RESUMO

As the Queen Alexandra's Royal Army Nursing Corps celebrates its 70th Anniversary, army nursing continues to advance patient care delivery to new levels. Advanced level nursing practice has moved from the relatively 'calm' confines of the NHS to the austere desert of Oman. This article will provide a personal account of the first deployment of a military nurse practitioner since it was formally introduced in 2012 to frontline medicine, leading an armoured prehospital treatment team.

RESUMO

La luxación de la articulación temporomandibular (ATM) es un trastorno que generalmente se desconoce y que supone un reto para el personal sanitario que lo recibe. La etiología es variable. Puede producirse de manera espontánea por causas relacionadas con la apertura de la boca como el bostezo, risa, canto, entre otros. Clínicamente el paciente presenta boca abierta en forma de bostezo, estado de ansiedad y dolor. El tratamiento de elección es su reducción mediante la maniobra de Nelaton, que debe realizarse a la mayor brevedad. Se presenta un caso de un paciente con luxación anterior bilateral de la ATM al que se le realiza maniobra de Nelaton con éxito, sin la necesidad de administración de relajantes y analgésicos en el momento de su ejecución


A dislocation of the temporomandibular joint (TMJ) is an event that, despite being unusual, supposes a challenge for the health's personnel. The etiology is variable and can occur spontaneously due to causes related to the opening of the mouth such as yawning, laughing, singing... Clinically the patient has an open mouth in the form of yawning, anxiety and pain. The treatment of choice is its reduction through the Nelaton maneuver that must be carried out as soon as possible. We present a case of a patient with a bilateral anterior dislocation of the TMJ who underwent successful Nelaton maneuvers, without the need for administration of relaxants and analgesics at the time of execution

BACKGROUND: Nurses who serve in the military have a unique perspective on nursing and health care delivery that nurtures wholeness and inspires peace and healing on a global scale. PURPOSE: The purpose of this study was to explore health promotion and healing from the military nurse perspective. DESIGN: Video-recorded interviews were conducted with 10 military nurses who represented various branches and times of service. Participants were asked to share their experiences as military nurses and discuss the challenges and rewards. FINDINGS: Thematic analysis of the recorded interviews revealed two major themes: interconnectedness and human potential. CONCLUSION: This study showed that military nurses have unique experiences that influenced their way of promoting health and healing. Interconnectedness with family (personal and military) had many positive and negative factors. Interconnectedness with the health care team was more prominent for the nurses during military service than in the civilian arena. Global interconnectedness included working with teams from around the world, helping children of detainees see that Americans were not evil, and caring for international communities. Military service strengthened the three human qualities of mind, body, and spirit, which resulted in increasing each military nurse's human potential by enabling them to serve as instruments of healing on a global scale.

La colaboración de la Guardia Civil con la Unión Europea a través de FRONTEX ha incrementado el número misiones de los Buques Oceánicos de la Guardia Civil en el Mediterráneo para aumentar el control de los flujos migratorios, lo que hace que, además de una función policial, sus operaciones se conviertan en misiones humanitarias, rescatando a miles de personas. En cada uno de estos buques va un Oficial Enfermero cuya labor es tan importante como la del resto de los miembros de la tripulación


The cooperation of Guardia Civil with the European Union through FRONTEX has increased the number of missions of the Oceanic Vessels of the Civil Guard in the Mediterranean sea for a better control of migration flows. Therefore, in addition to a police role, its operations turn into humanitarian missions, rescuing thousands of people. Into each vessel there is a Nurse Officer whose work is as important as that of the rest of the crew members

BACKGROUND: Combat casualties undergoing aeromedical evacuation are at increased risk for pressure injuries. The risk factors pressure and shear are potentially modifiable via solutions appropriate for en route care. OBJECTIVES: To compare transcutaneous oxygen levels and skin temperatures in healthy participants under offloaded (side lying) and loaded (supine or supine with 30° backrest elevation) under 4 conditions: control (no intervention), Mepilex sacral and heel dressings, LiquiCell pad, and Mepilex plus LiquiCell. METHODS: Participants were randomly assigned to 4 groups according to ideal body weight. Backrest positions were randomized. Transcutaneous oxygen level and temperature were measured on the sacrum and the heel; skin interface pressure was measured with an XSensor pressure imaging system. Measurements were obtained for 5 minutes at baseline (offloaded), 40 minutes with participants supine, and 15 minutes offloaded. RESULTS: In the 40 healthy participants, interface pressure, transcutaneous oxygen level, and skin temperature did not differ between the 4 groups. Peak interface pressures were approximately 43 mm Hg for the sacrum and 50 mm Hg for the heel. Sacral transcutaneous oxygen level differed significantly between unloaded (mean, 79 mm Hg; SD, 16.5) and loaded (mean, 57 mm Hg; SD, 25.2) conditions (P < .001) in a flat position (mean, 85.2 mm Hg; SD, 13.6) and with 30° backrest elevation (mean, 66.7 mm Hg; SD, 24.2) conditions (P < .001). Results for the heels and the sacrum were similar. Sacral skin temperature increased significantly across time (approximately 1.0°C). CONCLUSIONS: The intervention strategies did not differ in prevention of pressure injuries.

BACKGROUND: US Navy nurses provide en route care for critically injured combat casualties without having a formal program for training, utilization, or evaluation. Little is known about missions supported by Navy nurses. OBJECTIVES: To characterize the number and types of patients transported and skill sets required by Navy nurses during 2 combat support deployments. METHODS: All interfacility casualty transfers between 2 separate facilities in Iraq and Afghanistan were assessed. Number of patients treated, number transported, en route care provider type, transport priority level and duration, injury severity, indication for critical care transport, en route care interventions, and vital signs were evaluated. RESULTS: Of 1550 casualties, 630 required medical evacuation to a higher level of care. Of those, 133 (21%) were transported by a Navy nurse, with 131 (98.5%) classified as "urgent," accounting for 46% of all urgent transports. The primary indication for en route care nursing was mechanical ventilation of intubated patients (97%). Mean (SD) patient transport time was 29.8 (7.9) minutes (range, 17-61 minutes). The most common en route care interventions were administration of intravenous sedation (80%), neuromuscular blockade (79%), and opioids (48%); transfusions (18%); and ventilation changes (11%). No intubations, cricothyroidotomies, chest tube placements, or needle decompressions were performed en route. No deaths occurred during transport. CONCLUSIONS: US Navy nurses successfully transported critically injured patients without observed adverse events. Establishing en route care as a program of record in the Navy will facilitate continuous process improvement to ensure that future casualties receive optimized en route care.

Aim The aim of this qualitative descriptive study was to describe the transition from military nurse to nurse faculty. Background There is a global shortage of qualified nurse faculty. Recruitment of retiring or separating military nurses is a viable solution. Methods Content and thematic analysis of semi-structured interviews of former military nurses (N=13) was undertaken. Results The transition from military nurse to nurse faculty includes acknowledgment of the reality of academic culture, appreciation of the journey to bridge the gap, and culmination of a new identity. Leadership skills acquired during military service proved instrumental to a successful transition. Conclusions Recognizing this transition and the strengths these nurses offer provides evidence for focused orientation programs to increase recruitment and retention of these faculty.

Entrevista realizada a Cristina González García, Capitán Enfermera de Vuelo destinada en la Unidad Médica de Aeroevacuación del ejército del Aire, sita en la Base Aérea de Torrejón de Ardoz en Madrid. Su misión se centra en la evacuación aérea internacional de pacientes y es responsable del área de entrenamiento y simulación de los equipos multidisciplinares sanitarios. Su experiencia en el traslado y en el cuidado durante la repatriación desde Liberia de los hermanos de la Orden Hospitalaria "San Juan de Dios" Miguel Pajares Martín y Manuel García Viejo, afectos de ébola, la convierten en un referente nacional e internacional. His experience in the transport and caring, during repatriation from Liberia, for Brothers of "Saint John of God Hospitaller Services" Miguel Pajares Martín and Manuel García Viejo, suffering from Ebola, turns her into a national and international reference expert. In the content of the interview, the Nurse Captain discusses the role of nurses in the armed forces, arrangements for the air transport of persons with Ebola, pre-training, circumstances found on arrival, the relationship with the affected people, their learning experience, social opinion and the case of Teresa Romero

RESUMO

Defence nurses form the largest registered specialty in the UK defence medical services. Once qualified, defence nurses maintain and develop their nursing and clinical skills in appropriate healthcare settings, and can be deployed in operational environments such as Afghanistan. Workforce planning and staffing establishment levels are defined to meet the needs of British armed forces, allies and, potentially, local populations. Since the workforce is geographically dispersed, deployed or undertaking non-clinical duties, there are constraints on nurses' attempts to maintain basic skills and access continuing professional development. This article explores the concept and the developing role of defence nurse lecturers in improving educational support for defence nurses.

Today in the digital age, with our advances in modern technology and communication, there are additional stressors for our military personnel and Veterans. Constant dangers exist both on and off the battlefield, unlike prior wars that had clearly-defined war zones. In addition, medical advances have assisted in saving the lives of many more gravely injured troops than ever previously possible. As the wars in Iraq and Afghanistan come to an end, large numbers of service men and women are returning home with multiple injuries. This group of Veterans has significantly higher rates of posttraumatic stress disorder (PTSD) and traumatic brain injury than ever before reported. Although existing PTSD therapies have been found to be highly effective for many Veterans, there is a substantial minority unsatisfactorily treated. Mantram repetition, an innovative, complementary, evidence-based treatment, is proving to be successful for these new Veterans. When used regularly it helps with "road rage, impatience, anger, frustration, and being out of control." A mantram is a brief, sacred word or phrase that embodies divine power or the greatest positive energy one can imagine (Easwaran, 2008a). Mantram repetition is a simple, quick, personal, portable, and private complementary practice that may be used as an adjunct to current treatments for PTSD. Growing research evidence supports mantram repetition's value for dissemination and adoption in the 21st century. This chapter summarizes Mantram Program research conducted from 2003 to 2014. It describes the health-related benefits of the Mantram Program in various populations. The current research focuses on benefits for managing psychological distress and promoting quality of life in Veterans. Future areas for research are suggested.
