RESUMO

OBJECTIVE: Myxofibrosarcoma is clinically characterized by a high frequency of local recurrence after surgery. To improve the clinical outcome of patients with myxofibrosarcoma, it is imperative to control any postsurgical local recurrence. METHODS: In this study, we performed a retrospective clinicopathologic analysis of 100 consecutive patients with myxofibrosarcoma to identify factors related to poor prognosis. All of the patients had been diagnosed, and had undergone surgery at the National Cancer Center Hospital between 1999 and 2008. RESULTS: At the initial visit to our hospital, 64 patients had primary myxofibrosarcoma, whereas 36 had undergone primary unplanned resection at other facilities. Of the 36 patients, 11 consulted our hospital before recurrence and 25 did so after recurrence. A histologically positive margin after surgery was evident in 28% of the cases overall. The estimated 5-year recurrence-free survival rate was 74.8%. Univariate analysis showed that primary unplanned resection at another facility (P = 0.0001) and a histologically positive margin (P = 0.0224) were significant predictors of local recurrence. When these two factors were subjected to multivariate analysis, only primary unplanned resection at another facility was significantly correlated with the estimated recurrence-free survival rate (P = 0.0011). Primary unplanned resection was also significantly related to the 5-year disease-free survival rate (P = 0.0401). CONCLUSIONS: Our findings indicate that primary unplanned resection at a non-referral hospital is the most important risk factor related to poor prognosis of myxofibrosarcoma. Accurate diagnosis and adequate initial surgery are most important factors for improving the clinical outcomes of myxofibrosarcoma.

The differential diagnosis of swelling of the small finger joints is broad. We report on two young men presenting with progressive painless swelling of the proximal interphalangeal (PIP) joints where the physical examination was otherwise inconspicuous. Laboratory investigations including auto- antibodies were all normal, x-rays and magnetic resonance imaging (MRI) revealed no joint pathologies but only increased skin thickening. Skin biopsy demonstrated hyperkeratosis and acanthosis without tissue inflammation. We thus diagnosed pachydermodactyly in both cases. This rare disease predominantly affects young males, can affect one or both hands and is often associated with mechanical stress due to repetitive movements in patients with obsessive-compulsive disorders. Occupational exposure by monotonous actions (e.g. poultry processing) may also be a cause. A specific therapy is unnecessary in most cases but may include intralesional steroid administration or surgical resection. Cessation of mechanical stress can significantly improve pachydermodactyly.

Rat skin was exposed to the plateau region of the 1.0 GeV/nucleon 56Fe beam at the Brookhaven AGS. Rats were irradiated or not with single of split doses of 56Fe or argon; some 56Fe-exposed rats were fed 250 ppm retinyl acetate continuously in the lab chow beginning 1 week before irradiation. All lesions were noted, photographed and identified for eventual histological diagnosis. The preponderance of the tumors so far are fibromas. The data show that single doses of 56Fe ions are 2 or 3 fold more effective than argon in producing tumors at 4.5 Gy but are about equally effective at 3.0 Gy and 9.0 Gy. The presence of 250 ppm retinyl acetate in the lab chow reduced the incidence of tumors by about 50-60% in comparison to groups exposed only to the radiation. These are preliminary findings based on only about one-fourth the eventual number of tumors expected.

BACKGROUND: Deep musculoaponeurotic fibromatoses are rare soft tissue neoplasms with a propensity for local recurrence. METHODS: A retrospective analysis was carried out of the factors contributing to local disease control in 75 patients treated between 1963 and 1993. RESULTS: Multivariate analysis identified the type of surgical excision (P < 0.001) and involvement of pathological resection margins (P < 0.02) as significant factors contributing to local recurrence. After a median follow up of 47 months (range 24 months to 29 years) 31 (49 per cent) of the 63 patients who had an 'adequate' surgical resection developed local recurrence. The median time before development of local recurrence was 83.4 (range 8-129) months in patients with clear pathological resection margins. This was significantly shortened to 13.1 (range 2-35) months in those with positive margins (P < 0.001). CONCLUSION: Adequate surgical extirpation is the most important determinant in local disease control. Treatment of local recurrence ranged from observation during periods of disease stabilization to multimodality treatment for aggressive disease.

After the works on the promoting effect of trichinellosis on some viral infections in rodents, many studies successively demonstrated that Trichinella spiralis confers resistance to many unrelated antigens including pathogens, such as Protozoa, Bacteria and tumour cells (B16 melanoma). Considering the above contradictory results, the present work was undertaken to study, in rabbits, T. spiralis as a modulator of Shope's fibroma virus, an oncogenic virus responsible for a benign neoplasia. Four groups of 6 rabbits each were used. The rabbits of group I, II and III were inoculated per os with 3000; 6000 and 12,000 T. spiralis larvae, respectively. The rabbits of group IV were used as controls. Thirty-five days after the inoculation, all the animals were injected at the fixed doses of 0.5 ml with dilutions (10(-1) to 10(-8] of Shope's fibroma virus given intradermally into 8 different points of the skin of each pretreated and untreated rabbits. After 9 days tumour lesions affecting the inoculating area were noticed and the DI 50/0.5 of Shope's fibroma virus was then determined for each of the 4 experimental groups. The rabbits pretreated with T. spiralis exhibited much lower virus titres than the controls, which was evidently related to a certain degree of aspecific immunity conferred by the parasite. The results indicated that T. spiralis produces, in rabbits, resistance to Shope's fibroma virus and its neoplastic effect.

Female rats were treated with buformin, phenytoin, polypeptide pineal extract, L-DOPA or buformin combined with L-DOPA during 3 weeks before intravenous injections of DMBA (1.5 mg 6 times with one-week intervals) over a period of carcinogen injections and after it till the animals' death. The overall tumour incidence in the control group was 97%, while in buformin, phenytoin, pineal extract, L-DOPA and buformin + L-DOPA treated groups it amounted to 55, 71, 80, 50 and 62%, respectively (P < 0.05). The incidence of mammary adenocarcinoma amounted to 81, 36, 55, 26, 25 and 19%, respectively (P < 0.05). The mechanisms of the similar effects the drugs belonging to different classes produce on chemical carcinogenesis are discussed.
