Canine facial eosinophilic furunculosis (FEF) is a hyperacute dermatopathy especially of the nasal bridge of dogs and is probably associated with type I hypersensitivity secondary to arthropod bites. The aim of this study is to report on a FEF case in a four-year-old female free-roaming mixed-breed dog showing papules on the nasal bridge that evolved to an ulcerated plaque. No other clinical, hematological, or biochemical alterations were detected. Cytology revealed eosinophilic and neutrophilic inflammation associated with bacterial infection. Punch biopsies were obtained for histopathological and microbiological analysis. Histopathology revealed marked, acute, multifocal to coalescent granulomatous eosinophilic furunculosis, and mild, acute, multifocal eosinophilic folliculitis. Microbiology revealed growth of coagulase-positive Staphylococcus sp. Clinical and histopathological findings were suggestive of facial eosinophilic folliculitis and furunculosis. Complete remission of the lesions was obtained after treatment. This condition is hyperacute, progressive, with a papular and erosive to ulcerative pattern, good prognosis, and its development is linked to arthropod bites. Furthermore, anti-inflammatory therapy is effective in treating the disease.(AU)

Foram atendidos quatro casos de furunculose bacteriana, causados pelo agente Streptococcus spp., em equinos de um centro de equoterapia no município de Lavras MG. Microrganismos do gênero Streptococcus não são agentes usuais deste tipo de afecção. As lesões se apresentavam circunscritas, alopécicas, nodulares, crostosas, dolorosas e não pruriginosas. O diagnóstico se baseou na apresentação clínica e na cultura de secreções coletadas das lesões cutâneas. Antibioticoterapia sistêmica e pomadas tópicas foram utilizados no tratamento, levando à resolução dos casos em vinte dias. We report four cases of bacterial furunculosis caused by Streptococcus spp. in horses located in a therapeutic riding center inLavras-MG. Microorganisms of the Streptococcus genus are not usual agents of this affection. The lesions were circumscribed,alopecic, nodular, some showed crosts, were painful and not itchy. The diagnosis was based on clinical presentation and culture ofsecretions collected from these lesions. Treatment consisted of systemic antibiotics and topical ointments, leading to resolutionin twenty days.(AU)

RESUMO A trajetória do cliente oncológico ainda é permeada por percalços que interferem em seu prognóstico e qualidade de vida. Assim, este estudo visa a mapear o itinerário do paciente oncológico desde o diagnóstico até o tratamento e analisar suas implicações nas ações de enfermagem para o controle do câncer. Estudo qualitativo, realizado através da técnica de história oral de dez clientes oncológicos atendidos em um hospital federal no município do Rio de Janeiro, Brasil. Os dados foram tratados de acordo com a técnica de análise temática, além da confecção de representações do itinerário terapêutico de cada entrevistado. Foram identificados três eixos temáticos: trajetória antes do diagnóstico, trajetória do diagnóstico ao início do tratamento, e trajetória no tratamento. Percebe-se que o acesso aos serviços apresenta-se como um problema que culmina no agravamento da doença, no aparecimento de metástases e a morte do paciente, além de criar condições para a falta de adesão ao tratamento.


RESUMEN La trayectoria del cliente oncológico todavía es permeada por inconvenientes que interfieren en su pronóstico y calidad de vida. Así, este estudio pretende identificar el itinerario del paciente oncológico desde el diagnóstico hasta el tratamiento y analizar sus implicaciones en las acciones de enfermería para el control del cáncer. Estudio cualitativo, realizado a través de la técnica de historia oral de diez clientes oncológicos atendidos en un hospital federal en el municipio de Rio de Janeiro, Brasil. Los datos fueron tratados de acuerdo con la técnica de análisis temático, además de la confección de representaciones del itinerario terapéutico de cada entrevistado. Fueron identificados tres ejes temáticos: trayectoria antes del diagnóstico; trayectoria del diagnóstico al inicio del tratamiento; y trayectoria en el tratamiento. Se nota que el acceso a los servicios se presenta como un problema que culmina en el agravamiento de la enfermedad, el en surgimiento de metástasis y la muerte del paciente, además de crear condiciones para la falta de adhesión al tratamiento.


ABSTRACT The course of oncology patients is still crossed by setbacks that interfere with their prognosis and quality of life. This study aims to map the itinerary of cancer patients from diagnosis to treatment and to analyze its implications in nursing healthcare practices for the control of cancer. This is a qualitative study, conducted using oral history technique with ten cancer patients treated in a federal hospital in the city of Rio de Janeiro, Brazil. The data were analyzed according to thematic analysis technique and with the making of representations of the therapeutic itinerary of each interviewee. Three main themes were identified: course before diagnosis, course from diagnosis to the beginning of treatment, and course during treatment. We observed that the access to medical services is presented as a problem that culminates in the worsening of the disease, appearing of metastases and death of the patient, in addition to creating conditions that hinder the adherence to treatment.

Skin and soft tissue infections are a common reason for consultation in primary health care centers. Data from the local epidemiology of these infections are rare, but Staphylococcus aureus and Streptococcus pyogenes are known to be the major etiologic agents. The appearance in recent years of community-originated strains of methicillin-resistant S. aureus and erythromycin-resistant pyogenes raises controversy in the choice of initial empirical treatment. This national consensus is for pediatricians, dermatologists, infectiologists and other health professionals. It is about clinical management, especially the diagnosis and treatment of community-originated skin and soft tissue infections in immunocompetent patients under the age of 19.

Las infecciones de piel y partes blandas son una causa frecuente de consulta en los centros de atención primaria de la salud. Los datos de la epidemiología local de estas infecciones son escasos; el Staphylococcus aureus y el Streptococcus pyogenes son los principales agentes etiológicos. La emergencia, en los últimos años, de cepas de S. aureus meticilino resistentes provenientes de la comunidad y S. pyogenes resistentes a eritromicina plantea controversias en la elección del tratamiento empírico inicial. Este consenso nacional está dirigido a médicos pediatras, de familia, dermatólogos, infectólogos y otros profesionales de la salud. Trata el manejo clínico, especialmente el diagnóstico y tratamiento, de las infecciones de piel y partes blandas de origen bacteriano provenientes de la comunidad en pacientes inmunocompetentes menores de 19 años de edad.

Background: The eosinophilic furunculosis is an uncommon skin disease that affects young dogs aged between two and five years. Sex predilections are not noted. Most reported cases have been in large breeds with abundant access to the outdoors like public gardens and parks. The exact pathogenesis remains unknown but the pattern of lesions development suggests involvement reaction to insect bites and arthropods. The time between contact with the agent and the appearance of lesions is short, often less than 24 h. The skin lesions appear suddenly in the face, usually on the dorsal muzzle and/ or periocular region, pinnae and lips. More rarely on the trunk, chest and legs and it is characterized by predominantly hemorrhagic ulcers with edema. Pustules, nodules and plaques rapidly fistulate and drain serosanguinous exudates. Pruritus is variable, but may be severe. Severely affected dogs may be febrile, lethargic and anoretic. Peripheral blood eosinophilia is seen in the majority of cases. Clinical differential diagnosis include demodicosis, dermatophytosis, nasal deep bacterial folliculitis and furunculosis, pemphigus foliaceus, pemphigus erythematosus and drug reactions. None of these diseases share the fulminant rapid onset of eosinophilic furunculosis. The diagnosis is based on history, clinical signs, cytology and skin biopsies. The treatment involves oral steroids until complete remission of lesions and the antibiotic therapy is indicated in cases of associated bacterial infection. Case: The patient was a 4-year-old, male Pit Bull dog attended in a small animal clinic in Niteroi-Rio de Janeiro, which presented ulcerated, exudative lesions on the dorsal muzzle and right leg. After sedation, clinical examination was performed and an exudate was collected from the ulcerated skin lesion for cytopathological analysis .The slide containing the lesion impression was stained by a quick panoptic method. In order to collect samples, the dog was sedated with a combination of ketamine hydrochloride and acepromazine and a skin fragment was collected from the nasal bridge lesion with a 6 mm punch after local anesthesia with 2% lidocaine hydrochloride. The specimen was fixed in 10% buffered formalin and sent for histopathological analysis. The cytopathological exam revealed a marked eosinophilic inflammation. Histopathological examination revealed ulcerated skin. The epidermis was moderately acanthotic with mild espongiosis and the dermis was characterized by intense eosinophilic folliculocentric inflammations. An extensive folicular rupture, eosinophilic mural foliculitis were presented and PAS staining did not identify fungal structures. Oral prednisone (2 mg/Kg) at 24h intervals was prescribed until complete remission of the lesions. After fifteen days of glucocorticoids therapy, involution of the skin lesions was observed by physical examination and was also reported by the owner. Discussion: The eosinophilic furunculosis is an acute, severe predominantly facial disease of outdoor dogs, which occurrence is rare. The diagnosis and treatment of this disease are frequently neglected because they are not included in the differential diagnosis of diverse cutaneous infections. In view of the scarcity of reports and to alert veterinarians that the disease should be included in the differential diagnosis with other bacterial diseases, this report described a case of canine eosinophilic furunculosis.

Furuncular myiasis is an infestation of the skin caused by Dermatobia hominis larvae known as "ver macaque" in French Guyana, "berne" in Brazil, "torsalo" in Colombia, or "human botfly" in English-language literature. It has identical features in man and domestic mammals. The primary lesion consists of a boil-like inflammatory papule with a central punctum exuding a serosanguinous discharge. The respiratory sinus of the D. hominis larvae may be visible through the punctum. Myiasis secondary to D. hominis accounts for 10% of imported tropical dermatosis observed in Paris. Diagnosis of furuncular myiasis should be considered in any patient with a history of travel or residence in an endemic area. Treatment depends mainly on mechanical removal that may be facilitated by injection of lidocaine into the lesion or prior application of a 1% solution of ivermectin.

La infestación por larvas de Dermatobia hominis es frecuente en áreas rurales de Latinoamerica. Hasta la fecha es posible encontrar numerosos reportes de casos en turistas que visitan sus zonas selváticas o montañosas. Sin embargo, es poco lo que se ha escrito sobre la infestación en pobladores habituales de estas regiones. En el presente artículo se reporta una serie de casos en indigenas Pemón de la Gran Sabana venezolana y se hace una extensa revisión de la literatura concerniente a esta enfermedad.

We describe a case of a young boy and his father who presented with furunculosis secondary to infestation with the botfly. The infected patients live in South Florida and had been vacationing in Central America. Standard surgical treatment as well as multiple native remedies are described.
