RESUMO

Background: To evaluate hyperbaric oxygen therapy (HBOT) on infection rates and repair rates during the treatment of large jaw cysts. Methods: A prospective randomized, non-blinded, controlled clinical trial included 90 patients with jaw cysts, randomly divided into three groups. Patients were treated with enucleations and bone substitute was used in the experimental and control groups. The experimental group received HBOT. The primary predictor variable was HBOT. The infection rate, repair rate, preoperative volume of the jaw cysts, age, and sex were statistically analyzed. The Fisher exact test was used to compare the infection rate and postoperative complications. The repair rate of the bone defects was analyzed using the repeated-measures analysis of variance and the least significant difference tests. The Kendall's coefficient of concordance and Kappa statistics were calculated to evaluate the consistency between the two investigators. Results: The infection rate was 3.4% in the experimental group, 14.3% in the blank group, and 32.1% in the control group (P<0.05). The repair rate in the experimental group was significantly higher than in the control and blank groups at 1, 3 and 6 months after surgery (P<0.05). Conclusion: The results showed that HBOT reduced the postoperative infection rate following the enucleation of large jaw cysts with bone substitute filling, and it also improved the bone repair rate.

Los quistes son cavidades patológicas que adquieren importancia en el ámbito odontológico debido a la gran frecuencia en la que se encuentran en los maxilares y por el rol que el odontólogo presenta en su detección. Dentro de las lesiones tipo quísticas, mencionamos al quiste apical en bolsa donde existe una comunicación entre este y el conducto radicular, cuya curación puede lograrse a través de la terapia endodóntica. En el presente caso clínico se demostró cómo la pasta de hidróxido de calcio colocada en forma temporaria y en sucesivas sesiones en el interior de los conductos de los cuatro incisivos inferiores afectados por un gran proceso quístico, permitió la resolución paulatina hasta la erradicación completa del mismo. Se realizaron sucesivos controles clínicos y radiográficos mediante Rx periapicales, ortopantomografías y tomografías Cone Beam; dichos controles indicaron el momento oportuno de repetir el procedimiento de acuerdo a los resultados obtenidos en la disminución de las dimensiones del proceso quístico, optando por la terapia endodóntica definitiva a los 9 meses. Se destaca la eficacia de este tratamiento conservador, mínimamente invasivo, ante la presencia de ungran proceso quístico, con la finalidad de evitar las técnicas quirúrgicas, en pacientes comprometidos a las citas periódicas necesarias para lograr su erradicación.


The cysts are pathological cavities that acquire importance in the dental field due to the great frequency in which they are found in the jaws and the role that the dentist presents in their detection. Withincysts lesions cyst dental root in pocket can be mentioned, where there is a communication between itand the root canal, whose cure can be achieved through root canal therapy or endodontic therapy. In the present clinical case it was demonstrated how the calcium hydroxide paste placed temporarily andin successive sessions inside root canal of the four lower incisors affected by a large cystic process,allowed the gradual resolution until the complete eradication of it. Successive clinical and radiographic controls were carried out using periapical radiographs, orthopantomographies and Cone Beamtomography, these controls indicated the opportune moment to repeat the procedure according to theresults obtained in the reduction of the dimensions of the cystic process, opting for definitive endodontic therapy at nine months. The effectiveness of this conservative treatment is minimally invasive in the presence of a large cystic process, in order to avoid surgical techniques in patients committed to necessary periodic appointments to achieve eradication.

RESUMO

Aglossia is a rare congenital disorder with complete absence of tongue that can also be associated with limb deformities, syndromes and aberrant positioning of the visceral organs. The present case report describes multidisciplinary rehabilitation in a patient with Aglossia Congenita along with dextrocardia, situs inversus, mutilated dentition with a lack of occlusal table and compromised esthetics.

CONTEXT: The platelet rich fibrin (PRF) is second generation platelet concentrate that has been widely used and researched for stimulation and acceleration of soft tissue and osseous healing. Its continuous delivery of growth factors and proteins mimic the need of physiological wound healing and regenerative tissue processes. AIMS AND OBJECTIVES: The aim of this study was to evaluate the efficacy of PRF in osseous regeneration after enucleation of cystic lesions. The objectives of this study were: (1) To evaluate osseous regeneration radiographically with the use of PRF in intrabony defects after cystic enucleation. (2) To evaluate the degree of bone density in intrabony defects with the use of PRF postoperatively after 1 st , 3 rd , and 6 th months. SUBJECTS AND METHODS: 10 cases of cystic lesions were treated using PRF after cystic enucleation. Follow-up radiographs (orthopantomogram) were taken 1 st , 3 rd , and 6 th months postoperatively. Bone density was measured with grayscale histogram using Adobe Photoshop 7.0 software. RESULTS: The subsequent follow-up examinations revealed progressive, predictable, and significant radiographic osseous regeneration. CONCLUSION: The use of PRF in management of cystic lesions seems to be a novel therapeutic approach promoting faster osseous regeneration within 6 months postoperatively however further study is required with larger sample size and with a control group.

RESUMO

A description is given of the bioactive glass-ceramic material, BAS-O, and of clinical tests of this material in stomatology. Granules of BAS-O bioactive glass-ceramic were used for implantation into cyst cavities in jaw bones. Radiological examination was carried out after 3, 6 and 12 months. A total of 37 radiographs of 23 patients were recorded and evaluated. Symptoms of ossification were found in 28 cases (75.7%), while five radiographs (13.5%) did not show visible bone apposition and loss of the BAS-O granules from the operation site was found in four cases (10.8%). The authors have demonstrated the excellent healing properties and tolerance for the BAS-O material, and no significant changes were observed in biochemical and haematological examinations.

RESUMO

The main features of the nevoid basal cell syndrome, also called Gorlin-Goltz-syndrome, are basal cell nevi, jaw cysts, anomalies of the vertebras and ribs, and intracerebral calcifications. The diagnosis is often incidentally made by discovery of bilateral jaw cysts, as it was in the case of our patient and her family. The therapy is the complete enucleation of the cyst. The cavity is filled with homologous cancellous bone graft. Recurrence and little cysts are treated with the solution of Carnoy eliminating remaining microcysts.

RESUMO

Between 1977 and 1987 a total of 31 patients with keratocysts were treated at the maxillofacial surgery department of the University of Cologne. In two patient groups with 11 major mandibular cysts each, two different approaches were used: mandibular resection in one, and enucleation following previous fixation of the cyst epithelium with Carnoy's solution in the other group. The fact that only one recurrence was observed in the second group, gives us reason to recommend this safe and, for the patient, unstressful procedure.

RESUMO

The good properties of bovine bone collagen type 1 for filling of bony defects was demonstrated in 56 cysts of the alveolar bone in 52 patients. 49 cysts (approx. 87%) healed without irritation. 8 months postop. complete reossification of the defects had been achieved in 20 (approx. 36%) of the cysts. Further 32 defects (approx. 57%) were markedly reduced. The discussion will compare the advantages of bone collagen and other materials.

Cysts in jaws are generally excised by Partsch II when they are small. When they are large, they are excised by Partsch I to form a paranasal sinus in an oral cavity, or by an operation based on Caldwell-Luc's method to form it from nostrils, as Partsch II forms dead space after the operation and the cyst increases by the pressure of intracystic fluid. However, with Partsch I, the bone needs many days to regenerate, and since a surgical ciliated cyst of the maxilla is often formed after an operation of the maxillary sinus, Partsch I is not necessarily a good method. Therefore, in order to control cyst inflammation, the irrigation of cyst cavities was repeated, as it is held that prostaglandin-like (PG-like) substances are generated in the cyst walls when a cyst is inflamed by bacterial infection; prostaglandin E2 (PGE2) especially acts on the resorption of bone and enlarges the cyst. As a result of repeated irrigations, infection of the cyst cavities disappeared simultaneously with decreasing the pressure of intracystic cavities, the values of thiobarbituric acid, Prostaglandin E, Prostaglandin F2 alpha (TBA, PGE, PGF2 alpha), and the free radical intensity at g = 2.003 decreased. The bone regenerated around the cyst walls, and finally the cyst diminished in size. In the case of the viscous cyst fluids, a solution containing hyaluronidase enabled complete disappearance. It would be better to apply Partsch II after the cyst has become smaller through irrigation. After the irrigational method, irregular formation and tendency toward malignancy could be found in none of the cyst wall cells.
