RESUMO

Pediatric priapism can be a medical emergency but is not a common complaint seen in pediatric emergency department. Priapism in a previously healthy child is also rare. We report a case of painless stuttering priapism associated with an acute Mycoplasma pneumoniae infection in a previously healthy boy.

A 7-year-old intact male Boxer was referred to our services at the Veterinary Teaching Hospital of the University of Trás-os-Montes and Alto Douro, suffering from a persistently erect penis (including the bulbus glandis) that had been exposed for several days. Radiographic and ultrasonographic examinations detected a 5.0 x 3.5 cm mass located dorso-laterally to the urinary bladder. The microbial culture of the mass revealed Staphylococcus spp. At that time, we suspected the involvement of an abscess in the origin of the priapism. Medical and surgical treatments were promptly instituted, which allowed for penile withdrawal into the prepuce; however, the resolution of the penile erection was not accomplished in the following days and penile amputation was required. Histological evaluation of the excised penis revealed extensive infarction of the erectile tissue of the pars longa and bulbus glandis, and also of the blood vessels of the penis. Following penile amputation and antimicrobial therapy, the animal fully recovered. Ultimately, the animal died as a consequence of gastric torsion. At necropsy, some lesions compatible with a previous perforation of the intestinal wall were recorded. The data gathered from the anamnesis, the physical and imaging examinations, along with the post-mortem findings, allowed us to conclude that in this clinical case the primary cause of priapism was a perineal abscess due to bowel perforation.

Priapism seen occasionally in mice used in various mycoplasmal studies over several years prompted further investigation. Of six strains of young adult male mice inoculated intravenously with Mycoplasma pulmonis, of murine origin, priapism was seen only in CBA mice, penile erections persisting in some for seven to 44 weeks. A few mice given broth medium without mycoplasmas also developed priapism, and mice given five mycoplasmal species, of human origin, developed the condition in a way similar to these controls. However, those given M. pulmonis developed priapism earlier and of longer duration than other mice, suggesting that it was an enhancing factor. The duration of the phenomenon is remarkable and, as yet, has no clear explanation.

Priapism is a rare complication of hematological diseases. Among leukemia, it is most frequently seen in patients with chronic myeloid leukemia, due to the high leukocyte counts that these patients achieve. We report a 22 years old male who presented with a priapism lasting more than 24 hours. Thirty six hours after admission and subsequent to a leukopheresis, penile relaxation was obtained. Despite good hematological response to therapy, an extensive penile and uretral necrosis, associated to an Acinetobacter infection, ensued between the fourth and fifth day of admission, that required surgical treatment

OBJECTIVES: Priapism in the pediatric population is rare and most commonly occurs secondary to sickle cell disease or hematologic malignancy. We present a case of a 12-year-old boy with priapism who required aggressive surgical therapy for adequate detumescence. This patient had a recent viral upper respiratory infection and titers for Mycoplasma pneumoniae were indicative of infection. We propose that a hypercoagulable state was induced by the M. pneumoniae infection, which resulted in the priapism. METHODS: A 12-year-old boy with a recent upper respiratory illness presented to his pediatrician with priapism. After failing conservative management, the patient ultimately required a surgical shunt for detumescence. Serum was sent to detect antibodies against M. pneumoniae. RESULTS: The child failed to respond to corporeal irrigations and bilateral Winter shunts. He underwent an El-Ghorab procedure the following morning, which resulted in a flaccid penis. Serum M. pneumoniae antibodies were detected and indicated moderate infection. CONCLUSIONS: We propose that this 12-year-old boy had priapism secondary to infection with M. pneumoniae. M. pneumoniae infection can produce a hypercoagulable state, especially in selected areas of the circulation. This is the first reported case of priapism associated with M. pneumoniae.
