BACKGROUND: Little is known about the determinants of asthma among youth with high T helper 2 (Th2) immunity. We hypothesized that exposure to violence (ETV) and violence-related distress are associated with asthma in children and adolescents with high Th2 immunity. METHODS: We analyzed data from Puerto Ricans with high Th2 immunity aged 9-20 years in the Puerto Rico Genetics of Asthma and Lifestyle (PR-GOAL) and the Epigenetic Variation of Childhood Asthma in Puerto Ricans (EVA-PR) studies, and in a prospective study (PROPRA). High Th2-immunity was defined as ≥1 positive allergen-specific IgE and/or a total IgE ≥ 100 IU/mL and/or an eosinophil count ≥ 150 cells/µL. Asthma was defined as physician-diagnosed asthma and current wheeze. ETV and violence-related distress were assessed with the validated ETV Scale and Checklist of Children's Distress Symptoms (CCDS) questionnaires, respectively. RESULTS: In multivariable analyses, each 1-point increment in ETV score was significantly associated with 1.13-1.17 times increased odds of asthma in PR-GOAL and in EVA-PR (both at p ≤ 0.01), and each 1-point increment in CCDS score was significantly associated with 1.53-1.54 increased odds of asthma in PR-GOAL and in EVA-PR (both at p ≤ 0.03). Further, a persistently high ETV score was significantly associated with asthma in PROPRA (odds ratio [OR] = 2.83, 95% confidence interval [CI] = 1.10-7.29). Similar results were obtained in a sensitivity analysis using an eosinophil count ≥ 300 cells/µL instead of ≥150 cells/µL to define high Th2 immunity. CONCLUSIONS: ETV during childhood is associated with increased risk of persistent or new-onset asthma in youth with high Th2 immunity.

Cocoa contains bioactive components, which vary according to genetic and environmental factors. The present study aimed to ascertain the anti-allergic properties of native Peruvian cocoa populations ("Blanco de Piura" or BPC, "Amazonas Peru" or APC, "Criollo de Montaña" or CMC, "Chuncho" or CCC, and an ordinary cocoa or OC). To do so, after an initial in vitro approach, an in vivo study focused on the induction of an anaphylactic response associated with allergic asthma in Brown Norway rats was carried out. Based on their polyphenol content, antioxidant activity and in vitro effects, the APC and CMC were selected to be included in the in vivo study. Cocoa diets were tested in a model of allergic asthma in which anaphylactic response was assessed by changes in body temperature, motor activity and body weight. The concentration of specific immunoglobulin E (IgE), mast cell protease and leukotrienes was also quantified in serum and/or bronchoalveolar lavage fluid. CMC and OC populations exhibited a protective effect on the allergic asthma rat model as evidenced by means of a partial protection against anaphylactic response and, above all, in the synthesis of IgE and the release of mast cell protease.

BACKGROUND: Nasal (airway) epithelial methylation profiles have been associated with asthma, but the effects of such profiles on expression of distant cis-genes are largely unknown. RESEARCH QUESTION: To identify genes whose expression is associated with proximal and distal CpG probes (within 1 Mb), and to assess whether and how such genes are differentially expressed in atopic asthma. STUDY DESIGN AND METHODS: Genome-wide expression quantitative trait methylation (eQTM) analysis in nasal epithelium from Puerto Rican subjects (aged 9-20 years) with (n = 219) and without (n = 236) asthma. After the eQTM analysis, a Gene Ontology Enrichment analysis was conducted for the top 500 eQTM genes, and mediation analyses were performed to identify paths from DNA methylation to atopic asthma through gene expression. Asthma was defined as physician-diagnosed asthma and wheeze in the previous year, and atopy was defined as at least one positive IgE to allergens. Atopic asthma was defined as the presence of both atopy and asthma. RESULTS: We identified 16,867 significant methylation-gene expression pairs (false-discovery rate-adjusted P < .01) in nasal epithelium from study participants. Most eQTM methylation probes were distant (average distance, ∼378 kb) from their target genes, and also more likely to be located in enhancer regions of their target genes in lung tissue than control probes. The top 500 eQTM genes were enriched in pathways for immune processes and epithelial integrity and were more likely to have been previously identified as differentially expressed in atopic asthma. In a mediation analysis, we identified 5,934 paths through which methylation markers could affect atopic asthma through gene expression in nasal epithelium. INTERPRETATION: Previous epigenome-wide association studies of asthma have estimated the effects of DNA methylation markers on expression of nearby genes in airway epithelium. Our findings suggest that distant epigenetic regulation of gene expression in airway epithelium plays a role in atopic asthma.

BACKGROUND: Strongyloides stercoralis is an intestinal nematode unique in its ability to replicate in the human host, allowing ongoing cycles of autoinfection, persisting for decades within the same host. Although usually asymptomatic, overwhelming infections can occur in Strongyloides and HTLV-1 co-infected individuals (SS/HTLV-1). Regulatory T cells (Tregs) are able to blunt specific Th2 responses necessary to control the parasite. We previously reported that peripheral blood Tregs are increased in SS/HTLV-1 and correlate with low Th2 responses. We hypothesized that Tregs are also increased at the site of infection in duodenal mucosa. METHODS: Paraffin embedded duodenal biopsies were obtained from 10 SS/HTLV-1 patients, 3 controls with non-parasitic chronic duodenitis, and 2 healthy controls. Immunohistochemistry was performed using monoclonal antibodies against human CD3, CD8, IgE and FoxP3. The number of cells were counted using a conventional light microscope. The number of CD3+, CD8+, FoxP3+ and IgE positive cells per 0.35 mm2 was measured using ImagePro Plus software comparing areas adjacent or distant from parasite material. RESULTS: In patients with SS/HTLV-1, T lymphocyte counts and CD8+ cells were lower in areas adjacent to the parasite compared to non-adjacent areas (CD3+: adjacent: 6.5 [Interquartile range (IQR: 2.8-12.3)]; non-adjacent: 24.5 [IQR: 20.9-34.4]; Mann-Whitney p = 0.0003; CD8+: adjacent: 4.5 [IQR: 2.3-11.8]; non-adjacent: 21 [IQR: 15.3-42.9]; Mann-Whitney p = 0.0011). Tregs cells in the intestines (FoxP3+ expressing cells) were increased in patients with SS/HTLV-1 compared with patients with chronic duodenitis (SS/HTLV-1: 1.5 [IQR: 0.7-2.3]; duodenitis controls: 0 [range 0-0.7]; healthy controls: 0; Mann-Whitney p = 0.034). There was also a trend towards fewer eosinophils adjacent to the parasites. Among SS/HTLV-1 patients the number of IgE expressing cells was increased for in areas not adjacent to the parasite compared to non-adjacent areas (ANOVA, p = 0.001). CONCLUSIONS: Our data shows increased Treg cell numbers localized adjacent to the parasites in the duodenum SS/HTLV-1 patients. In addition, other T lymphocytes and IgE expressing cells were decreased adjacent to the parasites, suggesting an important role for Tregs in down-regulating local parasite effector responses.

Given the growing incidence and prevalence of life-threatening food allergies, health concerns have raised new perspectives for in vivo and in vitro diagnostic methodologies, pointing to saliva as a promising material, already used to diagnose other pathologies. Based on the above considerations, this study aimed to verify the possible use of saliva for the detection of IgE and IgG1 in the diagnosis of food allergy. This was a randomized, cross-sectional clinical study with a quantitative approach, developed at a hospital referral center in allergy in the state of Ceará, from January to July 2015. The sample consisted of 36 children of both sexes, aged between 1 and 60 months, with a diagnosis of cow's milk protein allergy (CMPA) by the RAST test. Children hospitalized or under immunosuppressive drugs were excluded from the study. Serum and saliva samples of the participants were collected and subsequently subjected to the indirect immunoenzymatic assay (ELISA) for the detection of specific serum and salivary immunoglobulins for food: corn, papaya, cow's milk, egg white, wheat, soybeans, peanuts, nuts, kiwi, cacao, fish, shrimp, bananas and tomatoes. For comparison of serum and saliva results, the T-test of independent samples and Mann-Whitney were adopted, for samples with normal and non-normal distribution respectively. A confidence interval of 95% was adopted for significant results. It was observed that 100% (n = 36) of the participants presented cow's milk allergy through the indirect ELISA, detecting IgE or IgG1 in serum and saliva. When serum IgE and IgG1 concentrations were compared, there was no statistical difference (p > 0.05) in 12 of the 14 foods evaluated. The same amount (n = 12) of non-significant differences (p > 0.05) was observed in the comparison of the 14 foods under IgE and IgG1 contractions in saliva. In the verification of the average values of IgE present in the serum and saliva of the foods, only cow's milk, fish and papaya showed statistically significant differences (p < 0.05). Of the total food evaluated, only the average levels of IgG1 present in serum and saliva showed a significant value (p < 0.05) in banana and tomato. These findings indicate that the detection of IgE and IgG1 in saliva proves to be as efficient as in the serum. The use of the salivary technique for use in the diagnosis of food allergy is suggested.

Considering its role in inflammation and recently described "alternative" roles in epithelial homeostasis and Th1/Th2 balance, we hypothesize that inflammasome genetics could contribute to the development of asthma. Selected functional polymorphisms in inflammasome genes are evaluated in a cohort of asthmatic children and their families. Gain-of-function NLRP1 variants rs11651270, rs12150220 and rs2670660 resulted significantly associated to asthma in trios (TDT) analysis; and rs11651270 and rs2670660 also with asthma severity and total IgE level in asthmatic children. NLRP1 activators in humans are still unknown, however we hypothesized that individuals with gain-of-function SNPs in NLRP1 could be more prone in activating inflammasome in the presence of asthma-related cell stressors (i.e. ER stress or ROS), and this activation contribute to exacerbate inflammatory response and asthma development. Gain-of-function IL1A rs17561 resulted significantly associated with a reduced pulmonary capacity in asthmatic children. IL18 rs5744256 which lead to lower serum level of IL-18 appeared to be associated to a worse response to bronchodilators. Concluding, this work provides evidences about the contribution of inflammasome genetics in the development of paediatric asthma, both considering its inflammatory role in alveolar macrophages (i.e.: NLRP1) or its homeostatic role in lung epithelial cells (i.e.: IL1A, IL18).

Abstract BACKGROUND: Chronic urticaria is characterized by transient, pruritic lesions of varying sizes, with central pallor and well-defined edges, with disease duration longer than six weeks. Its cellular infiltrate consists of neutrophils, lymphocytes and eosinophils. There is a subgroup of patients with eosinophilic or neutrophilic urticaria, resistant to the treatment with antihistamines, but that respond to a combination of antihistamine with other drugs. OBJECTIVE: To evaluate the present infiltration in chronic urticaria biopsies and correlate it with the clinical disease activity and response to treatment. METHODS: Forty-one patients with chronic urticaria were classified according to the score of severity of the disease, response to treatment and type of perivascular infiltrate. Inflammatory infiltrates were divided in eosinophilic (46.30%), neutrophilic and mixed. RESULTS: An association was found between the eosinophilic infiltrate and clinical scores of greater severity (p = 0.002). CONCLUSION: This association shows that the eosinophilic inflammatory infiltrates denote high clinical activity, which means more severe and exuberant clinical pictures of the disease.

OBJECTIVE:: To determine the presence of staphylococcal superantigen-specific IgE antibodies and degree of IgE-mediated sensitization, as well as whether or not those are associated with the severity of asthma in adult patients. METHODS:: This was a cross-sectional study involving outpatients with asthma under treatment at a tertiary care university hospital in the city of Rio de Janeiro, Brazil. Consecutive patients were divided into two groups according to the severity of asthma based on the Global Initiative for Asthma criteria: mild asthma (MA), comprising patients with mild intermittent or persistent asthma; and moderate or severe asthma (MSA). We determined the serum levels of staphylococcal toxin-specific IgE antibodies, comparing the results and performing a statistical analysis. RESULTS:: The study included 142 patients: 72 in the MA group (median age = 46 years; 59 females) and 70 in the MSA group (median age = 56 years; 60 females). In the sample as a whole, 62 patients (43.7%) presented positive results for staphylococcal toxin-specific IgE antibodies: staphylococcal enterotoxin A (SEA), in 29 (20.4%); SEB, in 35 (24.6%); SEC, in 33 (23.2%); and toxic shock syndrome toxin (TSST), in 45 (31.7%). The mean serum levels of IgE antibodies to SEA, SEB, SEC, and TSST were 0.96 U/L, 1.09 U/L, 1.21 U/L, and 1.18 U/L, respectively. There were no statistically significant differences between the two groups in terms of the qualitative or quantitative results. CONCLUSIONS:: Serum IgE antibodies to SEA, SEB, SEC, and TSST were detected in 43.7% of the patients in our sample. However, neither the qualitative nor quantitative results showed a statistically significant association with the clinical severity of asthma. Os pacientes foram alocados consecutivamente em dois grupos de gravidade da asma segundo critérios da Global Initiative for Asthma: asma leve (AL), com asmáticos leves intermitentes ou persistentes, e asma moderada ou grave (AMG). Foram determinados os níveis séricos de anticorpos IgE antitoxinas estafilocócicas, e os resultados foram comparados por análise estatística. RESULTADOS:: Foram incluídos 142 pacientes no estudo: 72 no grupo AL (mediana de idade = 46 anos; 59 do sexo feminino) e 70 do grupo AMG (mediana de idade = 56 anos; 60 do sexo feminino). BACKGROUND: Exposure to gun violence and African ancestry have been separately associated with increased risk of asthma in Puerto Rican children. OBJECTIVE: The objective of this study was to examine whether African ancestry and gun violence interact on asthma and total IgE in school-aged Puerto Rican children. METHODS: This is a case-control study of 747 Puerto Rican children aged 9 to 14 years living in San Juan, Puerto Rico (n = 472), and Hartford, Connecticut (n = 275). Exposure to gun violence was defined as the child's report of hearing gunshots more than once, and the percentage of African ancestry was estimated using genome-wide genotypic data. Asthma was defined as parental report of physician-diagnosed asthma and wheeze in the previous year. Serum total IgE (IU/mL) was measured in study participants. Multivariate logistic and linear regressions were used for the analysis of asthma and total IgE, respectively. RESULTS: In multivariate analyses, there was a significant interaction between exposure to gun violence and African ancestry on asthma (P = .001) and serum total IgE (P = .04). Among children exposed to gun violence, each quartile increase in the percentage of African ancestry was associated with approximately 45% higher odds of asthma (95% CI, 1.15-1.84; P = .002) and an approximately 19% increment in total IgE (95% , 0.60-40.65, P = .04). In contrast, there was no significant association between African ancestry and asthma or total IgE in children not exposed to gun violence. CONCLUSIONS: Our results suggest that exposure to gun violence modifies the estimated effect of African ancestry on asthma and atopy in Puerto Rican children.

BACKGROUND:: Chronic urticaria is characterized by transient, pruritic lesions of varying sizes, with central pallor and well-defined edges, with disease duration longer than six weeks. Its cellular infiltrate consists of neutrophils, lymphocytes and eosinophils. There is a subgroup of patients with eosinophilic or neutrophilic urticaria, resistant to the treatment with antihistamines, but that respond to a combination of antihistamine with other drugs. OBJECTIVE:: To evaluate the present infiltration in chronic urticaria biopsies and correlate it with the clinical disease activity and response to treatment. METHODS:: Forty-one patients with chronic urticaria were classified according to the score of severity of the disease, response to treatment and type of perivascular infiltrate. Inflammatory infiltrates were divided in eosinophilic (46.30%), neutrophilic and mixed. RESULTS:: An association was found between the eosinophilic infiltrate and clinical scores of greater severity (p = 0.002). CONCLUSION:: This association shows that the eosinophilic inflammatory infiltrates denote high clinical activity, which means more severe and exuberant clinical pictures of the disease.

OBJECTIVE: To describe the clinical picture, test results, and clinical evolution of patients with cerebral palsy associated with diagnosis of eosinophilic esophagitis, monitored at tertiary centre. METHODS: Cross-sectional, retrospective and descriptive study that evaluated the medical records data of pediatric patients with diagnosis of cerebral palsy and eosinophilic esophagitis in a tertiary center of pediatric gastroenterology between August 2005 and August 2013. RESULTS: Seven out of 131 patients with cerebral palsy had the diagnosis of eosinophilic esophagitis. The mean age at diagnosis of eosinophilic esophagitis was 52.3 months and the mean number of eosinophils in esophagus was 35 per high-power field. Symptoms more frequent were recurrent vomiting and disphagia. Endoscopic alterations found were mucosal thickening, vertical lines, mucosal opacificacion and white plaques. CONCLUSION: The frequency of eosinophilic esophagitis found was higher than in general pediatric population. The investigation of eosinophilic esophagitis should be done regularly in those patients, once this entity could overlap other gastrointestinal diseases.

The aim of the present study was to investigate the effect of a protein-free diet in the induction of food allergy and oral tolerance in BALB/c mice. The experimental model used was mice that were fed, since weaning up to adulthood, a balanced diet in which all dietary proteins were replaced by amino acid diet (Aa). The absence of dietary proteins did not prevent the development of food allergy to ovalbumin (OVA) in these mice. However, Aa-fed mice produced lower levels of IgE, secretory IgA and cytokines. In addition, when compared with mice from control group, Aa-fed mice had a milder aversive reaction to the allergen measured by consumption of OVA-containing solution and weight loss during food allergy development. In addition, mice that did not have dietary proteins in their diets were less susceptible to induction of oral tolerance. One single oral administration was not enough to suppress specific serum Ig and IgG1 levels in the Aa-fed group, although it was efficient to induce suppression in the control group. The present results indicate that the stimulation by dietary proteins alters both inflammatory reactivity and regulatory immune reactivity in mice probably due to their effect in the maturation of the immune system.

Los individuos alérgicos a látex pueden presentan reacciones mediadas por inmunoglobulina E (IgE) principalmente a frutas como cambur (banana, plátano), aguacate (palta), castaña y kiwi, lo cual se conoce como síndrome látex-frutas (SLF). Conocer la prevalencia del síndrome látex-frutas en una población conformada por los estudiantes de postgrado de la Facultad de Odontología de la Universidad Central de Venezuela (UCV) previamente diagnosticados con hipersensibilidad tipo I al látex. Se realizó un cuestionario a 27 participantes para conocer los antecedentes de atopia y reacciones alérgicas a frutas y vegetales así como también los factores de riesgo. También se diagnosticó la hipersensibilidad tipo I a aguacate, cambur, kiwi y tomate, mediante la prueba cutánea por técnica de punción superficial como prueba in vivo y un método ELISA utilizando el ENEASystem III como prueba in vitro. Estas dos pruebas diagnósticas arrojaron un total de 11/27 (41%) participantes con el síndrome látex-frutas. Referente a los factores considerados como de riesgo, 6 (55%) tenían antecedentes de atopia, 4 (36%) reportaron antecedentes familiares de alergia y 8 (73%) habían sido sometidos a intervenciones quirúrgicas. En este estudio se encontró una alta prevalencia del síndrome látex-frutas en la población estudiada. Los datos obtenidos sugieren un comportamiento dependiente entre la atopia y los otros factores de riesgo y el síndrome látex-frutas.


The individuals who are allergic to latex reactions are mediated by immunoglobulin E (IgE) mainly of fruit such as banana, avocado, chestnuts and kiwi, which is known as latex-fruit syndrome (LFS). To determine the prevalence of the latex-fruit syndrome in a group of dentists at the School of Dentistry at the Central University of Venezuela (UCV) previously diagnosed with type I hypersensitivity to latex. We conducted a questionnaire to find out the history of atopia, symptoms and allergic reactions to fruits and vegetables and the risk factors. Also we diagnosed type I hypersensitivity to avocado, banana, kiwi and tomato through the skin test technique of "prick by prick " as in vivo test and an ELISA method using the ENEASystem III as evidence in vitro. These two diagnostic tests yielded a total of 11/27 (41 %) participants with the latex-fruit syndrome. Concerning the factors considered as risk, 6 (55 %) had atopia, 4 (36 %) family history of allergy and 8 (73 %) had surgical interventions. This study found a high prevalence of the latex-fruit syndrome in the group studied. The data obtained suggest a behavior dependent between the atopia and other factors of risk and the syndrome latex - fruit.

Problema: no existen informes en Colombia sobre los niveles de IgE anti-Plasmodium en niños sin malaria y apenas hay uno en niños con malaria no complicada. Objetivo: medir la IgE anti-Plasmodium en niños sin malaria o con ella en función del sexo, el estado nutricional y la presencia de helmintos intestinales, y las relaciones con variables hematológicas. Metodología: la IgE anti-Plasmodium se midió en 335 niños sin malaria y en 125 con malaria no complicada (116 P. vivax, 9 P. falciparum). La medición se hizo con la técnica de ELISA, con antígeno de extracto crudo de P. falciparum, cepa FCB-2. Se usó el conjugado anti-anti-IgE de Sigma A3525. La IgE se midió y expresó según la densidad óptica (absorbancia). Resultados: la IgE anti-Plasmodium fue de 0,808 ± 0,508 en niños sin malaria (punto de corte usado para definir nivel alto: 0,584) y 1,968 ± 1,237 en niños con malaria (p = 0,000000). La IgE anti-Plasmodium no mostró diferencia significativa según la especie plasmodial, pero sí según el sexo y el estado nutricional de los niños. En niños sin malaria, el nivel de IgE anti-Plasmodium fue mayor en aquellos con presencia de áscaris, tricocéfalos y uncinarias, pero la diferencia fue significativa únicamente para áscaris. El nivel de IgE anti-Plasmodium mostró correlación lineal positiva y significativa con la cantidad de huevos de tricocéfalo y de áscaris por gramo de materia fecal. Conclusiones: los niños sin malaria o con ella de Urabá tienen elevados niveles de IgE anti-Plasmodium, que son más altos en mujeres y en los afectados por paludismo.


Problem: We are not aware of studies in Colombia on levels of specific IgE in children without malaria and only one is available in children with uncomplicated malaria.Objective: To measure anti-Plasmodium IgE in children with or without malaria according to sex, nutritional status, and the presence of intestinal helminthes, and the relations with hematologic variables. Methodology: anti-Plasmodium IgE was measured in 335 children without malaria and in 125 with uncomplicated malaria (P. vivax: 116; P. falciparum: 9). Measurement was done with the ELISA technique, with a crude antigen extract of P. falciparum FCB-2 strain. We used Sigma`s conjugated anti-anti-IgE A3525. IgE was measured and expressed as optical density (absorbance). Results: Anti-Plasmodium IgE in children without malaria was 0.808 ± 0.508 (cutoff point used to define high level: 0.584) and 1.968 ± 1.237 in children with malaria (p = 0.000000). Anti-Plasmodium IgE levels showed no significant difference according to plasmodial species but there was significant difference by sex and nutritional status. In children without malaria, the level of anti-Plasmodium IgE was higher in those with the presence of roundworm, whipworm and hookworm, but the difference was significant only for roundworms. The level of anti-Plasmodium IgE showed significant positive linear correlation with the number of eggs of Ascaris and whipworm per gram of stool. Conclusions: In Urabá, children with or without malaria have elevated levels of anti-Plasmodium IgE, that are higher in women and in those affected by malaria.

BACKGROUND: Atopic Dermatitis is a chronic inflammatory skin disease. Food allergens are important in the pathogenesis in 1/3 of the cases. Several mechanisms are involved in the pathogenesis of Atopic Dermatitis. Immediate reactions are identified by both measurement of specific IgE and skin prick test. Atopy Patch Test seems to be relevant in the investigation of patients with suspected delayed-type reactions. OBJECTIVES: To evaluate the standardization of this method concerning allergen concentration, occlusion time and interpretation, and determine the specificity and sensitivity of the Atopy Patch Test according to the skin prick test and specific IgE levels in food allergy diagnosis in children with Atopic Dermatitis. METHODS: Seventy-two children, aged 2-12 years were selected and followed at the allergy clinic of the Hospital São Zacharias. Skin prick test, specific IgE and food Atopy Patch Test (cow's milk, egg, soy and wheat) were carried out. Three groups were submitted to the Atopy Patch Test: (1) Atopic Dermatitis with or without Rhinitis and Asthma; (2) Rhinitis and or Asthma without AD; (3) Healthy individuals. RESULTS: In group 1, 40% of the patients presented positive reactions. The longer the exposure time (48h and 72h), the higher the sensitivity. In group 2, the test was more specific than sensitive for all the extracts, with increased sensitivity the longer the time of exposure (72h). In group 3, 8.3% presented positive tests. CONCLUSION: APT evidenced a great diagnostic value in late-phase reactions to food, with high specificity. It showed to be a specific and reliable tool in comparison with the healthy group's results.

FUNDAMENTOS: A Dermatite Atópica é uma doença inflamatória crônica da pele. Os alimentos são importantes na patogênese da doença em 1/3 dos casos. Diversos mecanismos estão envolvidos na fisiopatogenia da dermatite Atópica. As reações imediatas são identificadas pela dosagem de IgE específica e teste de puntura. O teste de contato atópico parece ter relevância na investigação de pacientes com suspeita de reação tardia. OBJETIVOS: Avaliar a padronização do método com relação à concentração do alérgeno, tempo de oclusão e de interpretação; e determinar a especificidade e a sensibilidade do teste de contato atópico em relação ao teste de puntura e a dosagem de IgE específica, no diagnóstico de alergia alimentar em crianças com dermatite Atópica. MÉTODOS: Setenta e duas crianças com 2 a 12 anos foram submetidas a teste de puntura e dosagem de IgE específicas para alimentos (leite de vaca, ovo, soja, trigo). O teste de contato atópico foi aplicado em 3 grupos: (1) Dermatite Atópica com ou sem Rinite e Asma; (2) Rinite e ou Asma sem Dermatite Atópica; (3) Saudáveis. RESULTADOS: No grupo 1, 40% dos pacientes apresentaram reação positiva. Quanto maior o tempo de exposição, maior foi a sensibilidade. No grupo 2, o teste foi mais específico que sensível para todos os extratos; com aumento da sensibilidade com maior tempo de exposição (72h). No grupo 3, 8.3% apresentaram testes positivos. CONCLUSÃO: O teste de contato atópico mostrou ter valor diagnóstico em relação às reações de fase tardia a alimentos, com elevada especificidade. Mostrou-se um teste específico e confiável ao comparar com os resultados do grupo controle.

La hipótesis higiénica intenta dar explicación coherente al incremento del asma en países desarrollados. Diversos estudios centran su atención en el papel que desempeñan las inmunizaciones sobre el sistema inmune y la influencia que ejercen sobre la atopia. El objetivo de este trabajo fue determinar las diferencias entre las concentraciones séricas de anticuerpos contra el antígeno de superficie del virus de la hepatitis B (anti-AgsHB), IgE total (IgE) y área de la prueba cutánea con el antígeno (Ag) vacunal, de la vacuna cubana contra la hepatitis B, Heberbiovac-HB, en niños asmáticos y no asmáticos. Se evaluó la influencia de la gravedad del cuadro clínico del asma sobre la respuesta inmune frente al Ag: se realizó estudio prospectivo y transversal, constituido por 103 niños asmáticos atendidos en el Hospital Infantil Docente William Soler y 37 no asmáticos de un área de salud del Centro de Atención Primaria "Julián Grimau" de la Ciudad de La Habana, Cuba. Todos los niños fueron vacunados con la vacuna Heberbiovac-HB, según esquema cubano de vacunación. Se realizó el test de ANCOVA para determinar las diferencias entre ambos grupos. En el grupo de niños asmáticos, se realizó un test de Duncan pos-hot, para establecer las diferencias, según la severidad del asma. Las diferencias significativas se centraron en las pruebas cutáneas, siendo menor el área de la prueba cutánea en asmáticos con respecto a los no asmáticos; la prueba cutánea control (PCC) y la prueba cutánea vs. antígeno vacunal de la hepatitis B (PCHB), PCC (166,5 vs. 233,6 mm2; p= 0,0077) y la PCHB (1 020,4 vs. 1 318,15 mm2; p= 0,0173). La severidad del cuadro clínico en asmáticos se relacionó de manera directa con las concentraciones de IgE e inversa con el área de la PCHB. Los niños asmáticos respondieron con menor área en la prueba cutánea retardada al compuesto vacunal, que los no asmáticos. La respuesta se deterioró a medida que se incrementó la gravedad del asma y las concentraciones de IgE.


The hygienic hypothesis tries to give a coherent explanation to the increase of asthma in the developed countries. Several studies focus their attention on the role played by immunizations in the immune system and on the influence they exert on atopy. The objective of this paper was to determine the differences among the serum concentrations of antibodies against the hepatitis B surface antigen (anti-AgsHB), the total IgE (IgE) and the area of the skin test with the vaccinal antigen (Ag) of the Cuban vaccine against hepatitis B, Heberbiovac-HB, in asthmatic and non-asthmatic children. The influence of the severity of the clinical picture of asthma on the immune response against Ag was evaluated. A prospective and cross-sectional study that included 103 asthmatic children attended at "William Soler" Children Teaching Hospital and 37 non-asthmatics from a health area of "Julián Grimau" Primary Care Center in Havana City, Cuba, was conducted. All the children were vaccinated with the Heberbiovac-HB vaccine according to the Cuban vaccination scheme. ANCOVA test was made to determine the differences between both groups. In the group of asthmatic children, Duncan's post-host test was carried out to establish the differences according to asthma severity. The significant differences were centered on the skin tests. The area of the skin test in asthmatics was lower than that of the non-asthmatics; the control skin test (CST) and the skin test against the hepatitis B vaccine antigen (STHB), CST (166,5 vs. 233,6 mm2; p=0,0077) and STHB (1 020,4 vs. 1 318,15 mm2; p=0,0173). The severity of the clinical picture in asthmatics was directly related to the IgE concentrations and inverse to the area of STHB. The asthmatic children responded with less area to the vaccinal compound in the delayed skin test. The answer deteriorated as the severity of asthma and the IgE concentrations increased.
