RESUMO

In 2012, the Japanese Society for Dialysis Therapy (JSDT) established the order of correction of P, corrected Ca (cCa), and whole PTH (w-PTH) in the treatment of Chronic Kidney Disease-Metabolic Bone Disorder (CKD-MBD) as P-first. However, there is no report that analyzes whether this rule is in line with reality and what the adequate rate of P is. Therefore, we analyzed the test values of our 48 patients during the year of 2019 and examined the validity of the results. The results showed that the adequate range rates were 70.8% for P, 100% for cCa, and 89.6% for w-PTH. This result is better than the JSDT Web-based Analysis of Dialysis Data Archives (WADDA) P adequacy rate of 66.2%. Although the guideline is P-first, it is often the case that we cannot reach the adequate level; therefore, healthcare professionals and patients often blame each other. We believe that this is due to the mismatch between the modern era of processed foods covered with P additives and treatment methods (P intake restriction and P-binders). The development of processed foods with P additives has brought light and darkness to mankind. The light side is freedom from starvation, and the dark side is a new condition caused by P burden: P burden disease including CKD-MBD.

Phosphorus trichloride and oxychloride are aggressive materials, widely used in the process industries. On escape to the atmosphere they create toxic clouds that may cause serious damage to people and to the environment. When spilled onto the ground they create liquid pools that can boil, evaporate or even solidify. The main feature of the pool behaviour is the exothermic reaction of these chemicals with water, which is complicated and depends heavily on the amount of water available for reaction, and as result of which the pool has changing composition and properties. The purpose of this paper is to describe the dangers involved in cases of accidental releases of phosphorus trichloride and oxychloride, to report their properties, referring to toxicity data and major accidents. The spill behaviour of phosphorus trichloride and oxychloride has been incorporated into REACTPOOL [R.F. Kapias, C. Griffiths, J. Haz. Mater.]. Model results indicate that the pool behaviour is strongly affected by the amount of water available for reaction. Surface roughness and wind speed, also have a strong effect on the results. Although there are no experimental data for model validation, it is shown that REACTPOOL gives useful insights into the behaviour of such spills. The paper concludes with a discussion comparing the behaviour for several water reactive chemicals to which REACTPOOL has been applied.

The results of patch testing with a series containing components of cigarettes and matches were reviewed. 2 years were reviewed, 1987 and 1997. 314 patients were patch tested to this series, 203 in 1987 and 111 in 1997. 31 patients had clinically relevant positive reactions to the series, 25 in 1987 and 6 in 1997. 26 patients had relevant positive tests to cigarette components in the series. There were 14 relevant positive reactions to phosphorus sesquisulfide in 1987 and one in 1997. All patients with relevant positive reactions to red match tips also had reactions to phosphorus sesquisulfide. There was a significant association between cigarette and fragrance hypersensitivity. The eczema in 3 of 4 patients who stopped smoking improved. A series containing match heads, smoked cigarette filters and remnants of tobacco from smoked cigarettes may be useful in smokers with eczema of the face, neck or hands.

Patch testing was performed with phosphorus sesquisulfide P4S3 in 2 groups containing equal numbers of patients using different concentrations (0.5% P4S3 in pet. and 1% P4S3 in pet., the usual suggested test concentration as recommended by the International Contact Dermatitis Research Group). We found that there was a statistically significant increase in the number of clinically irrelevant irritant reactions in the group tested with the concentration (chi 2 = 16, p < 0.0004). We recommend that patch testing with phosphorous sesquisulfide should be at a reduced concentration of 0.5% pet.
