RESUMO

Tubeless anaesthesia has become widespread in videothoracoscopic surgery, even in major procedures such as lobectomies. There are several advantages in avoiding general anaesthesia and one-lung mechanical ventilation, such as faster recovery and shorter hospital stays. However, hypoxaemia and hypercapnia are the most reported causes of conversion to general anaesthesia. High Flow Oxygen Therapy (HFOT) generates flow-dependent positive end-expiratory pressure, improves oxygenation and also carbon dioxide washout by flow-dependent dead space flushing. For this reason, intraoperative HFOT may reduce the rate of conversion to general anaesthesia. We report our experience with intraoperative HFOT in a 71-year-old female with lung adenocarcinoma undergoing VATS upper left lobectomy.

INTRODUCTION: The number of lung metastases (M1) of colorectal carcinoma (CRC) in relation to the findings of computed tomography (CT) is the object of study. METHODS: Prospective and multicenter study of the Spanish Group for Surgery of CRC lung metastases (GECMP-CCR). The role of CT in the detection of pulmonary M1 is evaluated in 522 patients who underwent a pulmonary metastasectomy for CRC. We define M1/CT as the ratio between metastatic nodules and those found on preoperative CT. Disease-specific survival (DSS), disease-free survival (DFS), and surgical approach were analyzed using the Kaplan-Meier method. RESULTS: 93 patients were performed by video-assisted surgery (VATS) and 429 by thoracotomy. In 90%, the M1/CT ratio was ≤1, with no differences between VATS and thoracotomy (94.1% vs 89.7%, p=0.874). In the remaining 10% there were more M1s than those predicted by CT (M1/CT>1), with no differences between approaches (8.6% vs 10%, p=0.874). 51 patients with M1/CT>1, showed a lower median DSS (35.4 months vs 55.8; p=0.002) and DFS (14.2 months vs 29.3; p=0.025) compared to 470 with M1/CT≤1. No differences were observed in DSS and DFS according to VATS or thoracotomy. CONCLUSIONS: Our study shows equivalent oncological results in the resection of M1 of CRC using VATS or thoracotomy approach. The group of patients with an M1/CT ratio >1 have a worse DSS and DFS, which may mean a more advanced disease than predicted preoperatively.

Introducción: El número de metástasis pulmonares (M1) de carcinoma colorrectal (CCR) en relación con los hallazgos de la tomografía computarizada (TC), es objeto de estudio. Métodos: Estudio prospectivo y multicéntrico del Grupo Español de Cirugía de las metástasis pulmonares del CCR (GCMP-CCR). Se evalúa el papel de la TC en la detección de M1 pulmonares en 522 pacientes intervenidos de una metastasectomía pulmonar por CCR. Definimos como M1/CT al cociente entre los nódulos metastásicos y los hallados en la TC preoperatoria. Se analizó la supervivencia específica de enfermedad (SEE), la supervivencia libre de enfermedad (SLE) y el abordaje quirúrgico mediante el método de Kaplan-Meier. Resultados: En 93 pacientes se utilizó la cirugía videoasistida (VATS) y 429 toracotomías. En un 90% el cociente M1/TC fue ≤1, sin diferencias entre VATS y toracotomía (94,1 vs. 89,7%; p=0,874). En el 10% restante existían más M1 que las predichas por la TC (M1/CT>1), sin diferencias entre abordajes (8,6 vs. 10%; p=0,874). Cincuenta y un pacientes con M1/CT>1, mostraron una menor mediana de SEE (35,4 vs. 55,8 meses; p=0,002) y SLE (14,2 vs. 29,3 meses; p=0,025) en comparación con 470 con M1/CT≤1. No se observaron diferencias en la SEE y la SLE según VATS o toracotomía. Conclusiones Nuestro estudio muestra unos resultados oncológicos equivalentes en la resección de M1 de CCR mediante abordaje VATS o toracotomía. El grupo de pacientes con un cociente M1/CT>1 presentan una peor SEE y SLE, pudiendo significar una enfermedad más avanzada de la predicha preoperatoriamente(AU)

Introducción: A pesar del creciente uso de la lobectomía por videotoracoscopía, este abordaje resulta aún controvertido en algunos aspectos. El objetivo del presente trabajo es comparar los resultados quirúrgicos de la lobectomía pulmonar por videotoracoscopía y por toracotomía. Material y métodos: En el Hospital Universitario Fundación Favaloro, entre los años 2008 y 2017, fueron intervenidos quirúrgicamente 153 pacientes (n = 89 masculinos). Resultados: Se realizó lobectomía por videotoracoscopía a 39/89 pacientes. Los pacientes intervenidos por videotoracoscopía y con diagnóstico de cáncer de pulmón, tenían predominantemente tumores en estadío I (25/39 pacientes). La incidencia de complicaciones postoperatorias y mortalidad fueron similares en ambos grupos. En el grupo intervenido por videotoracoscopía la permanencia del drenaje pleural fue significativamente menor (4 versus 3 días, p = 0.004) y, también resultó más breve la estadía hospitalaria (6 versus 4 días de internación, p = 0.03). El número de estaciones ganglionares exploradas durante la cirugía fue mayor en los pacientes operados por toracotomía (4 versus 3.5 estaciones ganglionares, p < 0.001). Conclusión: La lobectomía por videotoracoscopía resultó al menos equivalente a la cirugía por toracotomía en términos de complicaciones postoperatorias. Los principales beneficios del abordaje por videotoracoscopía fueron una menor permanencia del drenaje pleural y menor estadía hospitalaria. Se recomienda tener especial cuidado con el muestreo ganglionar cuando la cirugía se realiza por videotoracoscopía.


Introduction: Despite the widespread adoption of video-assisted thoracoscopic lobectomy, this surgical approach still remains controversial. The objective of this paper is to compare the surgical results of video-assisted thoracoscopic lobectomy to open thoracotomy lobectomy, performed at the Hospital Universitario Fundación Favaloro between 2008 and 2017. Materials and Methods: One-hundred and fifty three patients (n = 89 males) had surgery during the study period; 39/153 had video-assisted thoracoscopic lobectomy. Results: Patients that had video-assisted thoracoscopic lobectomy were more likely to have stage I lung cancer (25/39). Postoperative complications and mortality rates were similar in both groups. Video-assisted thoracoscopic lobectomy patients had a shorter chest tube drainage time (4 vs. 3 days, p = 0.004) and length of hospital stay (6 vs. 4 days of stay, p = 0.03). The median number of lymph node stations sampled at surgery was significantly higher in the patients that had open thoracotomy lobectomy (4 vs. 3.5 lymph node stations, p < 0.001). Conclusion: Video-assisted thoracoscopic lobectomy is at least equivalent to open thoracotomy lobectomy in terms of postoperative complications and mortality. The main benefits of the video-assisted thoracoscopic approach are a shorter chest tube drainage time and length of hospital stay. Special attention should be taken at the time of nodal sampling when the surgery is performed by the video-assisted thoracoscopic approach.

INTRODUCTION: Although the Nuss technique revolutionized the surgical treatment of pectus excavatum, its use has not become widespread in our country. The aim of this study was to analyze the current use of this technique in a sample of Thoracic Surgery Departments in Spain. METHODS: Observational rectrospective multicentric study analyzing the main epidemiological aspects and clinical results of ten years experience using the Nuss technique. RESULTS: Between 2001 and 2010 a total of 149 patients were operated on (mean age 21.2 years), 74% male. Initial aesthetic results were excellent or good in 93.2%, mild in 4.1% and bad in 2.7%. After initial surgery there were complications in 45 patients (30.6%). The most frequent were wound seroma, bar displacement, stabilizer break, pneumothorax, haemothorax, wound infection, pneumonia, pericarditis and cardiac tamponade that required urgent bar removal. Postoperative pain appeared in all patients. In 3 cases (2%) it was so intense that it required bar removal. After a mean follow-up of 39.2 months, bar removal had been performed in 72 patients (49%), being difficult in 5 cases (7%). After a 1.6 year follow-up period good results persisted in 145 patients (98.7%). CONCLUSION: Nuss technique in adults has had good results in Spanish Thoracic Surgery Departments, however its use has not been generalized. The risk of complications must be taken into account and its indication must be properly evaluated. The possibility of previous conservative treatment is being analyzed in several departments at present.

Introducción: El objetivo de este estudio fue determinar las distintas etiologías delos nódulos pulmonares solitarios (NPS) resecados quirúrgicamente; establecer la rentabilidad diagnóstica y la tasa de complicaciones de la resección videotoracoscópica (VATS) del NPS.Métodos: Se evaluaron 483 pacientes operados de NPS en el Hospital Británico de Buenos Aires entre el período 1987-2008. Los criterios de inclusión para NPS fueron: lesión intrapulmonar menor de 3 cm., rodeada de parénquima sano, no asociada aatelectasia, neumonía y/o adenomegalias.Resultados: El examen patológico de los nódulos resecados mostró 342 (70.8%) lesiones malignas y 141(29.2%) lesiones benignas. Entre las lesiones benignas los granulomas (48.2%) y hamartomas (22.7%) resultaron ser las principales causas. Los NPS malignos se observaron en pacientes de mayor edad y su tamaño fue mayor (p=<0.001). Sin embargo, no fueron diferentes en cuanto antecedente de tabaquismo (p=0.981) y de neoplasia previa (p= 0.918). La cirugía torácica videoasistida (VATS) se realizó en 238 pacientes (49.3%). La conversión a toracotomía se realizó en 7 casos (8.3%) de los nódulos benignos siendo los motivos principales: la imposibilidad de localizar la lesión ó la necesidad de ampliación de la resección. Se presentaron complicaciones vinculadas a la cirugía videotoracoscópica (VATS) en 8 casos (3.36%). Conclusión: La VATS es un procedimiento de muy baja morbimortalidad en la resección de nódulos benignos. La edad del paciente y el tamaño del nódulo pulmonar resultaron ser factores confiables para predecir la histología maligna.
