Background: Ischemic neuromyopathy is the most common reason for amputation in cats. In veterinary medicine, the use of prosthetic limbs is not widespread; therefore, in most cases total limb amputation is indicated. However, hyperbaric oxygen therapy (HBOT) is an alternative with several benefits for the treatment of vascular disorders with reperfusion, ischemia, and infection. Therefore, this study aimed to report the positive effects of HBOT on the treatment of ischemic neuromyopathy secondary to arterial thromboembolism on the patient's clinical improvement, and on the preparation of the patient for insertion of an osseointegrated prosthesis. Case: A 6-month-old mixed-breed kitten returned for treatment after undergoing surgery seven days earlier for reduction of traumatic diaphragmatic hernia, during which it suffered a cardiorespiratory arrest. The patient presented with acute pelvic limb paralysis with 24-h evolution, absent femoral pulse, plantar cushions and dorsal part of the limbs cold and pale. After supportive therapy and diagnosis of aortic thromboembolism by arterial Doppler, the patient started adjunctive treatment with HBOT from the first day of hospitalization. Sessions took place in an exclusive hyperbaric chamber for animals and lasted 60 min at a pressure of 2.5 absolute atmospheres and 100% oxygen, initially every 12 h. However, during the first 5 days of hospitalization, the distal region of both pelvic limbs began to show tissue devitalization and edema, and hematologic parameters showed changes on the 7th day. The right pelvic limb (RPL) showed more involvement of superficial tissues, extending to the tarsometatarsal joint region. After 8 days of hospitalization, the devitalized tissue was debrided. The RPL had an extensive devitalized area with exposed bone in the phalanges and necrosis in the pads. The left pelvic limb (LPL) suffered minor complications, with involvement of the phalangeal region. After 12 days, with HBOT every 48 h, exuberant granulation tissue was observed. After 17 days, the patient was discharged, and HBOT sessions were performed weekly. Gangrene of the midfoot and lack of proprioception were observed in RPL, while LPL showed bone divulsion of the 1st, 3rd, and 4th phalanges. Because of the poor prognosis for limb viability, the RPL was partially amputated, and a self-threaded intraosseous prosthesis was inserted. Discussion: The cardiorespiratory arrest that occurred during the surgical procedure to reduce the diaphragmatic hernia without thromboprophylaxis may have contributed to the peripheral ischemia. HBOT was proposed for the adjuvant treatment of ischemic injury because it is especially indicated for cases of ischemia-reperfusion injury. The main hematological parameters were evaluated at an average interval of 7 days. While the platelet count and hematocrit increased, the leukocytosis decreased. This demonstrates the benefit of oxygen therapy in the reported patient. The use of HBOT in orthopedic injuries is known to result mainly in stimulation of osteoblasts, promoting osseointegration of the prosthesis. We conclude that the adjuvant treatment with HBOT helped to preserve a large segment of both pelvic limbs, prevent the progression of necrosis, and provide a healthy bed for fixation of an osseointegrated prosthesis in the RPL, resulting in clinical improvement of the patient.

Na rotina veterinária, as lesões nas extremidades dos membros são constantes e representam um desafio especial pelas características anatômicas, susceptibilidade de exposição óssea e limitações de reparos e reconstruções quando comparado a outras regiões do corpo. A osteomielite, decorrente de mordeduras, tem alta prevalência e sua rápida evolução pode levar a medidas drásticas, como a amputação. Neste trabalho, é relatada a utilização da oxigenoterapia hiperbárica como terapia integrativa ao tratamento convencional e a avaliação de seus efeitos durante as 12 sessões realizadas em uma cadela vítima de mordedura, que apresentava necrose e exposição óssea com colonização por Staphylococcus sp e Pasteurella multocida. A avaliação evolutiva do quadro foi elaborada a partir de registro fotográfico da lesão, com escala, a cada três sessões e submetido à análise morfométrica por software analítico. A velocidade da marcha cicatricial revelou que a utilização desta terapia propiciou precocidade e efetiva atuação na cicatrização e no combate à osteomielite, evitando a amputação do restabelecendo as liberdades da paciente, contribuindo ao aprofundamento do emprego da oxigenoterapia hiperbárica na Medicina Veterinária.(AU)
In the veterinary routine, injuries in the extremities of the limbs are constant and represent a special challenge due to the anatomical characteristics, susceptibility of bone exposure and limitations of repairs and reconstructions when compared to other regions of the body. Osteomyelitis resulting from bites is highly prevalent and its rapid evolution can lead to drastic measures such as amputation. This work describes the use of hyperbaric oxygen therapy as an integrative therapy to conventional treatment and the evaluation of its effects during the 12 treatments performed in a female dog victim of a bite, that presented evolution to necrosis and bone exposure with colonization by Staphylococcus sp e Pasteurella multocida. The evolutionary assessment of the condition was elaborated from a photographic record of the lesion, with scale, every three treatments and submitted to morphometric analysis by analytical software. The speed of the healing gait demonstrated, that the use of this therapy proved precocity and effective action in healing and in the fight against osteomyelitis, avoiding the amputation of the limb, restoring the patient's freedoms. Thus contributing to the deepening on the use of hyperbaric oxygen therapy in veterinary medicine.(AU)

At the 5th session was identifiedleukopenia, neutropenia and lymphopenia. Wound re-epithelialization was initiated after the 5th session, and the completeepithelialization was identified at the 12th session of HBOT. During the HBOT no side effects were identified. We investigated the local and systemic effects of hyperbaric oxygen therapy in BALB/C mice, exposed to two different exposure times, under 2.4 atmosphere (ATM). Fifteen animals were divided into three groups (GI, GII and Control) and underwent a surgical excision of a skin fragment of approximately one square centimeter of the dorsal region. The wounds were treated and monitored for 21 days. In the control group, the wound was cleaned once a day with sterile 0.9% NaCl solution. GI and GII mice were submitted to daily hyperbaric oxygen therapy of 30 or 60minutes sessions, respectively. The wounds were photographed every three days and their surfaces were analyzed by an image analyzer. At 21 days, all animals were euthanatized for histopathological analysis of the skin, lungs and liver in order to identify eventual alterations in wound healing or in the analyzed organs. Animals belonging to GI showed a faster skin wound healing in comparison to the other groups. Animals from GII, however, showed a delayed wound healing process and exhibited lung and microcirculatory alterations. These findings allow us to conclude that the exposure time to the oxygen in hyperbaric environment is crucial and can help or disturb skin wound healing or even be deleterious to other organs.(AU)
Investigaram-se os efeitos locais e sistêmicos da oxigenoterapia hiperbárica em camundongos BALB / C, submetidos a dois tempos de exposição diferentes, sob atmosfera 2,4 (ATM). Quinze animais foram divididos em três grupos (GI, GII e controle) e submetidos à excisão cirúrgica de fragmento de pele de aproximadamente um centímetro quadrado da região dorsal. As feridas foram tratadas e acompanhadas por 21 dias. No grupo controle, a ferida foi limpa uma vez ao dia, com solução estéril de NaCl 0,9%. Camundongos GI e GII foram submetidos à oxigenoterapia hiperbárica diária de 30 ou 60 minutos de sessões, respectivamente. As feridas foram fotografadas a cada três dias, e suas superfícies analisadas por um analisador de imagens. Aos 21 dias, todos os animais foram submetidos à eutanásia para análise histopatológica da pele, do pulmão e do fígado, em busca de eventuais alterações na cicatrização da ferida ou nos órgãos analisados. Animais pertencentes ao GI apresentaram cicatrização mais rápida de feridas cutâneas em comparação aos outros grupos. Já os animais do GII apresentaram retardo na cicatrização da ferida e alterações pulmonares e microcirculatórias. Esses achados permitem concluir que o tempo de exposição ao oxigênio em ambiente hiperbárico é fundamental e pode auxiliar ou atrapalhar a cicatrização de feridas cutâneas ou mesmo ser deletério para outros órgãos.(AU)

PURPOSE: To develop a project of hyperbaric chamber that allows its safe and reliable use in veterinary and animal experimentation. METHODS: Based on the technical specifications for the construction of hyperbaric chambers for human beings, it has been developed a design of a chamber with dimensions and characteristics for the use of a midsize animal, (dog or pig), as well as a multiple chamber for the use in small animals (mice, rats, hamsters, rabbits or cats). The technical specifications allowed that the chamber could be used both for veterinary use and for use in experiments on Health Sciences. RESULTS: A chamber with the following characteristics was built: ASTM A36 steel for the manufacture of the master cylinder and rear cover; front door built in 5052 aluminum; internal diameter of 50.5 cm and 83.0 cm in length; weight 160Kg and internal area of 150cm³; internal space to accommodate 2 acrylic baskets; 150mm high, 280mm wide and 690mm in length. It was capable of supporting a maximum of hydrostatic pressure test of 3.0 to 4.0 BAR ACT and maximum working pressure of 2.0 BAR or 3.0 ACT; equipped with security devices and valves that triggers with load of 2.2 BAR or 3.2 ACT. Tests for engineering and biological use on animals showed the effectiveness of the device. CONCLUSION: The development of the project enabled the construction of a hyperbaric chamber with security features and reliability comparable to those required by the legal and technical specifications of a hyperbaric chamber human use.
OBJETIVO: Desenvolver um projeto de câmara hiperbárica que permita o seu uso seguro e confiável em veterinária e em animais de experimentação de pequeno e médio porte. MÉTODOS: Baseados em especificações técnicas de construção de câmaras hiperbáricas de seres humanos, foi desenvolvido uma adaptação de projeto de uma câmara com dimensões e características para uso em animal de médio porte (cão ou porco), assim como câmara múltipla para animais de pequeno porte (camundongos, ratos, hamsters, coelhos ou gatos). O atendimento às especificações técnicas deve permitir a sua utilização tanto para uso veterinário quanto para uso em experimentação em Ciências da Saúde. RESULTADOS: Uma câmara com as seguintes características foi construída: aço ASTM A36 para a confecção do cilindro principal e tampo traseiro, tampo frontal (porta) construído em alumínio 5052, diâmetro interno de 50,5cm e 83,0cm de comprimento; peso de 160Kg e espaço interno de 150cm³; espaço para acomodar 2 cestas de acrílico de 150mm de altura, 280mm de largura e 690mm de comprimento; capacidade para suportar um máximo de pressão hidrostática teste de 3,0BAR ou 4,0 ACT e uma pressão máxima de trabalho de 2,0 BAR ou 3,0 ACT; equipado com dispositivos e válvulas de segurança que disparam com carga de 2,2 BAR ou 3,2 ACT. Os testes de engenharia e de uso biológico com animais mostraram a efetividade do equipamento. CONCLUSÃO: O desenvolvimento do projeto permitiu a confecção de uma câmara hiperbárica com características de segurança e confiabilidade comparável às exigidas pelas especificações técnicas e legais de uma câmara para tratamento hiperbárico.

PURPOSE: To analyze and to evaluate the effect of the hyperbaric hyperoxygenation in the mechanical resistance of ischemic colon of rats. METHODS: Eighty rats, distributed in four groups of 20 animals in each one, were used. In group 1 (G1), the control group, ischemia was not caused. Group 2 was submitted to the lesser degree of ischemia. Group 3 was submitted to the intermediate degree of ischemia. In group 4, a bigger degree of intestinal ischemia was provoked. Each group was divided in two sub-groups of ten animals each: with hyperbaric chamber (CC) and without hyperbaric chamber (SC). The animals of the four CC subgroups were placed in an experimental hyperbaric chamber in order to inhale oxygen at 100 percent, at two Absolute Atmospheres, for 120 minutes, for a four-day consecutive period. The animals of the four SC subgroups were kept in environment air during the five days of the experiment. All animals have been submitted to the mechanical study of the intestinal loop by the pressure test of the rupture by liquid distension. The euthanasia occurred in the fifth post-operative day. RESULTS: Considering the ischemia factor, the four groups were different among them (p=0.0001). There was no statistical difference between subgroups CC and SC (p=0.3461). CONCLUSION: The hyperbaric oxygen-therapy did not present improvement on the induced ischemia in rats upright colic loop.(AU)
OBJETIVO: Analisar e avaliar os efeitos da hiperoxigenação hiperbárica na resistência mecânica do cólon isquêmico de ratos. MÉTODOS: Foram utilizados 80 ratos distribuídos em quatro grupos de 20 animais. No grupo 1, grupo controle, não se provocou isquemia. O grupo 2 foi submetido ao grau menor de isquemia. O grupo 3 foi submetido ao grau intermediário de isquemia. No grupo 4, provocou-se grau maior de isquemia intestinal. Cada grupo foi dividido em dois subgrupos de dez animais cada: com câmara hiperbárica (CC) e sem câmara hiperbárica (SC). Os animais dos quatro subgrupos CC foram colocados em uma câmara hiperbárica experimental para inalarem oxigênio a 100 por cento, a duas atmosferas absolutas, durante 120 minutos, por quatro dias consecutivos. Os animais dos quatro subgrupos SC foram mantidos somente em ar ambiente. Todos animais foram submetidos ao estudo mecânico da alça intestinal pelo teste de pressão de ruptura pela distensão por líquido. A eutanásia ocorreu no quinto dia pós-operatório. RESULTADOS: Os quatro grupos são diferentes entre si considerando o fator isquemia (p= 0.0001). Não houve diferença estatística entre os subgrupos CC e SC (p= 0.3461). CONCLUSÃO: A oxigenoterapia hiperbárica não apresentou melhora sobre a isquemia induzida em alça cólica íntegra de ratos.(AU)
