RESUMO

The aims of this article are to present a case demonstrating the connection between palatoradicular grooves and inflammatory collateral cysts and to discuss the related nomenclature. Radiographs in a 21-year-old man revealed a radiolucent, unilocular, well-defined area near the vital maxillary right lateral incisor and canine. Palatal swelling was present, and a 6-mm-deep periodontal pocket was found at the palatal surface of the right lateral incisor. The differential diagnoses were keratocystic odontogenic tumor, developmental lateral periodontal cyst, and inflammatory lateral periodontal cyst. The area was explored surgically, and the lesion was excised. Surgical exploration revealed a palatoradicular groove, which was scaled and planed with the aid of manual curettes with the intention of creating a flat surface to promote insertion of the periodontal fibers. Histopathologic analysis revealed that the lesion was an inflammatory cyst. The presence of a palatoradicular groove can put the periodontium at risk because a resulting lack of fiber insertion makes oral hygiene difficult. This established inflammatory process can initiate development of an inflammatory collateral cyst that may be misdiagnosed, hindering successful management. In this case, bone grafting and placement of a resorbable membrane were used to promote bone formation and subsequent sealing of the periodontal space.

BACKGROUND: Cytokines were thought to play an important role for the expansion of odontogenic cysts. The purpose of this study was to evaluate the cytokine and chemokine levels of radicular and residual cyst fluids. METHODS: Cyst fluids were aspirated from 21 patients (11 radicular and 10 residual cysts) and the levels of interleukin-1 alpha (IL-1alpha), tumor necrosis factor-alpha (TNF-alpha), monocyte chemotactic protein-1 (MCP-1), and regulated upon activation normal T cell expressed and secreted (RANTES) were determined by ELISA using commercially available kits. RESULTS: Both radicular and residual cyst fluids contained IL-1alpha, TNF-alpha, MCP-1, and RANTES, concentrations of which were significantly higher in the radicular cyst fluids than those in the residual cysts (P < 0.001 for IL-1alpha, TNF-alpha, and RANTES; P < 0.01 for MCP-1). Compared to the other mediators, the concentration of IL-1alpha was found to be highest in both of the cyst fluids. In addition, positive correlations were found between IL-1alpha, TNF-alpha, MCP-1, and RANTES in radicular and residual cyst fluids. CONCLUSION: If the radicular cyst is inadvertently left behind following tooth extraction, some degree of inflammation may carry on. Residual cysts, although to a lesser extend than radicular cysts, have the potential to expand.

There have been few case reports of cystic lesions occurring after gingival grafting. There are three reports of this type of sequela after connective tissue grafting, but this is the first known case of a cystlike lesion developing secondarily to a free gingival graft procedure. The bulky tissue, which had developed under the previously grafted area, was properly excised under local anesthesia. The small specimen removed was sent for histologic analysis. A new gingival graft was performed immediately after the lesion enucleation, as the cystic lesion had dislocated the earlier graft. The surgical wound healed uneventfully, and no recurrence was seen 18 months later The microscopic sections showed a cystic cavity lined with an orthokeratinized, hyperplastic, stratified squamous epithelium covered with fibrous connective tissue. The development of a cystlike lesion following a free gingival graft is, to date, an unpublished event. The fact that most cystic lesions appear in the mandibular lateral incisor-canine-first premolar area deserves further consideration.

Embora muitas já estarem bem definidas, algumas delas continuam gerando opiniões divergentes, como o Cisto Periodontal Lateral (CPL). Pela raridade da doença, o objetivo deste artigo é relatar um caso de cisto periodontal lateral com características não usuais, fazendo considerações quanto a etiologia, seus aspectos clínicos, radiográficos, histológicos, e o seu tratamento, enfocados a luz da literatura recente

RESUMO

O cisto odontogênico botrióide (COB) é uma lesão incomum dos ossos maxilares esporadicamente relatada na literatura, considerada uma variante multicística do Cisto Periodontal Lateral em virtude de sua apresentação microscópica. Apesar da maioria dos relatos de COB descreverem-no como uma imagem radiolúcida multilocular, alguns autores têm destacado que esta lesão também pode apresentar-se como uma imagem unilocular, assemelhando-se, assim, a várias outras entidades odontogênicas císticas ou neoplásicas. O objetivo deste artigo é relatar um caso de COB localizado na região posterior esquerda da mandíbula de uma mulher de 53 anos cuja apresentação radiográfica simulava um cisto periapical residual. As características clínicas, radiográficas e histopatológicas da lesão, bem como a abordagem terapêutica escolhida, são descritos, ao passo que algumas considerações acerca da histogênese, diagnóstico diferencial e comportamento biológico dessa entidade são tecidas e discutidas

We present the clinical, radiographic and histological findings of 15 paradental cysts. These were the second most common type of cyst associated with mandibular third molars, comprising 25% of all lesions at this site. Ten patients were aged 30 years or younger and 13 gave a history of pericoronitis. There was a close relationship between the site of the cyst and the angle of the impacted tooth; the cyst lay mesially to two mesioangularly impacted teeth, buccal to two vertically impacted teeth and distal/distobuccal to the remainder, all but one of which were distoangular in impaction. The pericoronal follicular space was preserved in nine cases, a useful diagnostic sign. Most cysts were lined with epithelium that resembled that seen in radicular cysts, although the lining of three cysts resembled that of unicystic ameloblastoma, a potential diagnostic pitfall. We propose that the relation between location of cyst and angle of impaction points to a role for food impaction in the pathogenesis of paradental cysts.

BACKGROUND: Many techniques have been developed to obtain root coverage. There have been few complications reported with these techniques. This case report chronicles the development of a cyst-like area after a connective tissue partial-thickness double pedicle graft was used to obtain root coverage. METHODS: The root coverage procedure was performed as previously reported. Postoperatively, in the surgical area, the patient developed occasional swelling. It was possible to express a white material from the area. A biopsy was taken to determine the cause of the clinical findings. RESULTS: Histological evaluation revealed the development of a cyst-like area. CONCLUSION: The development of cysts does not seem to be a common event. However, this case report demonstrates that they are a possible complication of root coverage procedures.

OBJECTIVE: The aim of the study was to evaluate the clinicopathologic features of 54 paradental cysts. METHOD AND MATERIALS: The cases were retrieved from the files of the Department of Oral Pathology of the University of São Paulo, in Brazil. Only case files that included complete data, radiographs, and clinical history were used for the study. RESULTS: Most of the cysts were adjacent to a partially erupted mandibular third molar; only two cysts were adjacent to mandibular second molars, and one cyst was adjacent to a first molar. Women were affected more often than were men (33:21), and most of the patients were in their second or third decades. In most cases, the affected tooth had a history of one or more episodes of pericoronitis. Histologically, these cysts presented the typical features of an inflammatory cyst, having a lining epithelium, which apparently resulted from a union between reduced enamel epithelium and oral epithelium. CONCLUSION: Development of the paradental cyst could be avoided if episodes of pericoronitis were properly treated, because the cyst is usually related to pericoronitis.

The lateral periodontal cyst (LPC) is a relatively uncommon but widely recognized odontogenic cyst of developmental origin. It is found mostly in adults and has no sex predilection. LPC is usually discovered during routine radiographic examination, is located mainly between the roots of vital mandibular canines and premolars, and seldom causes pain or other clinical symptoms. The defect appears on radiographs as a round or teardrop-shaped, well circumscribed radiolucency. Due to its location it can easily be misdiagnosed as a lesion of endodontic origin. In this paper, two cases of lateral periodontal cysts are presented. In the first case, the patient (woman, 62 years old) complained of an asymptomatic gingival swelling in the region between the right maxillary canine and premolar. The radiographic examination revealed a well circumscribed radiolucency with a radiopaque margin between the roots of the canine and premolar. The adjacent teeth had vital pulp. Surgical enucleation of the lesion was performed and the histological examination revealed that the lesion was a "lateral periodontal cyst of developmental origin". In the second case, the patient (women, 44 years old) complained of a swelling in the area of tooth 32. During radiographic examination a well circumscribed radiolucency between the roots of the lateral incisor and the canine was discovered. Surgical enucleation of the lesion was performed and the histological examination revealed that the lesion was a "lateral periodontal cyst of developmental origin".

Cystic change can occur in association with partially and unerupted lower third molars. This case report describes an inflammatory paradental cyst which developed over a relatively short period of time and presented with acute symptoms.

The number and variety of cysts that occur in the jaws are greater than those in any other part of the body. Consequently, dental practitioners must keep up with advances in this field, including the definition of new entities, such as the glandular odontogenic cyst; the deletion of the old incorrect concept of maxillofacial fissural cysts, including the globulo-maxillary cyst; and the advances in the molecular biology of established cysts, such as the dentigerous cyst and the odontogenic keratocyst.

A retrospective study is reported of 14 cases of gingival cyst of adults. Clinical data were pooled with well-documented cases in the literature. Most cases occurred in the 5th and 6th decades and in the premolar-canine-incisor area. Microscopically, the epithelial lining ranged from a thin, flat to cuboidal layer, 1-3 cells thick, resembling reduced enamel epithelium, to a nonkeratinized stratified squamous epithelium without rete ridges, of about 5-6 layers. Epithelial cells were frequently atrophic and this could involve the full thickness of the epithelium leaving an amorphous band of eosinophilic material. Plaque-like epithelial thickenings are sometimes present. Multicystic or "botryoid" forms may occur. The cysts are of developmental origin, arising either from basal cell extensions of overlying epithelium; or from postfunctional odontogenic epithelial residues of the dental lamina or rests of Malassez; or from junctional epithelium in the gingival crevice. The latter theory of origin is favored.

RESUMO

A case is described in which two separate developmental odontogenic cysts were associated with an unerupted lower third molar tooth. Radiological and histological examination showed that these were a lateral periodontal cyst and a follicular (dentigerous) cyst. This unusual occurrence provides substantial evidence that the periodontal cyst may have an origin from the cell rests of Malassez.
