La ptosis de la ceja resultó corregida en 18 párpados, para el 75 por ciento. Se presentó el 25 por ciento de complicaciones (3 pacientes). Conclusiones: La técnica de elevación de la ceja transblefaroplastia logra la corrección de la ceja en el mayor número de pacientes con un bajo porcentaje de complicaciones, y constituye una buena opción quirúrgica para este tipo de afección palpebral(AU)
ABSTRACT Objective: To assess the surgical outcomes of the eyebrow lift technique of transblepharoplasty and the complications presented. Methods: A descriptive, retrospective, longitudinal study was carried out with 12 patients (24 eyelids) with eyebrow ptosis associated with upper eyelid dermatochalasis, from May to September 2018, in the Ocular Plastic Surgery office of Ramón Pando Ferrer Cuban Institute of Ophthalmology. Those who already had a previous surgery of the upper facial third were excluded. Results: 83.3 percent of the patients were female. Regarding the age groups, the group of people over 62 years of age was more frequent. There was a predominance of the white race, accounting for 58.3 percent. In 60 percent of female cases (6 patients), dermatochalasis of the upper eyelid, associated with eyebrow ptosis, was moderate. Eyebrow ptosis was corrected in 18 eyelids, for 75 percent. 25 percent of complications were presented (3 patients). Conclusions: The eyebrow lift technique of transblepharoplasty achieves eyebrow correction in the greatest number of patients with a low percentage of complications, and constitutes a good surgical option for this type of eyelid condition(AU)

RESUMEN Se presentó un caso operado de ptosis palpebral severa del ojo izquierdo, en un paciente de 83 años de edad, chofer profesional. el mismo fue corregido mediante la técnica de Fox, fijando el párpado superior al músculo frontal con una banda de silicona de 1 mm de ancho. Se presentaron los buenos resultados de esta cirugía correctiva en el trans-operatorio y post-operatorio de 7 días y al mes. Se mostraron resultados satisfactorios visual y estético con el uso de la misma. No se reportó reacción adversa a la banda de silicona. Se recomienda extender el uso de esta técnica a todos los servicios de la provincia. Continuar el perfeccionamiento de la misma en el servicio (AU).
ABSTRACT An 83 years old patient, professional driver, operated of a severe ptosis in the left eye, is corrected by Fox procedure, fixating the upper eyelid to the frontal muscle with a silicone band of 1 mm. The good corrective results, during surgery, after that, at 7 days and at a month, are showed as a great achievement in visual and aesthetic fields. There was not allergic reaction to the silicone material. We recommend to extend the use of this procedure to all provinces services and to continue the improvement of the procedure in our service (AU).

Facial lichen planus pigmentosus (LPPig), a rare variant of classic lichen planus, was first described in patients with frontal fibrosing alopecia (FFA) by Dlova [Br J Dermatol 2013; 168: 439-442] in 2013. The diagnosis of facial LPPig is sometimes not easy, since clinical signs and histopathological features may frequently be confused with melasma or postinflammatory hyperpigmentation. We describe a case of a postmenopausal black woman diagnosed with FFA who presented with an identical brown-grayish pigmentation of the face and upper eyelids and typical dermoscopy analysis on both regions. We suggest that the hyperpigmentation of the upper eyelid with typical LLPig dermoscopy (upper eyelid sign) may be a clue for the diagnosis of LPPig and may avoid a scar-causing face biopsy.

Objetivo: Describir la utilidad de la técnica de resección modificada de la aponeurosis. Método: Se realizó un estudio observacional, descriptivo y prospectivo de serie de casos con un solo grupo de control, de 13 pacientes (17 párpados), con ptosis aponeurótica sin desinserción, de febrero a diciembre del año 2017, en el Instituto Cubano de Oftalmología "Ramón Pando Ferrer". Resultados: El 61,5 por ciento fue del sexo masculino y el 30,7 por ciento eran mayores de 60 años. El 30,7 por ciento presentaba ptosis senil y el 47,1 por ciento era severa. La función del músculo elevador resultó buena en el 64,7 por ciento y se logró la corrección quirúrgica en el 88,3 por ciento. Se presentó el 30 por ciento de complicaciones. Conclusiones: La ptosis resulta más frecuente en pacientes mayores de 60 años del sexo masculino, con predominio de la ptosis involutivo-senil, unilateral y severa. La técnica de resección modificada de la aponeurosis resulta efectiva en la mayoría de los casos, en los que se presenta un mínimo de complicaciones. En los pacientes en quienes no se logra la corrección, esto ha estado determinado por una débil inserción de la aponeurosis al tarso, dado por el sangramiento profuso que impide la correcta visualización de los planos quirúrgicos(AU)
Objective: Describe the usefulness of the technique of modified aponeurosis resection. Methods: An observational descriptive prospective case-series study was conducted with a single control group of 13 patients (17 eyelids) with aponeurotic ptosis without disinsertion, from February to December 2017 at "Ramón Pando Ferrer" Cuban Institute of Ophthalmology. Results: 61.5 percent of the study sample were male and 30.7 percent were aged over 60 years. 30.7 percent had senile ptosis, and in 47.1 percent it was severe. Levator muscle function was good in 64.7 percent of the cases, and surgical correction was achieved in 88.3 percent. Complications occurred in 30 percent. Conclusions: Ptosis is more common among male patients aged over 60 years, with a predominance of senile involutional, unilateral and severe ptosis. The technique of modified aponeurosis resection is effective in most cases, with minimum complications. In a number of patients correction is not achieved, due to a weak insertion of the aponeurosis into the tarsus, given the profuse bleeding, which hampers correct visualization of the surgical planes(AU)

Las cejas y el párpado superior están íntimamente relacionados, por lo que se recomienda realizar un plan quirúrgico basado en la evaluación de las cejas y del área periorbitaria para lograr los resultados deseados de una apariencia descansada y natural. Para el levantamiento de las cejas se han realizado diferentes técnicas. Cada técnica tiene sus ventajas, desventajas e indicaciones. Se realizó una búsqueda bibliográfica en bases de datos que ofrece Infomed, como Ebsco, Hinari, Pubmed, Google académico y textos básicos de Oftalmología y Cirugía Estética. La elevación de la ceja mediante la blefaroplastia produce beneficios, tales como un abordaje menos invasivo, cicatrices posteriores menos evidentes, un plano de disección más fácil que disminuye el sangrado transquirúrgico, la disminución del tiempo de cirugía y por consiguiente menores complicaciones posoperatorias. Además, permite que los resultados sean duraderos, al igual que los obtenidos en la cirugía de elevación de las cejas mediante técnicas endoscópicas, con una marcada disminución en los costos al no utilizar el endoscopio. La presente revisión se realizó con el objetivo de ampliar los conocimientos sobre el envejecimiento del tercio superior facial, así como el empleo de la blefaroplastia ampliada como una alternativa quirúrgica de rejuvenecimiento periocular(AU)
The eyebrows and upper eyelids are closely related structures. It is therefore recommended to develop a surgical plan based on evaluation of the eyebrows and the periorbital area in order to achieve the desired result of a rested, natural appearance. Different techniques have been used for eyebrow lifting. Each technique has advantages, disadvantages and indications. A bibliographic search was conducted in the databases offered by Infomed, such as Ebsco, Hinari, Pubmed, Google Scholar, as well as ophthalmology and aesthetic surgery basic textbooks. Eyebrow lift by transblepharoplasty has benefits such as a less invasive approach, less visible scars after surgery, an easier dissection plane reducing intraoperative bleeding, reduced surgery time and therefore fewer postoperative complications. On the other hand, results are as lasting as those obtained by eyebrow lift surgery by endoscopic techniques, but costs are markedly lower due to the non-use of an endoscope. The present review was conducted with the purpose of broadening our knowledge about aging of the facial upper third, as well as about the use of extended blepharoplasty as a surgical alternative for periocular rejuvenation(AU)

El lagoftalmo secundario a daño del nervio facial trae como consecuencia alteraciones funcionales y estéticas que pueden tributar a serias complicaciones corneales y eventualmente a la ceguera. El correcto manejo depende de su severidad y del tiempo de recuperación. El implante de pesas de oro en el párpado superior juega un importante papel en el tratamiento a mediano y largo plazo de esta afección. Se presenta un paciente masculino de 74 años con síntomas y signos de parálisis facial periférica derecha, incluyendo lagoftalmo ipsilateral con gran deterioro corneal. Se le implantó una pesa de oro pretarsal en el párpado superior para corregir el lagoftalmo paralítico y se obtuvieron excelentes resultados funcionales y estéticos. Mientras novedosas modalidades de tratamiento se encuentran en el horizonte con gran potencial para alterar el algoritmo de tratamiento actual, este proceder continúa siendo una técnica confiable, segura y efectiva para la rehabilitación permanente del párpado superior(AU)
Lagophthalmos secondary to facial nerve damage brings about functional and esthetic alterations which may result in serious corneal complications and eventual blindness. Appropriate management depends on severity and recovery time. Gold weight implantation in the upper eyelid plays an important role in the medium- and long-term treatment of this condition. A male 74-year-old patient presents with signs and symptoms of right-side peripheral facial paralysis, including ipsilateral lagophthalmos with great corneal deterioration. A pretarsal gold weight was implanted in the upper eyelid to correct the paralytic lagophthalmos, and excellent functional and esthetic results were obtained. While novel treatment modes lie in the horizon with great potential to alter the algorithm of current treatment, this procedure continues to be a reliable, safe and effective technique for the permanent rehabilitation of the upper eyelid(AU)

Lagophthalmos secondary to facial nerve damage brings about functional and esthetic alterations which may result in serious corneal complications and eventual blindness. Appropriate management depends on severity and recovery time. Gold weight implantation in the upper eyelid plays an important role in the medium- and long-term treatment of this condition. A male 74-year-old patient presents with signs and symptoms of right-side peripheral facial paralysis, including ipsilateral lagophthalmos with great corneal deterioration. A pretarsal gold weight was implanted in the upper eyelid to correct the paralytic lagophthalmos, and excellent functional and esthetic results were obtained. While novel treatment modes lie in the horizon with great potential to alter the algorithm of current treatment, this procedure continues to be a reliable, safe and effective technique for the permanent rehabilitation of the upper eyelid(AU)

BACKGROUND: The study was designed to compare the outcome of full-thickness blepharotomy and transconjunctival eyelid lengthening in the correction of upper eyelid retraction (UER) in patients with Graves' orbitopathy (GO). METHODS: This is a prospective randomized interventional study. Following ophthalmic examination, determination of the ocular surface disease index (OSDI) and photography, 27 patients with UER were randomly assigned to either graded full-thickness blepharotomy (G1) or transconjunctival Müller muscle recession and graded disinsertion of the levator palpebrae superioris muscle (G2). Six months later, patients were reevaluated. Digital images were analyzed with the assistance of customized software. A standardized "normal range" of upper eyelid height and contour was calculated based on healthy controls. The outcome of the two groups was compared. RESULTS: Forty-seven eyelids of 27 patients (19 female) with UER were included. Twenty-seven eyelids (15 patients) were allocated to G1 and 20 eyelids (12 patients) to G2. On average, surgery lasted 37.46 ± 5.73 min in G1 and 32.70 ± 8.39 min in G2. Based on the margin reflex distance, 93% of the eyelids in G1 and 85% in G2 were within the normal range after surgery. The corresponding figures for lid contour were 63 and 55%. Both groups displayed significant improvement in OSDI scores. No significant difference was observed in the overall comparison. CONCLUSIONS: The two surgical techniques were equally effective in the treatment of UER from GO. Postoperative contour outcomes were considerably worse in patients with severe UER than in patients with mild or moderate UER, regardless of group. PURPOSE: To evaluate the efficacy of transconjunctival botulinum toxin type A (BTX-A) in the treatment of upper eyelid retraction in the active inflammatory phase of dysthyroid orbitopathy, establish the ideal dose, and evaluate side effects. METHODS: This is a comparative, prospective study in patients with thyroid orbitopathy, conducted at the Conde Ophthalmology Institute in Valenciana, Mexico. The patients included had dysthyroid orbitopathy in the inflammatory phase, and they were treated with subconjunctival injection of botulinum toxin type A (BTX-A) in the upper eyelid. Five units (group 1) and ten units (group 2) of BTX-A, in a single subconjunctival dose were applied to the non-dominant eye. We evaluated visual acuity, margin-to-reflex distance (RPM1), crease height, ocular motility, diplopia and keratitis, before and after administration of the toxin. The patients were followed at one, 4 and 16 weeks, with the Student t-test as a statistical analysis. RESULTS: At week 4, 15 patients (100%) showed a reduced margin to reflex distance. The mean result for group 1 was -1.75mm (range -1 to -2.5mm) and group 2 was -2mm (range -1 to -4mm). Statistically significant differences were seen between pre-treatment and week 4 in both groups, but no differences between doses. Complete improvement of keratitis and lagophthalmos was observed in 5 and 2 patients, respectively. Visual acuity, ocular motility and crease height did not change in 93% of the patients. One patient (group 1) exhibited complete ptosis and vertical diplopia, which resolved spontaneously at week 6. CONCLUSION: Transconjunctival BTX-A application is safe and effective for the treatment of eyelid retraction in dysthyroid orbitopathy. No difference was found between doses. No severe side effects were reported.

Coloboma palpebral congênito é um raro defeito embriológico que ocasiona falhas no desenvolvimento palpebral. Pode afetar tanto a pálpebra superior quanto a inferior, uni ou bilateralmente. Sua importância clínica reside no fato de que, em sua localização mais comum, a pálpebra superior, tais defeitos provocam exposição da córnea, o que pode levar a ulcerações e, posteriormente, a cegueira. Existem várias técnicas cirúrgicas reparadoras dos colobomas de pálpebra superior, a escolha da técnica dependerá da extensão do defeito. O diagnóstico imediato e sua posterior reparação poderá evitar futuras complicações. O presente trabalho tem como objetivo a descrição de uma técnica cirúrgica simples e eficaz e uma revisão sobre coloboma palpebral.
Congenital eyelid coloboma is a rare embryonic defect that causes failure in eyelid development. It can affect both the upper and the lower eyelid, uni or bilaterally. Its clinical significance lies in the fact that, in their usual location the upper lid , such defects cause exposure of the cornea, which can lead to ulceration and eventually blindness. There are several surgical techniques to repair upper eyelid colobomas, the choice of technique will depend on the extent of the defect. The immediate diagnosis and subsequent repair may prevent further complications. This paper aims at describing a simple and effective surgical technique and a review of eyelid coloboma.
