RESUMO

Radial pulse diagnosis is the most common method to examine the human health state in Traditional East Asian Medicine (TEAM). A cold stress-related suboptimal health state (subhealth) is often undetectable during routine medical examinations, however, it can be detected through the palpation of specific pulse waves, particularly a 'tight pulse', in TEAM. Therefore, this study examined a correlation between 'tight pulse' and vascular changes in the radial artery (RA) induced by a cold pressor trial (CPT). Twenty healthy subjects underwent sequentially control trial and CPT with room-temperature and ice-cold water, respectively, on the right forearm. The radial pulse and vascular changes were then examined on the left arm. The radial pulse scores for frequencies of 'tight pulse' with strong arterial tension increased after the CPT compared with the control trial. The pulse scores were reversely correlated with the RA thickness and volumes in ultrasonography, but not with changes in the systolic/diastolic blood pressure. The RA thickness-based vascular surface and three-dimensional images visualized a 'tight pulse' showing the vasoconstriction and bumpy-/rope-shaped vascular changes in the radial pulse diagnostic region after the CPT. These findings provide valuable insights into the potential integration of clinical radial pulse diagnosis with ultrasonography for cold-related subhealth.

Transcurrimos un tiempo en el que están cambiando muchos paradigmas, con lo que se abre la perspectiva de un universo holístico e interconectado. Estos paradigmas están transformando también la medicina, proporcionando la comprensión y el respeto entre diferentes filosofías y sus correspondientes prácticas. Los niños son el futuro y la esperanza para el mundo; la Pediatría es la disciplina más importante para el cuidado de su salud, desde la concepción hasta el final de la adolescencia. La Pediatría debe ocuparse no solo de los individuos y sus organismos, sino también de influencias ambientales, culturales y sociales, que tienen impacto considerable sobre la salud física, emocional y mental de los niños y sus familias.

Owing to the diversity of pulse-wave morphology, pulse-based diagnosis is difficult, especially pulse-wave-pattern classification (PWPC). A powerful method for PWPC is a convolutional neural network (CNN). It outperforms conventional methods in pattern classification due to extracting informative abstraction and features. For previous PWPC criteria, the relationship between pulse and disease types is not clear. In order to improve the clinical practicability, there is a need for a CNN model to find the one-to-one correspondence between pulse pattern and disease categories. In this study, five cardiovascular diseases (CVD) and complications were extracted from medical records as classification criteria to build pulse data set 1. Four physiological parameters closely related to the selected diseases were also extracted as classification criteria to build data set 2. An optimized CNN model with stronger feature extraction capability for pulse signals was proposed, which achieved PWPC with 95% accuracy in data set 1 and 89% accuracy in data set 2. It demonstrated that pulse waves are the result of multiple physiological parameters. There are limitations when using a single physiological parameter to characterise the overall pulse pattern. The proposed CNN model can achieve high accuracy of PWPC while using CVD and complication categories as classification criteria.

BACKGROUND: Pulse diagnosis researches acquiring pulse waves from the wrist radial artery has not yet addressed the issue of whether this information is affected by differences in the hemodynamic characteristics of pressure waves derived from different locations. This study aimed to clarify whether the blood dynamic states are identical with regard to the "three positions and nine indicators" ((Equation is included in full-text article.)) listed in traditional Chinese medicine (TCM). METHODS: A total of 37 participants of CAD group and 20 participants of healthy group were recruited, and pressure pulse waves were measured at 18 locations on both hands. A multivariate analysis (MANOVA) was performed with a "randomized block design" using SPSS 22.0 and R 3.4.1 to examine the time-domain parameters that represented certain hemodynamic characteristics. RESULTS: In CAD group, the results showed significant differences (p < 0.05) among the h1, h2, h3, h1/t, and h3/h1 measurements of the pulse waves using different indicators at the same position; the h1, h2, h3, and h1/t measurements of the pulse waves at different positions using the indicator "Superficial"; and the h1, h2, h3, h1/t, and h3/h1 measurements of the pulse waves at different positions using the indicator "Medium". In healthy group, the results showed significant differences (p < 0.05) among the h1, h2, h3, and h1/t measurements of the pulse waves using different indicators at the same position; the h1, h2, and h1/t measurements of the pulse waves at different positions using the same indicator. CONCLUSION: Because of the differences in the hemodynamic characteristics among the different positions and indicators, the article might provide a new opinion for future pulse diagnosis investigations to carefully consider the measurement location to ensure the completeness of the information.

RESUMO

We performed a comparative analysis of the types of spectral density typical of various parameters of pulse signal. The experimental material was obtained during the examination of school age children with various psychosomatic disorders. We also performed a typological analysis of the spectral density functions corresponding to the time series of different parameters of a single oscillation of pulse signals; the results of their comparative analysis are presented. We determined the most significant spectral components for two disordersin children: arterial hypertension and mitral valve prolapse.

Pulse diagnosis, recognized as an important branch of traditional Chinese medicine (TCM), has a long history for health diagnosis. Certain features in the pulse are known to be related with the physiological status, which have been identified as biomarkers. In recent years, an electronic equipment is designed to obtain the valuable information inside pulse. Single-point pulse acquisition platform has the benefit of low cost and flexibility, but is time consuming in operation and not standardized in pulse location. The pulse system with a single-type sensor is easy to implement, but is limited in extracting sufficient pulse information. This paper proposes a novel system with optimal design that is special for pulse diagnosis. We combine a pressure sensor with a photoelectric sensor array to make a multichannel sensor fusion structure. Then, the optimal pulse signal processing methods and sensor fusion strategy are introduced for the feature extraction. Finally, the developed optimal pulse system and methods are tested on pulse database acquired from the healthy subjects and the patients known to be afflicted with diabetes. The experimental results indicate that the classification accuracy is increased significantly under the optimal design and also demonstrate that the developed pulse system with multichannel sensors fusion is more effective than the previous pulse acquisition platforms.
