RESUMO

HLA A, B, and C antigens were studied in 25 patients with ankylosing hyperostosis. There was no evidence of an increased frequency of any antigen in association with the condition itself. However, a high frequency of several antigens was found, reflecting the predominance of Jewish patients in the series.

RESUMO

Vertebral ankylosing hyperostosis is a common skeletal disorder among Pima Indians. Prevalence rates of approximately 50% have been reported in Pima Indian males aged 44 or older. The HLA-B27 antigen is also found more commonly in Pima Indians than in Caucasians (18% versus 6%). Because of the increased prevalence of both vertebral ankylosing hyperostosis and HLA-B27 in Pima Indians, a possible association between the two was thought to exist. Therefore, histocompatibility testing was done on 44 adult male Pima Indians age 55 or older with classic vertebral ankylosing hyperostosis and 33 age-matched controls. No significant association between any histocompatibility antigen phenotype or haplotype and vertebral ankylosing hyperostosis was found. This disease, therefore, connot be classified with the HLA-B27 associated spondylarthropathies.

HLA antigens were determined in 50 patients with hyperostotic spondylosis (Forestier's disease), and compared with 1244 normal controls. There was no significant difference in the distribution of HLA B27 between the two groups. However, there was an increased incidence of HLA B8 among the patients with hyperostotic spondylosis (42 per cent), as compared to the normal controls (17.5 per cent). This difference was statistically significant (p less than 0.005). Sixteen of the 50 patients with hyperostotic spondylosis had evidence of diabetes mellitus, 12 of whom also had the HLA B8 antigen. Thus patients with hyperostotic spondylosis have an increased incidence of HLA B8, associated with an increased incidence of diabetes mellitus.

HLA antigens were studied in three different groups of 50 patients each. These included (a) Forestier's disease, (b) ankylosing spondylitis, and (c) polyarthrosis of the hands. HLA typing included 12 specificities from locus A and 15 from locus B, the frequencies being compared to those in 700 normal controls. No significant differences were found in the frequency of distribution between the polyarthrosis patients and the normal population. In patients with Forestier's disease, B5 was increased, but this was not a significant difference. The antigen B27 was present in 94 per cent of patients with ankylosing spondylitis, confirming previous studies.

Despite the lack of apophyseal or sacroiliac joint involvement, Forestier's disease (vertebral ankylosing hyperostosis) shares with the inflammatory forms of spondylitis, the roentgenographic appearance of spinal new bone formation. Because of this apparent similarity, the prevalence of the HL-A B27 antigen was determined in 47 white patients with Forestier's disease. Sixteen of the patients (34 per cent) possessed the B27 antigen (P is less than 0.001). The mere presence of B27 therefore, does not confirm the diagnosis of inflammatory spondylitis in all patients with roentgenographic evidence of osseous bridging. Applying available epidemiologic information, these data further suggest that patients with the B27 antigen may be at substantial risk of developing Forestier's disease. As Forestier's disease and virtually all of the other B27 associated arthropathies manifest abundant new bone, an association may exist between this antigen and genes controlling new bone formation.

RESUMO

Of the many associations between histocompatibility antigens and human diseases a prominent one is that between HLA-B27 and inflammatory arthropathies. Hypotheses to explain this association include the B27 gene being linked to a specific immune-response gene required for disease expression and the B27 antigen acting via molecular mimicry with a microorganism or as a microorganism receptor. Alternatively, the HLA-B27 gene might be closely related to a gene which influences bone formation. The finding of a significant association between B27 and Forestier's disease, a disease characterised by abundant new bone formation, supports such a hypothesis.
