Infection of the larval form (cysticerco) of Taenia in any tissue or organ is known as the disease cysticercosis. Many sites of infection have been documented but the central nervous system has been the most common. It present a case report of a 19 years old patient with a subcutaneous cysticercosis confirmed with biopsy.

La infección por la forma larvaria (cisticerco) de Taenia solium en cualquier tejido u órgano se conoce como cisticercosis. Existen numerosos reportes de casos, siendo la mayoría de ellos cisticercos en sistema nervioso central. El compromiso de otros órganos es raramente detectado. Se presenta el caso de una mujer de 19 años con una cisticercosis subcutánea que fue confirmada con biopsia.

RESUMO

Subcutaneous dirofilariasis is a parasitic infestation found in endemic areas in Mediterranean countries such as Italy. It is occasionally reported in India, mostly from the state of Kerala. Presentation in an infant is extremely rare. We report a case of subcutaneous dirofilariasis in a child that was diagnosed by ultrasonography and confirmed by surgery.

RESUMO

Hydatid disease is an endemic zoonosis caused by the cestode Echinococcus granulosus. The most common affected organs are the liver and the lungs. Echinococcal cyst located in the subcutaneous tissue constitutes an extremely rare manifestation of the hydatid disease. Here we present a case report with a cystic mass in the subcutaneous tissue of the left gluteal area, diagnosed as being a hydatid cyst.

We present a study of 36 cases of solitary soft tissue cysticercosis retrieved from pathological reports and medical records during 1990-2006. The soft tissue was the most common organ affected by cysticercosis in our study accounting for 75% of all affected cases. The brain was the second most common, and accounted for 15%. The most common location for the soft tissue lesions was upper extremities which accounted for 33% of cases, and the least common sites were the back, eyelid, buttock and cheek, which accounted for 3% each. Preoperative diagnoses of the lesions were as follows: lipoma, masses of undetermined nature, cysts, abscesses, fat necrosis and lymphadenitis. The mean size of lesions was 1.5 cm; the minimum and maximum sizes were 0.3 and 3 cm, respectively. The mean age of the patients was 33.5 years and the range was 9 to 68 years. The ratio of males to females was 1 to 5, which may reflect differences in eating habits and social life between males and females in our society. Our findings indicated that none of the cases in our study were diagnosed as cysticercosis preoperatively. Therefore, clinicians should include cysticercosis in the differential diagnosis of soft tissue cystic nodules or masses. Data regarding the sizes of lesions should be helpful in differentiating this entity from other soft tissue lesions in clinical practice and on magnetic resonance imaging.

Human dirofilariasis caused by Dirofilaria repens D. repens is a common zoonosis in the Mediterranean countries and parts of South Asia. During the last decade, it has been reported from countries previously considered non-endemic. This is likely due to the increased awareness regarding Dirofilaria infection. In some such cases however, dirofilariasis correlated with the travel of the patient to the endemic areas. We present the case of a Saudi male who had D. repens infection in the subcutaneous tissues of the forearm. The patient had traveled to Iraq, Syria, United Arab Emirates and Kuwait in the last 2 years. However, the known Dirofilaria endemic countries were not visited. Any prior occurrence of the human dirofilariasis in the Kingdom of Saudi Arabia is not documented. Lack of epidemiological studies or incidence reports in the Arabian region precludes any factual evaluation of Dirofilaria prevalence, which requires increased awareness amongst health workers regarding its potential occurrence in the native population.

Hydatidosis or echinococcosis is a parasitic disease caused by Echinococcus granulosus or E. multilocularis, which forms cysts in the liver and lung after penetrating the duodenal mucosa and entering the portal circulation. The liver and lung act as a filter but some embryos enter the general circulation and disseminate throughout the body. Musculoskeletal involvement is a rare manifestation of hydatidosis, which is usually reported to affect a single muscle. We report here a rare case of a 68-year-old man with widespread hydatidosis of the retroperitoneum and the subcutaneous adipose tissue, and with multiple muscle involvement in the absence of liver, lung, and spleen involvement. The patient underwent surgical excision of a subcutaneous hydatid cyst 7 years earlier. It is likely that the large dissemination of parasites resulted from accidental rupture of the primary focus during surgery with consequent release and spreading of scolices via lymphatics.

Infestation in soft tissue by Echinococcus granulosus is not a common disease, and its diagnosis is based on clinical, laboratory data and radiological findings. The aim of our retrospective study is to give an overview of the different signs and patterns shown by MRI that can be useful in characterizing soft tissue hydatid disease. The MRI images obtained in seven patients with soft tissue and subcutaneous hydatidosis were reviewed. Typical signs of hydatidosis were multivesicular lesions with or without hypointense peripheral ring ("rim sign"). Related to the presence and absence, respectively,of viable scolices in the microscopic exam, daughter cysts were presented either as high signal intensity or low signal intensity on T2-weighted images. Low-intensity detached layers within the cyst and peripheral enhancement with gadolinium-DTPA were also presented. Atypical signs were presented in an infected muscular cyst, a subcutaneous unilocular cyst and several unilocular cysts. Knowledge of the different patterns in MRI of soft tissue hydatid disease can be useful in diagnosing this entity. We observed that the "rim sign" is not as common as in other locations, and in addition, MRI seems to be of assistance when evaluating the vitality of the cysts.
