PURPOSE: This multicenter, retrospective cohort study aimed to compare the risk of ulnar nerve injury in pediatric supracondylar humeral fractures treated with percutaneous lateral-pins, blinded-crossed-pins, and crossed-pins with a mini-incision. METHODS: Data were collected from 1705 children treated between January 2010 and December 2023 at four orthopedic centers in Colombia. The incidence of postoperative ulnar nerve injury was compared among three fixation techniques: lateral-pin, blinded-crossed-pin, and crossed-pin with a mini-incision. RESULTS: A statistically significant difference in nerve injury rates was observed between the lateral-pin and both blinded-crossed-pin and mini-incision crossed-pin techniques (p < 0.001), with the lateral-pin technique demonstrating a significantly lower risk of injury. No significant difference was found between the blinded-crossed-pin and mini-incision crossed-pin techniques (p = 0.67). CONCLUSION: Crossed-pin fixation was associated with a higher incidence of ulnar nerve injury, regardless of the use of a mini-incision. The lateral-pin technique remains the safest option for minimizing iatrogenic nerve injury. There is insufficient evidence to support the mini-incision as a safer alternative to traditional crossed-pin fixation.

PURPOSE: Infection at the pin site remains the most common complication of external fixators (EFs). It is known that hydroxyapatite (HA)-coated pins increase bone adhesion and may lead to reduced rates of reported infections. The present study compares the rates of pin track infection associated with stainless steel and HA-coated pins. METHODS: This is a prospective, multicenter, nonrandomized, comparative intervention study among patients undergoing surgical treatment with EFs of any type between April 2018 and October 2021. Patients were followed up until the removal of the EF, or the end of the study period (ranging from 1 to 27.6 months). The definition of pin track infection was based upon the Maz-Oxford-Nuffield (MON) pin infection grading system. RESULTS: Overall, 132 patients undergoing external fixation surgery were included. Of these, 94 (71.2%) were male, with a mean age of 36.9 years (SD ± 18.9). Infection of any type (score > 1) was observed in 63 (47.7%) patients. Coated and uncoated-pin track-infection occurred in 45.7% and 48.5% of patients, respectively (P= 0.0887). The probability of developing infection (defined as a score ≥ 2) adjusted for comorbidities and follow-up time was not statistically higher among those who received uncoated pins compared to those who received pins coated with HA (odds ratio (OR) = 1.56, 95% confidence interval (95% CI): 0.67-3.67, p <0.05). CONCLUSION: In the present study, the external fixator pin infection rates were similar when using HA coating and standard steel pins.

INTRODUCTION: Intramedullary nailing is the most frequent procedure used for the treatment of tibial fractures that do not involve the articular surfaces. The goal of this study is to analyse the clinical outcomes, time of surgery and use of fluoroscopy in patients with midshaft and distal tibial fractures treated with suprapatellar and infrapatellar nailing and compare the results between these different groups. METHODS: A retrospective study was conducted, and patients were selected between the period of March 2018 and October 2019. The following variables were studied: age, gender, AO classification, time of surgery (minutes), time of fluoroscopy (seconds), quality of reduction in postoperative controls and 1 year follow-up (using the angles MPTA, LDTA, PPTA, ADTA). Functional outcomes were studied at 1 month and 1 year follow-up. The visual analogue scale was used to study and compare the postoperative pain of the patients. The Lysholm Knee Score was used to evaluate the functional outcomes of the patients. RESULTS: A total of 80 patients meeting the inclusion criteria were included in this study. The suprapatellar approach was used in 44 patients and the infrapatellar in 36 patients. The time of surgery (p=0.008) and the mean time of use of fluoroscopy (p <0.001) difference were significant in favour of the suprapatellar approach. In the quality of reduction analysis, we have statistically significant results in the LDTA and ADTA angles. Also, in the evaluation of postoperative pain, significant difference was found in favour of the suprapatellar approach. CONCLUSION: In conclusion, our study compared the suprapatellar approach and the infrapatellar approach for the treatment of midshaft and distal tibia fractures with intramedullary nailing. The results showed lesser time of surgery and use of fluoroscopy, lower knee pain, and lower rate of malalignment with the suprapatellar approach than with the infrapatellar approach.

INTRODUCTION: the gold standard for tibial diaphyseal fracture treatment is represented by the intramedullary nail (IMN). This study aimed to assess the relevance of nail diameter in bone healing of tibial diaphyseal fractures. MATERIAL AND METHODS: a retrospective study was conducted analyzing patients with closed 42 OTA/AO tibial fractures, treated with a reamed and locked IMN between January 2014 and December 2020. The variables assessed were gender, age, comorbidities, number of bolts used, canal/nail index (difference between the diameter of the medullary canal and nail), nail/canal ratio (ratio between nail diameter and medullary canal), related to consolidation and failure rates (delay and non-union). RESULTS: 96 patients were included. The consolidation rate was 91.7% (n = 88). Patients with consolidation had a significantly larger nail diameter than those who failed (p = 0.0014), increasing the chance of consolidation 5.30 (p = 0.04) times for each millimeter that the nail increased its diameter. Using a nail > 10 mm increased the chance of consolidation 13.56 times (p = 0.018). A nail/canal ratio 0.80 increased the chance of consolidation 23.33 times (p = 0.005). CONCLUSION: our findings suggested that reamed and locked IMN in tibial diaphyseal fractures should be implanted with the largest possible diameter (> 10 mm and with a nail-to-canal ratio 0.80) to promote bone healing.
INTRODUCCIÓN: el estándar de oro de tratamiento para la mayoría de las fracturas diafisarias de tibia está representado por el clavo endomedular (CEM). El objetivo de este estudio fue analizar la importancia del diámetro del CEM sobre la consolidación de fracturas diafisarias de tibia. MATERIAL Y MÉTODOS: se realizó un estudio retrospectivo en pacientes con fracturas cerradas de tibia 42 OTA/AO, tratados con un CEM fresado y acerrojado, entre Enero de 2014 y Diciembre de 2020. Las variables analizadas fueron género, edad, comorbilidades, cantidad de cerrojos utilizados, relación clavo/canal (diferencia entre el diámetro del canal medular y clavo), el índice clavo/canal (razón entre diámetro del clavo y el canal medular), en relación con la tasa de consolidación y falla (retardo de consolidación y seudoartrosis). RESULTADOS: la serie final se conformó por 96 pacientes y la tasa de consolidación fue de 91.7% (n = 88). Se observó un diámetro de clavo significativamente mayor en los pacientes que consolidaron respecto a los que fallaron (p = 0.0014), incrementando la posibilidad de consolidación 5.30 (p = 0.04) veces, por cada milímetro que el clavo aumentó su diámetro. Se observó un incremento de probabilidad de consolidación de 13.56 (p = 0.018) veces utilizando un clavo > 10 mm de diámetro. El índice clavo/canal 0.80 aumentó la posibilidad de consolidación 23.33 veces (p = 0.005). CONCLUSIÓN: nuestros hallazgos sugieren que los CEM fresados y acerrojados en fracturas diafisarias de tibia deben colocarse del mayor diámetro posible (> 10 mm y con un índice clavo/canal 0.80) para favorecer la consolidación.

Introducción: La indicación de realizar una osteosíntesis con un clavo endomedular retrógrado en las fracturas de fémur se ha incrementado en los últimos años y, con ello, la cantidad de complicaciones. Se describen tres técnicas quirúrgicas para el manejo del fragmento proximal de la osteosíntesis endomedular rota. Desde marzo de 2001 hasta enero de 2019, se realizaron 321 osteosíntesis con clavos endomedulares retrógrados de fémur en nuestra institución. La tasa de rotura del implante asociada a una seudoartrosis fue del 0,9%. Se realizaron técnicas mínimamente invasivas para la extracción del implante, preservando las partes blandas. Se logró la reosteosíntesis definitiva con la consiguiente consolidación en un tiempo medio de 140 días. Conclusiones: Las técnicas utilizadas fueron simples, seguras, mínimamente invasivas y muy reproducibles. Nivel de Evidencia: IV
background: The indication for osteosynthesis with a retrograde intramedullary nail in femur fractures has increased in recent years and with it, the number of complications. Three surgical techniques are described for the management of the proximal fragment of the broken intramedullary osteosynthesis. From March 2001 to January 2019, 321 osteosyntheses with retrograde femoral intramedullary nails were performed at our institution. The implant rupture rate associated with nonunion was 0.9%. Minimally invasive techniques were performed to remove the implant, preserving the soft tissues. Definitive reosteosynthesis was achieved with the consequent consolidation in an average time of 140 days. Conclusions: The techniques used were simple, safe, minimally invasive, and reproducible. Introducción: Las fracturas ipsilaterales proximales de fémur ocurren en el 1 al 9% de las fracturas diafisarias. Existen múltiples tratamientos propuestos para dicha asociación lesional. El objetivo de este trabajo es revisar los diferentes métodos de tratamiento propuestos, y comparar sus resultados funcionales y principales complicaciones. Materiales y Métodos: Se utilizaron los buscadores electrónicos: PubMed, Lilacs, Cochrane y Ovid SP. La búsqueda llegó a un total de 1829 trabajos, de los cuales se seleccionaron 21 según criterios de inclusión y exclusión. Discusión: No existe un consenso acerca de cuál es el mejor método de fijación para esta asociación lesional. Esta ocurre en pacientes jóvenes por un mecanismo axial de alta energía cinética y la opción elegida debe buscar la reducción anatómica de la fracturas proximales de fémur. Las complicaciones como la pseudoartrosis de cuello femoral y la necrosis avascular son de difícil manejo en este grupo etario. Conclusión: Las fracturas ipsilaterales proximales y diafisarias de fémur son lesiones que presentan un problema diagnóstico y terapéutico. Hay un subdiagnóstico de las mismas y no existe un implante ideal, ni un consenso de cual es el mejor método de fijación.
Introduction: Ipsilateral proximal femoral fractures ocurred in 1 to 9% of femoral shaft fractures. There are multiple treatments proposed for these injuries. The objetive of the present work is revise the different treatment options and compare their functional results and main complications Materials and methods: A comprehensive literature search was carried out using: Pubmed, Lilacs, Cochrane and OVID SP. Initially there were identified a total of 1829 studies. Only 21 studies remained after inclusion and exclusion were applied. Discussion: There is no consensus about which is the best fixation option for these injuries. These occurred in young patients as a result of a high energy axial trauma, and the selected treatment must achieve anatomic reduction of the proximal femoral fracture. Complications such as femoral neck no-union and avascular necrosis are difficult to manage at this age. Conclusion: Ipsilateral proximal and shaft femoral fractures present diagnostic and therapeutic problems. There is an underdiagnosis of these injuries, and there is no ideal implant or consensus on which is the best fixation method.

INTRODUCTION: Controversy exists regarding the use of reamed interlocking nailing in femoral shafts with extremely narrow medullary canals (diameter ≤ 9â¯mm). The aims of this study were to (1) investigate the association of age and sex on femoral canal diameter in patients with a simple femoral shaft fracture and (2) compare the outcomes and complications of interlocking nailing between wide and extremely narrow intramedullary canals. PATIENTS AND METHODS: For the purposes of this retrospective cohort study, consecutive patients with simple femoral shaft fractures were recruited between January 2009 and December 2016. The patient demographic data were analyzed. Then, fractures treated with interlocking nailing were divided into the wide group (canal diameter > 9â¯mm) and narrow group. The primary outcome was union rate, and the secondary outcomes were complications such as thermal necrosis, fat embolism syndrome, iatrogenic fracture, and implant failure. RESULTS: This study included 340 femoral shaft fractures. The average canal diameter was 9.97⯱â¯1.79â¯mm, with significantly wider canals in men than in women. Overall, 289 of the patients had undergone interlocking nail fixation, and a similar union rate and complications were noted between the wide canal and narrow canal groups, with the exception of the incidence of iatrogenic fracture. CONCLUSIONS: Femoral shaft fractures associated with extremely narrow medullary canals are more common in women than in men. There was a similar union rate found when using interlocking nailing in a femoral shaft fracture in cases with extremely narrow and wider canals. Iatrogenic fracture is the only significant risk when using interlocking nailing in femoral shafts with extremely narrow canals.

INTRODUCTION: The purpose of this study was to evaluate the results achieved after the use of lateral minimally invasive plate osteosynthesis (MIPO) in oligotrophic humerus nonunions resulting from failed intramedullary nailing (IM). METHOD: We evaluated nine patients with humerus nonunion after failed locked intramedullary nailing, all treated using 3.5-mm locked compression plates (LCP) placed through lateral minimally invasive approaches, between 2010 and 2016. Patient's age averaged 39.7 years. All nonunions were diaphyseal and oligotrophic. All nonunions had previous surgical treatment with static locked nails (seven antegrade and two retrograde). The IM nails were all well inserted in the humerus (none of them protruded or had rotator cuff lesions associated). Pre-operative Disabilities of the Arm, Shoulder and Hand (DASH) score averaged 25.5 points. Pre-operative Constant's score averaged 80.2. Pre-operative visual analog scale of pain averaged 2.4 points. RESULTS: Follow-up averaged 17.7 months. Time between initial surgery and revision procedure averaged 11.7 months. Union was achieved in all cases, after an average of 4.8 months. DASH score at last follow-up averaged 5.1 points, and final Constant's score averaged 93.7 points. The analog scale of pain averaged 0.7 points. Time from definitive surgery to work return averaged 3.9 months. Long 3.5-mm LCPs were used (plate length averaged 16.9 screw holes). In two cases, a third 4-cm incision at the nonunion site was performed and cancellous autologous iliac crest bone graft was associated. CONCLUSION: In our series of nine patients, we achieved union and good objective and subjective results, with high patient satisfaction, using a lateral MIPO technique and placing long 3.5-mm LCPs in selected oligotrophic humerus nonunions after failed IM nailing.

Resumen: Las lesiones del ligamento cruzado anterior (LCA) hoy en día son tratadas principalmente con injerto de isquiotibiales y una amplia gama de técnicas y sistemas de fijación del LCA en busca de una menor morbilidad del paciente. Se reporta un caso de una paciente de 42 años que sufrió ruptura de LCA, tratada con reconstrucción de LCA con injerto autólogo de isquiotibiales, fijación femoral con pines transversos bioabsorbibles y fijación tibial con tornillo bioabsorbible. La paciente evolucionó con presencia de dolor en región lateral de rodilla, diagnosticado como síndrome de fricción de la banda iliotibial un año posterior a la cirugía. Se realizaron estudios de imagen que incluyeron rayos X y resonancia magnética (RM) donde se observó adecuada fijación del LCA con presencia de migración y ruptura del pin proximal bioabsorbible. Se decidió extraer el fragmento del pin bioabsrobile en un segundo tiempo quirúrgico y la paciente regresó a sus actividades cotidianas y deportivas sin dolor y con una adecuada estabilidad.
Abstract: Injuries of the anterior cruciate ligament are currently treated primarily with the use of hamstring graft with a wide range of different techniques and fixation systems for anterior cruciate ligament in reducing patient morbidity. We report the case of a female patient aged 42 that suffered an anterior cruciate ligament rupture and was treated with anterior cruciate ligament reconstruction with hamstring autograft with femoral fixation with bio-absorbable cross-pin and tibial fixation with bio-absorbable screw. The patient presented lateral knee pain that was diagnosed one year after the operation as an iliotibial band friction syndrome. Imaging studies were performed. X-rays and magnetic resonance imaging demonstrated adequate fixation of the anterior cruciate ligament with the presence of migration and rupture of the proximal bio-absorbable cross-pin. It was decided to remove the bio-absorbable cross-pin fragment in a second surgical procedure after which the patient went back to her daily activities and sports without pain and with stability stable knee.

Introducción: Se relaciona la bibliografía con la experiencia de nuestros cirujanos frente a un instrumental defectuoso en el tratamiento de fracturas de fémur con osteosíntesis endomedulares. Objetivos: 1) Enumerar inconvenientes técnicos que se presentan en las cirugías de fémur, 2) comparar diferentes Centros y problemas afrontados, 3) estimar los canales de reclamo. Materiales y Métodos: 1) Estudio retrospectivo, de observación, descriptivo sobre historias clínicas y archivo radiológico, 2) encuesta en línea enviada a traumatólogos generales, 3) consulta con la ANMAT, el IRAM, los Ministerios de Salud de la Ciudad Autónoma de Buenos Aires y de la Nación. Resultados: 1) 31 pacientes con fracturas de fémur tratados con osteosíntesis endomedulares entre enero de 2008 y agosto de 2013. Dieciocho casos de fallas o defectos del instrumental de colocación en 14 pacientes. Los problemas más frecuentes fueron las guías y las mechas, 2) 270 respuestas, 19 provincias argentinas, respuestas de Colombia, Ecuador, Italia, Australia y Bolivia. Se obtuvieron 180 respuestas de Centros privados y 90 de Centros públicos, 3) cuatro vías de reclamo: ANMAT bajo el programa de Tecnovigilancia, IRAM por el incumplimiento de las Normas ISO 9001, Ministerio de Salud por incumplimiento de la resolución 255 y la Ley Básica de Salud N.º 153, art. 12 (ítems k y l) y AAOT, en la subcomisión de Implantes. Conclusiones: Queda explícita la diferencia entre Centros públicos y privados; sin embargo, el medio laboral solo definió la prevalencia de inconvenientes técnicos, pero los inconvenientes fueron los mismos. Existen formas para denunciar y enfrentar esta problemática.
Background: This paper intends to correlate data obtained from different orthopedic surgical teams and literature on faulty instrumentation during femoral fracture treatment with intramedullary nailing. Objectives: 1) To define technical problems arising during surgical procedures to treat femoral fractures; 2) to compare different centers and problems; 3) to analyze possible ways to find a solution. Methods: 1) A retrospective, descriptive, observational study on clinical records and radiologic files was performed; 2) an on-line survey was sent to orthopedic surgeons; 3) a query was sent to ANMAT, IRAM, and Ministerios de Salud de la Ciudad Autónoma de Buenos Aires y de la Nación. Results: 1) 31 intramedullary femoral nailing procedures were performed from January 2008 to August 2013. Eighteen cases of surgical tool deficiencies in 14 patients. More frequent problems were related to drill bits and guide wires; 2) 270 forms were filled up, from 19 provinces of Argentina, and from Colombia, Ecuador, Italy, Australia and Bolivia. We received answers from 180 private institutions and 90 public hospitals; 3) there are four possible ways to reach a solution: ANMAT with its Programa de Tecnovigilancia, IRAM for non-fulfillment of ISO 9001 rules, Ministerios de Salud for nonfulfillment of resolution 255 and Ley Básica de Salud N.º 153, art. 12, items k and l, and AAOT Implants Subcommittee. Conclusions: A clear-cut difference was established between public and private centers; however, the working place only defined the prevalence of technical problems, but problems themselves were the same. There are legal and administrative tools to deal with these problems.

INTRODUCTION: Impingement and penetration of the anterior cortex of the femur have been reported as complications after cephalomedullary nailing. The purpose of this study was to determine factors related to nail impingement in our population of Hispanic patients. MATERIALS AND METHODS: A non-matched case-control study was carried out and 156 patients who underwent cephalomedullary nailing from 2010 and 2013 were included; 78 cases with anterior cortical impingement and 78 control cases without impingement were documented. Demographic variables and specifications of the nails such as manufacture and radius of curvature were recorded. The presence of impingement, angle of incidence on radiographs--indirect measurement of the femoral bow on the sagittal plane--and nail entry site were determined. Bivariate and multivariate logistic regression analyses were performed to identify the factors associated with cortical impingement. RESULTS: The distribution by sex corresponded to 87 females (56%) and 69 males (44%) with a mean age of 75 years [SD 18.2]. Cortical impingement was presented in 78 cases (50%) and 6 (3.8%) patients evidenced penetration of the anterior cortex of the femur. On the bivariate analysis the posterior nail start site is highlighted, which showed a positive association with impingement (OR 4.3; 95% CI 1.1-36 and p = 0.04). After the multivariate analysis, the factors associated with anterior cortical impingement included female gender (OR 2.2; 95 % CI 1.1-4.6 and p ≤ 0.038), straight nails-short nails-(OR 4.9; 95% CI 2.2-10 and p ≤ 0.001) and angle of incidence ≥7° (OR 4.9; 95% CI 2.2-10 and p ≤ 0.001), the latter showing a likelihood of 57 % for impingement, increasing to 90% with an angle of incidence of 11°. CONCLUSIONS: Posterior entry site should be avoided and an anterior site should be used. Female gender, straight nails and greater angle of incidence of the femur were associated with cortical impingement. A specific intramedullary nail design is needed for the Hispanic population due to high impingement and anterior cortical penetration rates seen with conventional nails. Short curved nails and long nails with a lower radius of curvature are required.

Sin embargo, la asociación de osteoporosis con fractura inestable está relacionada con diversas complicaciones del método de fijación, entre las que se señalan por su frecuencia: perforación de la cabeza femoral, pérdida de reducción y seudoartrosis. En este estudio se analizan las fallas con CCD, la relación de éstas con la osteoporosis evaluada con la clasificación de Singh y la utilidad de la técnica de Dimon para las fracturas inestables. Materiales y métodos: De un total de 111 pacientes tratados con CCD por fractura lateral de cadera entre octubre de 1999 y junio de 2003, se evaluaron en forma retrospectiva 95 casos con seguimiento posoperatorio promedio de 15 meses (9-24 meses). Se determinó la estabilidad fracturaria según la clasificación de Evans modificada; y el grado de osteoporosis con la clasificación de Singh. Fueron excluidos 16 casos por diversas causas. En todas las fracturas estables (51 casos, 53,6 por ciento) se realizó reducción anatómica. En las inestables (44 casos, 46,4 por ciento), en 15 se realizó reducción anatómica y en 29 reducción no anatómica con técnica de Dimon. Resultados: En 11 casos se presentaron fallas en el tratamiento (11,5 por ciento). Sólo una falló en las estables (1,9 por ciento). En las inestables 10 fallaron (22,7 por ciento), cuatro con reducción anatómica (26,6 por ciento) y seis con reducción con técnica de Dimon, pero de estas últimas una falló por infección con punto de partida en una flebitis y dos por errores de técnica solucionados dentro del primer mes posoperatorio y evolucionó favorablemente.
