RESUMO

A 21-year-old man was admitted to hospital because of recent anosmia and liquorrhoea. He also complained of moderate headache and concentration problems in the past few years. On CT scan and MRI scans a big subfrontal process was seen, partially solid and partially cystic. Neurosurgical and histological findings proved that the lesion was an osteoma of the anterior skull base, concomitant with an intradurally extending mucocele.

RESUMO

Osteomas of the temporal bone are benign neoplasms that may be encountered by otolaryngologists. Clinically they should be distinguished from exostoses, which involve the external auditory meatus and are a well recognized entity. Osteomas involving the middle ear and ossicles are extremely rare. There is only one case report in the literature of an osteoma involving an ossicle and in that patient, who presented with conductive loss, the incus was involved. The present report presents a 48-year-old white male, who on routine examination was found to have a mass in his left tympanic membrane. Under local anesthesia the mass was totally excised, after it had been separated from the umbo. Histopathologic sections of the mass revealed a benign osteoma. A brief review of osteomas and exostoses of the temporal bone is presented.

RESUMO

Osteoma is a slow-growing tumor formed by mature bone tissue. In the temporal bone, intracanalicular osteomas are more frequent than extracanalicular osteomas. In Spain, only twelve cases of extracanalicular tumors of the temporal bone have been described in the literature. We present three new cases of extracanalicular osteoma (mastoids). The clinical and radiological features of each case are described and the relevant literature is reviewed.

RESUMO

The histologic and ultrastructural features of a tibial lesion strongly resembling cementifying fibroma of the jaws are described. Histologically the lesion consisted of fibrous tissue containing small calcified spherules. In some areas acellular trabeculae or islets with lobulated margins of a cementlike substance occurred. Spherules and small calcified bodies were composed of apatite crystal depositions along the collagen fibers and were often in close contact with membrane-surrounded cytoplasmic processes resembling matrix vesicles. In fibrous tissue fibroblasts, myofibroblasts, and undifferentiated cells with numerous cytoplasmic processes and intracytoplasmic microfibrils were present. The character of the cellular component of this lesion and the presence of calcified spherules arranged in a manner rarely observed in fibrous dysplasia indicate a histogenic relationship between the two lesions.

En el presente trabajo se estudiaron un total de 35 casos de teratomas ováricos, los cuales se recibieron fijados en formalina al 10%, previamente diagnosticados por el patólogo, como quistes dermoides benignos del ovario. Un total de 7 casos presentaron estructuras mineralizadas especificamente hueso. En lo que respecta a tejido óseo en el inferior de estos tumores, se observa con relativa frecuencia, hueso haversiano, sin embargo se observó la presencia también de abundante hueso compacto, muchas veces de láminas paralelas, en algunos casos se aprecia hueso de aspecto grumoso y en otros hueso con zonas de hipomineralización o mal mineralizado. La presencia de tejido osteide fue escasa, así como también los osteoblastos; un posible osteoclasto se observó en uno de los casos. Se encontraron dos casos con presencia de hueso cercano a estructuras dentarias, lo cual indica la existencia de posible hueso alveolar y en consecuencia perteneciente a mandíbula. Con respecto al cartílago observado mostró un aspecto normal en algunos casos con grupos isógenos coronarios, pero en otros casos se observó la presencia de hipertrofía lacunar semejante a la encontrada en osificación de algunos huesos de la cara y huesesillos del oído

Among a series of 42 ossifying fibromas and 32 fibrous dysplasias, 5 cases were studied by histoenzymological and electron microscopic methods. The histological study in 2/3 of cases is sufficient for the diagnosis between the two diseases: trabecular bone in a connective tissue with regular collagen fibres in the ossifying fibroma; nodular indented bony areas with irregular collagen frame in fibrous dysplasia. By histoenzymology, the diagnosis between the two affections is not easy: same activity of ATPases and alkaline phosphatases in fibroblasts and osteoblasts. By electron microscopy, the morphology of the two lesions is different: in ossifying fibroma, numerous well-differentiated osteoblasts and large areas of ossification are seen; in fibrous dysplasia, undifferentiated cells are numerous and the collagen frame is irregularly mineralized. This method is also useful for the histogenetic understanding of these two osteopathies.

Extracellular matrix vesicles from bovine fetal alveolar bone and from a dog osteosarcoma were isolated by differential centrifugation and then fractionated on a discontinuous sucrose density gradient. The fractions were examined by electron microscopy and were analyzed for protein, alkaline phosphatase, aminotripeptidase, and four different beta-naphthylamidase activities. The low-density peak of enzyme activities was shown by electron microscopy to be much more homogeneous than the crude matrix vesicle fraction. Two major peaks of protein and enzyme activities were present, one in the high and one in the low density layers. There was good correlation between the activities of alkaline phosphatase and the various peptidases in the fractions from the sucrose density gradient. These results indicate a coexistence of peptidase and alkaline phosphatase in matrix vesicles. On the other hand, there was generally no correlation between the peptidase and alkaline phosphatase activities in vesicular specimens from bovine liver obtained in the same way. Most of the peptidase activity and about half of the alkaline phosphatase activity were solubilized from bone matrix vesicles by detergents. The extracted alkaline phosphatase and alanyl beta-naphthylamidase activities were separated from each other on a DEAE-cellulose column.

RESUMO

Electron microscopic investigation of 16 cases of human bone tumor revealed the presence of intranuclear vermicellar bodies (IVB) in cells of two osteosarcomas and one ossifying fibroma. The spherical nuclear inclusions consisted of electron-dense, interwoven, threadlike structures measuring approximately 20-30 nm in diameter. The nature and significance of the IVB remain obscure at present.

Osteomas and exostoses have distinct clinical and histopathologic features. Osteomas are usually solitary, pedunculated, bony growths attached to the tympanosquamous or tympanomastoid suture line, characterized histologically by an internal structure of abundant discrete fibrovascular channels surrounded by irregularly oriented lamellated bone. Exostoses are usually multiple, bilaterally symmetrical, broad based elevations of bone involving the tympanic bone. They are histologically characterized by parallel, concentric layers of subperiosteal bone. The infrequent symptoms resulting from these lesions can usually be managed medically; however, on occasion surgical removal is indicated. While surgical removal of the osteoma is usually possible via the external auditory meatus, we recommend that an exostosis be removed utilizing a postauricular approach.

RESUMO

Osteomas of the external auditory canal are considered clinically to be discrete, pedunculated bone lesions arising along the tympanosquamous suture. They are benign lesions but often are slowly progressive in size. Exostoses of the external auditory canal are broad-based elevations of bone usually multiple and bilaterally symmetric, involving the tympanic bone. It appears that both clinical and histopathologic evidence is sufficient to justify the impression that these two lesions should be considered separate entities and clinical diagnostic and histopathologic criteria have been suggested for diagnosis of these lesions.
