RESUMEN Se presenta a un paciente que ingresó en el antiguo Hospital Provincial Clínico Quirúrgico Docente "José Ramón López Tabrane", de Matanzas. El motivo del ingreso fue una proptosis ocular derecha de instauración súbita en el curso de la infección-enfermedad por VIH-sida. Se le realizaron múltiples estudios para poder definir la causa, y, pese a imponer tratamiento oportuno, falleció a los 15 días del mismo. La epidemia de sida continúa creciendo mundialmente. El conocimiento de las manifestaciones oculares; tanto las alteraciones del segmento anterior como las del posterior, y las infecciones asociadas, permiten mayor posibilidad de preservar la función visual en estos pacientes (AU).


ABSTRACT We present the case of a patient who entered the former Provincial Teaching Clinical Surgical Hospital "José Ramón López Tabrane", of Matanzas. The cause of admission was a sudden right ocular proptosis in the course of an HIV-AIDS infection-disease. Many different studies were carried out to define the cause, and although he was timely treated, he died 15 days after the treatment. The AIDS epidemic is still growing around the world. The knowledge of ocular manifestations, the alterations of the anterior segment as much as the alterations of the posterior one, and the associated infections allow a higher possibility of preserving the visual function in these patients (AU).

OBJECTIVE: To review and determine the attributed causes of deaths among patients who died in Sekuru Kaguvi Hospital (SKH) Eye Unit Wards between August 1984 and October 2008 STUDY DESIGN: Retrospective study of completed BD12 forms and case records of inpatients who died betweenAugust 1984 and October 2008. SETTING: Sekuru Kaguvi Hospital Eye Unit, Parirenyatwa Group ofHospitals, Harare, Zimbabwe. RESULTS: Eighty three deaths occurred in the unit during the 24 year period, 51% were females and 33.7% were children below 13 years of age. Ninety three percent of deaths among children were attributed to neoplasias, while 22% of deaths among adults were attributed to infections. Non-ophthalmic causes of deaths were seen among adults. CONCLUSION: Malignancies are the commonest attributable causes of mortality in the Ophthalmic Wards at Sekuru Kaguvi Hospital Eye Unit. Resource mobilization is essential to improve Eye Care Services.

PURPOSE: The purposes of this study are to determine the incidence of infectious opportunistic choroiditis in patients with the acquired immune deficiency syndrome (AIDS), to study the association of these choroidal infections with systemic dissemination, and to investigate the life expectancy and cause of death in patients with infectious opportunistic choroiditis. METHODS: A total of 470 eyes of 235 consecutive autopsies of patients with AIDS were examined by histopathologic methods. The clinical charts and autopsy reports of these patients were subsequently reviewed for presence of systemic dissemination of various infectious agents. RESULTS: Of the 235 patients, 18 were found to have infectious choroiditis. The etiologic agents found were: Cryptococcus neoformans, Pneumocystis carinii, Mycobacterium tuberculosis, Histoplasma capsulatum, Candida, Aspergillus fumigatus, Toxoplasma gondii, and Mycobacterium avium-intracellulare. In 15 of these 18 patients, the cause of death was considered to be due to systemic dissemination of the organism causing the choroiditis. Only 4 of the 18 cases of infectious choroiditis were diagnosed during life, and the survival time of these patients after diagnosis was only 25 days. Five of the 18 patients also were found to have cytomegalovirus (CMV) infection of the retina. CONCLUSION: Multifocal choroiditis due to endogenous infectious emboli in patients with AIDS reflects systemic dissemination and localization of infectious agents predominantly in the choriocapillaris. Early diagnosis and treatment are imperative and may be life-saving.
