RESUMO
Trigeminal neuralgia, also called tic douloureux, is a commonly misdiagnosed disorder characterized by intense facial pain. It is a chronic pain disorder that affects the fifth cranial nerve, usually in the fifth to seventh decade of life. Medication is the first line of treatment but frequently fails over time. At that point, many patients seek surgical intervention. This study reviews 108 patients treated over a 1-year period at the University of Florida with one of two surgical procedures: radiofrequency lesioning and microvascular decompression. The short-term results of this experience are reviewed and discussed.

Classic trigeminal neuralgia (TN) was described as early as the first century AD, when treatments ranged from bloodletting to the application of poison-laden bandages. Current medical treatment utilizes a variety of drug therapies. Surgery is reserved for those who are unable to tolerate the side effects or in situations in which the drugs prove to be ineffective. Surgical options include microvascular decompression, percutaneous balloon compression, radio-frequency thermocoagulation, and glycerol injection. Nurses play a critical role in the multidisciplinary care of patients undergoing surgical procedures for TN.

A coordinated care model was developed on a neuroscience unit to achieve positive outcomes in a cost-effective environment. This included the development of a patient care coordinator (PCC) role, critical paths and a system for variance tracking. The PCC was responsible for coordinating care of patients and ensuring that patients progressed toward expected outcomes. Multidisciplinary critical paths were developed for four medical diagnoses. To evaluate the effectiveness of the program, an analysis of length of stay data, cost comparison, patient and staff satisfaction, and variance reports of one critical path, the microvascular decompression for trigeminal neuralgia were completed. Results from the pilot project were positive and provided valuable information for the use of coordinated care as a hospital-wide patient care delivery model.

Patients who have undergone unsuccessful medical treatment of trigeminal neuralgia may require some form of neurosurgical intervention. Injecting glycerol into the trigeminal cistern via the anterior percutaneous route has gained increasing acceptance as a method of relieving the excruciating paroxysmal pain of trigeminal neuralgia. The purpose of this article is to provide neuroscience nurses with an understanding of the rationale for selection of this particular procedure over other methods. The technique and potential complications will be briefly described. Nursing implications for the perioperative period will be discussed.
