RESUMO

AIMS AND OBJECTIVES: The aim of this study was to explore the consequences of the nurse's use of advanced assessment skills on medical and surgical wards. BACKGROUND: Appropriate, accurate, and timely assessment by nurses is the cornerstone of maintaining patient safety in hospitals. The inclusion of "advanced" physical assessment skills such as auscultation, palpation, and percussion is thought to better prepare nurses for complex patient presentations within a wide range of clinical situations. DESIGN: This qualitative study used a hermeneutic pragmatic approach. METHOD: Unstructured interviews were conducted with five experienced medical and surgical nurses to obtain 13 detailed narratives of assessment practice. Narratives were analyzed using Van Manen's six-step approach to identify the consequences of the nurse's use of advanced assessment skills. RESULTS: The consequences of using advanced assessment skills include looking for more, challenging interpretations, and perseverance. The use of advanced assessment skills directs what the nurse looks for, what she sees, interpretation of the findings, and her response. It is the interpretation of what is seen, heard, or felt within the full context of the patient situation, which is the advanced skill. CONCLUSION: Advanced assessment skill is the means to an accurate interpretation of the clinical situation and contributes to appropriate diagnosis and medical management in complex patient situations. RELEVANCE TO CLINICAL PRACTICE: The nurse's use of advanced assessment skills enables her to contribute to diagnostic reasoning within the acute medical and surgical setting.

INTRODUCTION: Endodontic diagnostic tests are often used clinically to assess pulp status as a basis for the diagnosis and determination of whether root canal treatment (RCT) is indicated. Response to cold and pain on percussion are 2 common tests, yet their validity in identifying nonvital pulp in regular dental practice has not been reported. METHODS: We assessed the validity of cold and percussion tests to identify nonvital pulp in teeth requiring RCT in a dental practice setting performed by 46 general dentists and 16 endodontists in the National Dental Practice-Based Research Network. The influence of patient-, tooth-, and dentist-related characteristics was investigated. Observed bleeding from the pulp chamber was the clinical reference. Sensitivity (SN), specificity (SP), overall test accuracy (TA), positive (PPV) and negative (NPV) predictive values, and likelihood and diagnostic odds ratios (LR+, LR-, dORs) were calculated for each single test and the combined cold and percussion tests. RESULTS: Seven hundred eight patient teeth were included. Cold test showed high validity to identify a nonvital pulp status (SN = 89%, SP = 80%, TA = 84%, PPV = 81%, NPV = 88%, LR+ = 4.35, LR- = 0.14, dOR = 31.4), whereas pain on percussion had lower validity (SN = 72%, SP = 41%, TA = 56%, PPV = 54%, NPV = 60%, LR+ = 1.22, LR- = 0.69, dOR = 1.78). Combining the 2 tests did not increase validity, whereas preoperative pain, medication intake, patient age and sex, and dentist training level affected test validity significantly. CONCLUSIONS: In regular dental practice, the cold test exhibits higher validity to discriminate between vital and nonvital pulp than the tooth percussion test.

STATEMENT OF PROBLEM: The detection of cracks and fractures in natural teeth is a diagnostic challenge. Cracks are often not visible clinically nor detectable in radiographs. PURPOSE: The purpose of this study was to evaluate the diagnostic parity of quantitative percussion diagnostics, transillumination, clinical microscopy, and dye penetration. MATERIAL AND METHODS: Three independent examiners provided blind testing for the study. Examiner 1 transilluminated 30 extracted teeth and 23 three-dimensional copy replica control teeth and documented any visible cracks. Each tooth was then mounted in acrylic resin with a periodontal ligament substitute. Examiner 2 examined each specimen aided by the clinical microscope and transillumination and documented visible tooth cracks and fractures. Examiners 1 and 3 then independently tested all specimens with a device developed for quantitative percussion diagnostics. All visible cracks/fractures were removed with a water-cooled fine diamond rotary instrument. Crack visibility was enhanced by the use of a clinical microscope, dye penetrant, and accessory transillumination. This disassembly process was video documented/photographed for each specimen. One more quantitative percussion diagnostics testing was administered when the disassembly was complete. RESULTS: Quantitative percussion diagnostics crack detection agreed with the gold standard microscope and transillumination method in 52 of 53 comparisons (98% agreement). Moreover, the method achieved 96% specificity and 100% sensitivity for detecting cracks and fractures in natural teeth. When all tooth cracks were removed, quantitative percussion diagnostics indicated no further structural instability. CONCLUSIONS: Quantitative percussion diagnostics can nondestructively detect cracks and fractures in natural teeth with accuracy similar to that of the clinical microscope, transillumination, and dye penetrant. In addition, the method was able to reveal the presence of many cracks that were not detected by conventional transillumination.

STATEMENT OF PROBLEM: Immediate loading of implants may be a predictable treatment alternative when cross-arch stabilization with a fixed provisional is observed. PURPOSE: This study investigated the effect of immediate masticatory loading on the stability of single-standing dental implants with 4 different surfaces. MATERIAL AND METHODS: A total of 40 solid screw implants (diameter 3.3 mm, length 8 mm) were placed in the mandibles of 4 beagle dogs. Test groups included 3 hydroxyapatite (HA) coatings of titanium plasma-sprayed (TPS) implants. Implants with TPS alone served as control. Gold crowns were inserted 2 days after implant placement and the dogs were immediately put on a hard food diet. Implants were followed for 6 months after loading. Clinical and radiographic assessments of implants were performed at time of crown insertion (baseline) and after 1, 3, and 6 months of loading. The Periotest instrument was used for mobility measurements and radiographs were obtained for evaluation of peri-implant radiolucency and measurement of crestal bone changes. RESULTS: Of 40 implants, 39 displayed no discernible mobility, corresponding to successful clinical function. Peri-implant radiolucencies were absent for all but the 1 mobile implant. The reduction in crestal bone levels adjacent to the implants between baseline and 6 months was statistically significant (P <.0001). No statistically significant differences in crestal bone level changes over time were found between the various coatings demonstrating the absence of a treatment effect initiated by the surface coatings. CONCLUSION: In this study in beagle dogs, immediate masticatory loading of single-standing dental implants did not jeopardize tissue integration, provided the implants had excellent primary stability.

STATEMENT OF PROBLEM: There are numerous articles regarding implant overloading, interfacial integrity and component failure. A significant amount of literature review supports the concept of the progressive loading and damping, and it should be addressed. PURPOSE: This study was designed to correlate durometer measurements of the elastomeric test materials and actual Periotest values (PTV) of the test materials as preloaded sleeve rings in Compliant Keeper abutments to support the Compliant Keeper as a viable implant damping system. MATERIAL AND METHODS: A 4-unit fixed-removable implant-supported partial denture was the test model. Sleeve ring combinations replaced "O" rings and 220 Periotest measurements were graphically recorded, then analyzed by least square of means, 2-way analysis of variance, and a Tukey test. RESULTS: There was a correlation between durometer measurements and PTVs of sleeve rings. Finite movement was measured accurately with the Periotest (P =. 0001) so that Compliant Keeper systems can be programmed to match the movement of another implant abutment, because they might be able to match the PTV of a natural abutment. CONCLUSIONS: Significant differences exist between the "O" rings and sleeve rings. The sleeve ring, as a controllable damping element, enables the Compliant Keeper system to provide progressive loading and damping for implant-supported prostheses.

Osseointegration is a histometric process that occurs gradually over a period of time. The load that an implant is able to bear depends upon, among other parameters, the quality of the bone-implant contact. For 8 years the damping capacity of 1,182 Brånemark implants inserted consecutively in 315 patients was measured using the Periotest method. The following clinical observations were made: (1) a relationship was found between implants with a specific Periotest value range that, at the moment of the transepithelial connection, were considered to be clinically stable, asymptomatic, and whose radiograph image was not radiolucent; (2) a relationship was found between clinically nonintegrated implants with a different Periotest value range; (3) variations in the Periotest value were related to the type of bone in which the implant was placed; (4) a small percentage of borderline implants with a Periotest value between the osseointegrated and the nonosseointegrated Periotest values was detected; (5) the percentage of secondary failures was related to an initial Periotest value corresponding to a borderline implant; (6) the healing time of each implant was individualized in accordance with the successively obtained Periotest value; (7) the load and the design of the prosthesis were individualized; (8) early detection of failing implants before fabrication of prostheses is possible; (9) communication was improved between the surgeon and the prosthodontist; and (10) variations in Periotest value were related to bone remodeling. The sensitivity of the principal clinical test in evaluating osseointegration is discussed regarding the moment of its application. The use of Periotest values as an initial success criteria of an implant is proposed.

There are several problems related to the diagnosis and treatment of the acute painful tooth associated with a deep carious lesion, including the fact that there is little or no correlation between the clinical signs and symptoms and also that the diagnostic tools are not reliable. It is important to understand the problems experienced by dental practitioners and how they routinely make diagnosis and formulate treatment plans. The way general dental practitioners diagnosed, categorised and treated teeth with acute pain was ascertained by a questionnaire completed by practitioners attending postgraduate courses. The results indicate that dental radiographs and the dental history were most frequently used and that few practitioners had, and fewer used electric pulp testers. The concept of using simple clinical categories which correspond to treatment categories does not seem to be in widespread use, though the generally accepted role of calcium hydroxide as an indirect and direct capping material is widespread. The use of corticosteroid materials is not as widespread in younger practitioners as in their older colleagues and the use of emergency pulpotomy is infrequent.
