Soft and hard tissue resection in periodontal therapy.

The stabilization of inflammatory periodontal diseases (gingivitis and periodontitis) and result periodontal maintenance is achieved most effectively by first establishing health within the dento-gingival complex (connective tissue, junctional epithelium, and sulcus). Two forms of surgical periodontal therapy commonly employed to reach this endpoint are resective in nature:, the gingivectomy/gingivoplasty technique for soft tissue resection and the ostectomy (osteoectomy)/ osteoplasty approach for hard tissue resection. Periodontal pockets deeper than 4-5 mm require surgical treatment. Records of surgical journals of a periodontology room over 6 years are reviewed and 2183 operations performed by six periodontologists are analyzed. The results of surgery are determined by patient's status, site of anesthesia, type of secondary adentia, convenience of surgical access, terms between operations. Individual features of surgical technique of each of the six surgeons are shown.

Can surgery be justified for patients with periodontitis?

There is a debate concerning the role that surgical procedures should play in the treatment of inflammatory periodontal diseases. This review details some of the situations in which surgery can be of benefit. Benefits include obtaining shallower probing depths and therefore reducing the areas that are hospitable to pathogenic bacteria associated with disease. Also, removal of tooth-accumulated materials such as plaque and calculus in probing depths greater than 5 mm is more predictable with surgery. In some forms of aggressive periodontitis, elimination of potentially pathogenic bacteria is found more frequently with surgical procedures. Regeneration of lost bone and attachment apparatus are predictable only with surgery.

The provision of periodontal services in general dental practice in Australia.

Recent studies have documented the low percentage of professional services which are associated with periodontal care. The aims of the present study were to describe the provision of periodontal services by private general practice dentists in Australia and to investigate the variation of provision of periodontal services. Using a day log approach via a mailed questionnaire one in eight services provided were found to be related to periodontal care. Most were of a preventive nature. There was great variation between dentists in the number of services provided indicating the possibility of both under and overservicing. The findings are discussed with regard to the changing philosophies of provision of periodontal services and known methods of eliciting change among providers.
