RESUMO

【Objective】 To investigate the situation of whole blood collection in Tianjin after COVID-19 prevention and control measures were fully lifted. 【Methods】 The relevant data on whole blood collection of voluntary blood donors in Tianjin 15 days before Spring Festival (2023.01.07-2023.01.21, when China has managed COVID-19 with measures against Class B infectious disease instead of Class A infectious diseases) and 15 days before Spring Festival in 2018 (2018.02.01- 2018.02.15) and 2019 (2019.01.21-2019.02.04) before the breakout of COVID-19 were retrospectively collected and compared. 【Results】 The comparison between the above period in 2023, 2018 and 2019 was as follows: the number of blood donors was 6 124 vs 3 940 vs 4 069; blood collection volume (U) was 9 623 vs 7 378 vs 7 808; the proportion of first-time blood donors, local blood donors and group blood donors was 69.17% (4 236/6 124) vs 65.86% (2 595/3 940) vs 62.05% (2 525/4 069), 59.31% (3 632/6 124) vs 23.27% (9170) vs 18.19% (740/4 069) and 43.42% (2 659/6 124) vs 8.05% (317/2 595) vs 0.15% (6/4 069) (all P<0.05). 【Conclusion】 The adjustment of COVID-19 prevention and control policy has a significant impact on voluntary blood donation, and the corresponding adjustment of blood donor recruitment strategy in blood centers should be conducted to increase the whole blood collection.

【Objective】 To establish a paper-free system of the total whole blood donation flow in constructing intelligent blood stations, and build digitalized whole blood donation system for practice. 【Methods】 A paper-free whole blood collection system was constructed through information process reforming, system frame designing and data network transportation constructing, and was applied in various blood donation scenario. 【Results】 Fixed blood collection sites carried out 49 063 donations via paper-free information system from November 2022 to July 2023, and 24 822 donations( group blood donation) were conducted via paper-free system from April to July 2022. Compared with the traditional paper-based model, paper-free system is safer, more standardized and more convenient, effectively enhancing the experience of blood donors. 【Conclusion】 The construction of paper-free whole blood collection system effectively enhances the experience of blood donors, improves the safety, accuracy, traceability of the data, and has good social value and economic value, which is worth popularizing.

BACKGROUND: Acute normovolemic hemodilution is recommended as a technique to reduce allogeneic red blood cell (RBC) transfusions in cardiac surgery, but its efficacy to reduce non-RBC transfusion has not been consistently demonstrated. We hypothesized that intraoperative large-volume autologous whole blood (AWB) collection and reinfusion improves viscoelastic coagulation parameters. STUDY DESIGN AND METHODS: Prospective observational study of cardiac surgery patients at the University of Maryland Medical Center between December 2017 and August 2019. Rotational thromboelastometry parameters were compared between AWB and control groups (n = 25 in each group) at three time points: T1, baseline; T2, on cardiopulmonary bypass (CPB) after the cross-clamp removal; and T3, 30-60 minutes after protamine administration. The study's primary outcomes were whole blood viscoelastic coagulation parameters that included EXTEM clotting time (CT), FIBTEM amplitude at 10 minutes, and EXTEM amplitude at 10 minutes (EXTEM-A10 ). Chest tube drainage and allogeneic transfusion were secondary outcomes. RESULTS: Reinfusion of AWB after CPB resulted in a significantly shorter EXTEM CT; mean difference, -11.4 seconds (-21.4 to -1.4; P = .03). It also resulted in a greater percentage increase in EXTEM A10 from T2 to T3; mean difference, 7.8% (95% CI, 1.1%-14.5%; P = .02). Statistical significance was not found in 24-hour chest tube drainage. CONCLUSION: Large-volume AWB collection and reinfusion are feasible in selected cardiac surgical patients, and may be associated with prohemostatic effects according to thromboelastometry, warranting further investigation with a prospective randomized study.

【Objective】 To explore the factors affecting the off-site vasovagal reactions in whole blood donors, aimed at providing references for formulating interventions. 【Methods】 The off-site vasovagal reactions in whole blood donors in 2019 was investigated in terms of gender, age, frequency of blood donation, weight and season were classified and analyzed. 【Results】 Among the 42 874 donors who donated 400 mL of whole blood, 67 had non-severe off-site vasovagal reaction, accounting for 0.16%, and none developed severe reaction. Single-factor and multi-factor correlation analysis showed that the age (18~25 years old) and weight (< 55 Kg) were the main influencing factors of off-site vasovagal reaction, while gender, blood donation frequency and season showed no statistical significance. 【Conclusion】 We should pay close attention to the factors predicting the off-site vasovagal reaction and formulate corresponding interventions to reduce the reaction rates, so as to improve the quality of blood donation service, and ensure the safety of blood donors.

【Objective】 To explore the whole blood collectionstatusat street fixed blood donation sites in different time periods and regions, among different populations with different recruitment methods in Chengdu. 【Methods】 The whole blood donation data at street fixed blood donation sites in Chengdu from 2014 to 2018 were collected, and then statistically analyzed in terms of weekdays/ weekends/holidays, the urban/suburban counties, the first/ repeated blood donation and local/ non-local household registration.On weekdays, blood donors were recruited at fixed blood donation sites while on weekends/holidays they were recruited via on-site recruitment combined with "computer-network-mobile phone" mode. 【Results】 There are 36 fixed blood donation sites in Chengdu, among which 22 were in suburban counties and 14 in urban area.There were more blood drives on weekdays than weekends / holidays every year, with fewer average daily collection on weekdays in comparison with weekends/holidays(P<0.05). The collection units on weekends/holidays were high in January, July, August and December, medium in March, April, June, September and October, and low in February, May and November (P<0.05). There were more blood drives in urban area than suburban counties on weekends/holidays, with fewer average daily collection in urban areain comparison with suburban counties. From 2014 to 2018, the total collection units at street fixed blood donation sites in suburban counties increased year by year (P<0.05). The proportion of first-time blood donors in urban area (59.62%) was significantly higher than that in suburban counties (43.45%) (P<0.05). The proportion of non-local registered blood donors in urban area (53.39%) was significantly higher than that in suburban counties (20.39%)(P<0.05). The number of text messages we sent for recruitment on weekends and holidays and the number of blood donors who responded increased year by year (P<0.05). 【Conclusion】 Whole blood collection at street fixed blood donation sites in Chengdu suburban counties on weekends and holidays was enhanced by using information interactive mode as "computer-network-mobile phone", which allows blood station managers to schedule blood collection in different periods and formulate reasonable blood collection plan.
