RESUMO

El divertículo de Meckel complicado es poco frecuente en el adulto. Presentamos un paciente de 19 años de edad, con dolor abdominal de 11 horas de evolución, encon­ trando al realizar la laparotomía exploradora un divertículo de Meckel gigante, necrosa­ do y volvulado. Las presentaciones más frecuentes del divertículo de Meckel son la obstrucción intestinal y la diverticulitis, esta última da una sintomatología similar a la apendicitis aguda, por lo que al realizar la exploración quirúrgica y encontrar el apéndi­ ce cecal normal, se debe visualizar los últimos 180 cm de íleo terminal.


Meckel's diverticulum is rare in adults. We present a 19­year­old patient with abdominal pain of 11 hours of evolution, finding a giant Meckel diverticulum, necrotic and volvula­ ted, when performing the exploratory laparotomy. The most frequent presentations of Meckel's diverticulum are intestinal obstruction and diverticulitis, the latter being a symptomatology similar to acute appendicitis, surgical surgery and the normal cecal ap­ pendix, the last 180 cm of terminal ileum should be visualiz

RESUMO

Johann Friedrich Meckel (1781-1833) was a 19th century anatomist born into an eminent dynasty. He was a professor of anatomy, pathology, and zoology at the University of Halle, in Central Germany. The diverticulum, a congenital remnant of the vitellointestinal duct was named after him. Other eponyms include Meckel's cartilage, Meckel syndrome, and Meckel-Serres law of recapitulation. His concepts in comparative anatomy, embryology, and teratology anticipated Darwin. This review is a short tribute to this legend and his prolific contributions. LEVEL: Historic review, level V.

RESUMO

The use of eponyms in medicine has a long and captivating tradition, which applies to pediatric surgery as well. Unfortunately, even though these eponyms are widely used, oftentimes the fascinating personalities and lives behind these names remain unrecognized or underappreciated, especially among residents, fellows and younger-generation pediatric surgeons. Therefore, in this article, we review 15 names that are frequently used as eponyms in modern pediatric surgery and hereby enlighten the personalities behind them. Given their particular frequency, we choose the following eponyms: Ramstedt pylorotomy, Nissen fundoplication, Murphy's sign and sequence, McBurney's sign and incision, Meckel's diverticulum, Kasai portoenterostomy, Ladd's procedure, Morgagni and Bochdalek hernia, Ravitch operation, Nuss procedure, Hirschsprung disease, Swenson pullthrough, Peña procedure and Wilms tumor. A detailed description of the historical importance of these personalities and their contribution to our field is given. Without the appropriate historical background, it is difficult for the current younger and next generation pediatric surgeons to grasp the full spectrum of the ongoing progress in our field. Therefore, our article conveys not only important insight into the past, but also provides young surgeons with an important historical perspective essential to understand the current and future developments in modern pediatric surgery.

In 1700, the French surgeon Alexandre de Littré described for the first time a new form of inguinal hernia. This hernia varied from the known forms of hernias in its clinical course and in the postmortem examination results performed by Littrè himself. The characteristic feature of this hernia was the fact that the entire circumference of the bowel wall was not part of the hernial sac, but only the antimesenteric part of the intestinal wall. The underlying pathomechanism was explained 100 years later by Meckel. In a scientific paper about hernias some years earlier, Richter described the intestinal wall hernia, and this initiated the confusing use of the clinical entity known as the Richter-Littré hernia in Germany. In this case report we describe the historic development of this entity.

RESUMO

We have described a patient with strangulation of a Meckel's diverticulum in a femoral hernia. In the case of Littre's hernia, true diagnosis before surgical intervention is rare if not impossible. It is therefore important that the surgeon be well versed in the management of such cases, as well as in distinguishing a Littre's hernia from a Richter's hernia during the intraoperative period.
