AIM: To develop a comprehensive classification system of distinctive clinical and anatomical features of congenital microphthalmia and anophthalmia in children and to specify indications, contraindications, and optimal timing of the primary and subsequent prosthetic treatment. MATERIAL AND METHODS: A total of 70 patients with congenital micro- or anophthalmia aged from 1 month to 12 years were examined. Besides the routine ophthalmic examination, all patients underwent eye and orbit ultrasound (axial length measurement and B-scan), computed tomography of the orbits and skull, and immunological tests for infectious diseases (enzyme-linked immunosorbent assays). RESULTS: Basing on the examination RESULTS: we have determined the common types of congenital micro- and anophthalmia in children. We have also developed a stepwise prosthetic treatment aimed at better cosmetic rehabilitation. Indications and contraindications for the use of ocular prostheses in children with congenital micro- and anophthalmia have been identified. CONCLUSION: The proposed method of stepwise prosthetics is the principal option for conservative rehabilitation of children with congenital micro- or anophthalmia.

This literature review considers the treatment of an anophthalmic socket and the controversial aspects of the implants used to restore the lost volume after enucleation or evisceration, including the different materials employed and the main problems encountered during anophthalmic socket reconstruction. Since the 1980s, when integrated implants were proposed, there has been much controversy about what is the best implant for restoring the lost volume in an anophthalmic socket: integrated or non-integrated implants. Thus, we present this literature review to provide guidance to doctors and consumers.

RESUMOEsta revisão da literatura abrange o tratamento da cavidade anoftálmica, assim como os controversos aspectos relacionados com os implantes de cavidade utilizados para recompor o volume perdido após enucleações ou eviscerações, tais como os diferentes materiais utilizados e os principais problemas enfrentados na reconstrução da cavidade anoftálmica. Depois dos anos 80 do século passado, quando foram introduzidos os implantes integrados, há muitas controvérsias sobre qual seria o melhor implante para recompor o volume perdido na cavidade anoftálmica: implante integrado ou não integrado? Desta forma, apresentamos esta revisão da literatura, que procura melhor orientar médicos e consumidores.

AIM OF THE STUDY: From a light asymmetry to a sunken eye aspect, a great disparity between the results after anophtalmic socket rehabilitation is noticeable: what are the factors involved in the degree of residual enophtalmos following excision of the eye? The litterature's response is based on physiopathological considerations around intraorbital architectural disturbance. We propose a geometrical approach related to the existence of different morphological types of orbit. PATIENTS AND METHOD: Eighty-six records of eviscerated and enucleated patients have been studied and submitted to a statistical analysis. A preliminary study has defined four types of orbit depending on the shape and operture of the orbital "window": two opposite types IA and III, a type II intermediate and a particular one, the type IB. A classification of enophtalmos' degree allows to analyze the parameters chosen and to identify the predictive factors. RESULTS: The statistical analysis confirms the incidence of the orbital morphology on the degree of enophtalmos but do not support the theories based on the intraorbital septal architecture changes. Depending on the orbital shape and the container-content relation, the volume loss is more visible on the whole orbitopalpebral surface of opened and high orbit but remains centered on the anteroposterior position of the implant of a closed and lengthened orbit. At the contrary to the type III, the type IA is not favorable for the anophtalmic patient and predispose to a higher degree of enophtalmos. This new approach has therapeutic implications on primary and secondary surgery for volume loss replacement. CONCLUSION: The success of anophtalmic socket rehabilitation is influenced by the orbital morphological type that has to be considered in the therapeutic strategy.

Realizou-se um estudo retrospectivo, durante um período de 22 anos, em 380 pacientes com cavidades anoftálmicas sem implantes, tendo sido encontrado um total de 83 casos com entrópio. Säo apresentados e analisados a etiologia, os tipos de cavidade, a localizaçäo do entrópio, as técnicas cirúrgicas utilizadas e os resultados. Concluímos que o entrópio é uma deformidade habitualmente presente nessas cavidades, e que deveria ser incluído no quadro clínico que compöe a Síndrome da Cavidade Anoftálmica

The authors analyze 8 cases of congenital anophthalmia (CA). In all the cases this condition has been a component of multiple congenital developmental abnormalities; in 4 cases the defect has been nosologically diagnosed. Developmental defects of the central nervous system have been revealed in all the cases. Histologic studies have made it possible to single out, with due consideration for the normal embryogenesis of the eye, the following variants of CA: (1) complete anophthalmia, (2) partial anophthalmia--(a) purely neuroectodermal, (b) mixed mesodermal/ectodermal, (3) anophthalmia with cysts, (4) anophthalmia with intracranial synophthalmus. The possible mechanisms of teratogenesis acting in every case of CA are discussed.

Review of 50 histopathologic cases and a number of clinical cases of congenital retinal anomalies has permitted classification under the following headings: 1) Coloboma-orbital cyst--"anophthalmos" group due to aberrant closure of the embryonic fissure; 2) Retinal fold-central stalk-detachment group comprising a series that varies from simple retinal folds to total retinal detachment and anomalous stalk formation. Cases of the 13-15 trisomy syndrome constitute a special subgroup in this rubric; 3) Retrolental fibroplasia, due to hyperoxia of premature infants, is manifest by "dragged" disks and gliovascular proliferation with occasional detachment; 4) Persistent hyaloid system is occasionally associated with mild anomalies of the retina; 5) Massive gliosis of the retina is usually a hamartomatous manifestation; 6) Congenital absence of ganglion cells occurs with cerebral maldevelopment and 7) Congenital absence of the photoreceptors is the congenital form of retinitis pigmentosa.
