A comparison between the life expectancy of patients with pacemakers and that of a control population of similar structure shows a parallel course, although originally, after a postoperative phase of 2 months, the mortality rate of the patients with pacemakers was higher. Today the results are considerably better. Implantation of pacemakers is one of the greatest cardiological advances in the last 10 years for the effective treatment and prevention of cardiac arrest in atrioventricular block. The significance of this type of therapy becomes understandable if the fact that the mortality of Adams-Stokes attacks used to be about 50% is taken into consideration. In France, about 5000 persons each year have Adams-Stokes attacks.

One hundred and twelve patients with heart block and chronic tendency to syncope were ECG-monitored during syncope. Ventricular tachycardia and/or fibrillation (VT-VF) was observed as the cause of syncope in 11 patients: in 6 of 20 patients with chronic third degree A-V block, in 3 of 65 with paroxysmal A-V block and in 2 of 27 with S-A block. The R-R interval preceding the escape beat which initiated VT-VF varied between 1.2 and 2.2 seconds. The cerebral attacks were amenable to long-term pacemaker treatment. However, relapses of VT-VF were observed during pacing with a low rate of 55 per minute and during short interruptions in pacing, as produced by intermittent pacemaker failure or threshold determination. In one patient, supplementary treatment with a beta-blocking agent had to be given to suppress exercise-induced attacks of VT-VF after pacemaker implantation.

This is a report on a survey of 1126 deaf children in three schools for the deaf in the Province of Ontario. One case of the surdo-cardiac syndrome previously reported is included. There were four other children who had a history of syncope but no prolongation of the Q-T interval on the ECG. It is possible that these may represent a variant of the syndrome. Five more children had a prolonged Q-T interval but no fainting spells.
