BACKGROUND: Hidradenitis is a disorder where abscesses appear after the infection of the apocrine sweat glands. It is located normally in the axillae, groin, perineal region, and the scalp. CLINICAL CASE: A 37 year old male was referred by his GP to the General Surgery Department with axillary hidradenitis which had evolved over the years. The physical examination shows signs of hidradenitis in both axillae, with a noticeable suppurative hidradenitis in the right armpit. En bloc extirpation was performed to remove the whole affected area. The pathological examination revealed a cutaneous leishmaniasis. Subsequently, fucidin was administered topically, as well as local infiltrations of one millilitre of Glucantime™. DISCUSSION: Hidradenitis normally appears in intertriginous areas and its manifestation is accompanied by recurrent subcutaneous nodules. The incidence rate in females is three times higher than in males. The isolated Hidradenitis caused by Leishmania is a rare condition presented only in endemic areas or in immunocompromised patients, such as HIV-infected patients. Clinical manifestations can be different and the diagnosis can be confirmed through haematoxylin-eosin. The main pattern displays a disorganised granuloma without necrosis. Systemic or topical treatment can be applied. Immunotherapy treatment is the most common. CONCLUSIONS: Hidradenitis caused by Leishmania in HIV-negative patients is a rare condition. Therefore it is important to perform a good histological diagnosis and to administer the right treatment.

RESUMO

Lymphatic filariasis is a neglected tropical disease caused by the parasite Wuchereria bancrofti. Involvement of the breast is relatively uncommon and may affect only 10% of individuals with lymphatic filariasis. We present a case of an elderly woman with breast pathology. The condition clinically appeared as a malignancy. Routine diagnostic tests including tissue histopathological studies failed to clinch a diagnosis. However, a serological test helped in identifying the pathology, which turned out to be a manifestation of breast filariasis, and aided in instituting appropriate therapy for this condition.

RESUMO

Echinococcosis (hydatid disease) is a zoonosis caused by the larval stage of Echinococcus granulosus (or Taenia echinococcus). The adult form of the parasite lives in the gut of the dog, while the intermediate hosts, where the tapeworm develops to larval stage are cats, cattle, pigs and humans(considered to be accidental intermediate hosts). The parasite has a worldwide distribution, but the endemic areas are Canada and Alaska, Australia, New Zealand, South America and the Mediterranean region. Hydatid cyst can grow many years before the symptoms and clinical signs appear. The liver and the lungs are the most affected organs, but primary location of the hydatid disease in the axilla is extremely rare. In our country we did not find any records of axillary hydatid disease, while the literature contains only 12 cases of axillary location. We present the case of a woman, 60 years old, with a primary axillary location of hydatid cyst, who underwent a total cystectomy.

RESUMO

Echinococcosis is a zoonotic infection, affecting humans in a characteristic geographic distribution. The disease generally involves the liver, the lung and sometimes an axillary location. We reported a case of 43-year-old man, who was diagnosed with a cystic lesion of the left axilla, which was presumed to be a hydatid cyst or a vascular malformation by ultrasound and computed tomography. Surgical excision of the lesion was performed confirming the hydatid cyst diagnosis by histological studies.

OBJECTIVE: The aim of this work was to report a case of hydatid cyst located in the axilla. CLINICAL PRESENTATION AND INTERVENTION: A 36-year-old female presented with a painless right axillar swelling of 6 months duration. We removed the entire cyst surgically, and macroscopic and histopathological examinations confirmed the diagnosis of axillar hydatid disease. Seventeen months after the operation there was no recurrence. CONCLUSION: This case report showed that in cases of a cystic mass in an endemic zone, the diagnosis of hydatid cyst should be considered.

INTRODUCTION: Human hydatid disease is a parasitic infection caused by the larval form of Echinococcus granulosus. It has worldwide distribution and is endemic in many countries, especially the Mediterranean region. It most commonly affects the liver and lungs although multi-organ involvement has been observed in 20-30% of patients. CASE REPORT: A 45-year-old woman presented to a gynaecologist because of a mass in the axillary region. Her mother and her two sisters were undergoing treatment for breast cancer. In her examination, a hard, semi-mobile, painless mass was found that was approximately 3 cm in diameter. Axillary ultrasonography showed lymphadenopathy. No abnormality was found in mammographic examination of either breast, or in abdominal ultrasonography and chest X-ray. Occult breast cancer was suspected but when the mass was excised for pathological examination the biopsy showed a hydatid cyst with germinative membranes. Subsequent lung, abdomen and brain tomography scans, whole body bone scintigraphy and hydatid serology, including indirect haemagglutination and enzyme-linked immunosorbent assay, were negative. For these reasons an isolated axillary hydatid cyst was diagnosed.  CONCLUSION: Parasitic cysts should be considered in endemic areas in patients presenting with a soft tissue mass in the axillary region. Imaging methods should be planned to include this possibility.

Human subcutaneous dirofilariasis is a rare infection caused by filarial worms of the genus Dirofilaria. The parasites are transmitted to man by zooanthropophilic bloodsucking insects and the infection is manifested as subcutaneous nodules. Excisional biopsy is both diagnostic and therapeutic. We herein report three cases of human subcutaneous dirofilariasis diagnosed, over a period of ten years (1991-2000), in North East Greece. The patients, two women and one man, were all adults between 32 and 67 years of age. In two cases the infection was manifested as breast nodules, while in the third case as a painful nodule in the axillary region. The excisional biopsy showed the presence of an adult Dirofilaria, identified as Dirofilaria (Nochtiella) repens. It is emphasised that both clinicians and pathologists should have an increased awareness of this clinicopathologic entity and include dirofilariasis in the differential diagnosis in patients presenting with breast or subcutaneous nodules.
