Informe de Rotación Electiva

Dentro del programa de la Residencia Interdisciplinaria de Educación y Promoción de la Salud (RIEPS) se encuentran pautadas la realización de rotaciones electivas. Estas rotaciones están pensadas para residentes en su tercer año de formación, con el objetivo de que lx residente pueda ahondar en un tema que supere el desarrollo científico técnico de los Servicios del Gobierno de la Ciudad de Buenos Aires y del país, y en un lugar que sea referente en la materia (Cabral, Di Lorenzo, Lorusso, Rodríguez, 2003). En este marco, se realizó la rotación en los Servicios de Trabajo Social y Cuidados Paliativos del Hospital Zonal de Bariloche "Dr. Ramón Carrillo" desde el 1 de Marzo al 31 de Mayo del 2021. El presente informe pretende dar cuenta de las actividades realizadas durante la rotación electiva a fin de lograr los objetivos propuestos. También señalará los aprendizajes más significativos y los aportes realizados desde la antropología como desde la educación y promoción de la salud. (AU)

The St Thomas' Hospital Emergency Department Homeless Health Initiative: improving the quality, safety and equity of healthcare provided for homeless patients attending the ED.

We carried out a quality improvement (QI) project (QIP), aiming to improve the quality, safety and equity of healthcare provided for homeless patients attending the emergency department (ED). We used QI methodology to identify areas for improvement, and introduced and modified interventions over four Plan, Do, Study, Act cycles. We launched a departmental 'Homeless Health Initiative' (HHI), the chief intervention being the provision of 'Homeless Health Boxes' in the ED, which contained a 'Safe Discharge Checklist for Homeless Patients', maps to specialist homeless general practitioner surgeries and homeless day centres, information on other inclusion health services, copies of a local rough sleepers' magazine and oral hygiene supplies. Voluntary Homeless Link Nurses and a number of informal 'Homeless Health Champions' were appointed. The HHI was embedded in departmental awareness through regular presentations to staff and incorporation into the induction programme for new doctors. Staff satisfaction, in terms of how satisfied staff members were with the care they were able to provide for homeless patients in the ED on a 0-10 scale, improved modestly over the course of the QIP from median 6/10 to median 7/10. The number of staff who were severely dissatisfied with the care they were able to provide for homeless patients improved more markedly: first quartile staff satisfaction improved from 3.875/10 to 6.125/10. Staff compliance with the checklist was poor, with full compliance observed in only 15% of cases by the end of the QIP. An HHI is a cheap and worthwhile QI project, with the potential to significantly improve the quality, safety and equity of healthcare provided for homeless patients, while improving staff satisfaction concurrently. Similar initiatives should be considered in any ED which sees a significant number of homeless patients.

Guía de orientación para el abordaje familiar en tiempos de pandemia: elaborada por trabajadores sociales de Colombia, Venezuela y Argentina

Documento producido por la Comisión de Trabajo Social de la Asociación Latinoamericana de Cuidados Paliativos, con el objetivo de sumar algunos conceptos y herramientas prácticas que surgen de la experiencia de los trabajadores sociales y de la bibliografía consultada, para brindar apoyo a quienes asisten a personas enfermas durante la enfermedad, la muerte y el duelo en contexto de Aislamiento, debido a la pandemia por COVID19. Se describe la situación social de cada país o ciudad de actuación, y se incluye material de interés para el seguimiento familiar en distintos escenarios de la pandemia.

A Calming Cushion as a Therapeutic Wellness Tool for Youths with Disabilities and History of Trauma.

Seclusion and restraint are commonly used by residential staff to control unsafe behaviors of youths with disabilities and complex trauma histories. This pilot study examined the effect of a nonrestraining cushion as a calming device and wellness tool in a setting that prohibits supportive physical contact, to see to what extent the cushion might help these youths to better regulate their emotional distress and unwanted behaviors. Volunteers used the cushion for 15 minutes over 24 sessions. Nine students completed the study and reported reduction in anxiety and agitation, and a soothing feeling with residual effects to their daily routines. This pilot study suggests that the use of a nonrestraining, deep-pressure cushion may simulate a secure holding effect for young people with emotional dysregulation. The cushion is a safe and healing wellness tool that empowers youths and provides programs with an alternative to seclusion and restraint.

El trabajador social la unidad de corta estancia infanto-juvenil del hospital clínico universitario lozano blesa de Zaragoza / Social worker in the child psychiatry short-stay unit (UCE) lozano blesa university clinical hospital (Zaragoza)

La presente comunicación trata de realizar una sistematización de la práctica profesional del trabajador social en la Unidad de Corta Estancia Infanto-Juvenil (U.C.E. P.I) de Aragón, localizada en el Hospital Clínico Universitario Lozano Blesa de Zaragoza. El rol profesional del trabajador social en este dispositivo de atención psiquiátrica a población infantojuvenil y su familia se basa en la especialización profesional en el campo de la salud mental y en una preponderancia de la función clínica. Durante la hospitalización de los menores, la trabajadora social ofrece acompañamiento, tratamiento psicosocial y orientación adaptados a la situación de cada menor y de cada familia, priorizando aquellos casos cuyas familias se encuentran en una situación de mayor fragilidad, se enfrentan a patologías graves que provocan un fuerte impacto en la familia y situaciones de riesgo social. The present communication tries to carry out a systematization of the social worker professional practice in the child psychiatry short-stay unit of Aragon (Spain), located in the Lozano Blesa University Clinical Hospital (Zaragoza). The professional role of the social worker in this system of psychiatric care for children and adolescents and their families is based on professional specialization in mental health and a preponderance of the clinical function. During the hospitalization of minors, the social worker offers accompaniment, psychosocial treatment and guidance adapted to the situation of each child and each family, prioritizing those cases whose families are in a more fragile situation, face serious pathologies that cause a strong family impact and situations of social risk. The Use of Smartphones in Norwegian Social Care Services.

This study aims to understand how smartphone technology was perceived by social workers responsible for piloting social services software and the experiences of involving end-users as co-developers. The pilot resulted in an improved match between the smartphone software and workflow as well as mutual learning experiences among the social workers, clients, and the vendor. The pilot study revealed several graphical user interface (GUI) and functionality challenges. Implementing an ICT social service smartphone application may further improve efficiencies for social workers serving citizens, however; this study validates the importance to study end-users' experiences with communication and the real-time use of the system in order reap the anticipated benefits of ICT capabilities for smart phone social service applications.

Description of a psychosocial assessment instrument and risk criteria to support social work recommendations for kidney transplant candidates.

Developing uniform criteria and a risk assessment clinical intervention plan to identify, assess, and assign psychosocial risk levels may help guide transplant social workers' interventions and justify their listing recommendation. This article describes a kidney pre-transplant psychosocial assessment instrument that is supported by psychosocial risk criteria and a risk assessment clinical intervention plan.

Mobile social network services for families with children with developmental disabilities.

As Internet technologies evolve, their applications have changed various aspects of human life. Here, we attempt to examine their potential impact on services for families with developmentally delayed children. Our research is thus designed to utilize wireless mobile communication technologies, location services, and search technology in an effort to match families of specific needs with potential care providers. Based on the investigation conducted by our counselors, this paper describes a platform for smooth communication between professional communities and families with children with developmental disabilities (CDD). This research also looks into the impact of management of mobile social network services and training on the operation of these services. Interaction opportunities, care, and support to families with CDD are introduced.

Missing links in evidence-based practice for macro social work.

The paradigm of evidence-based practice includes a process for searching, appraising, and synthesizing evidence to answer a question. Two important elements related to macro social work practice must be addressed in this process. One missing link relates to the imperative to foster client self-determination and empowerment by allowing clients to choose among equally salient interventions. The second missing link is to ensure that the intervention is implemented with fidelity to the original model. This article describes the need for incorporating these elements in evidence-based macro practice. Guidelines are provided for implementing the elements and their implications for practice, research, and policy.

Assessing child and youth well-being: implications for child welfare practice.

The measurement of child well-being has become increasingly important in child welfare practice in the past ten years with the federal emphasis on measuring positive outcomes for children and families. Practical and methodological barriers to evaluating well-being exist alongside positive developments in the field. This article reviews the research literature related to child and youth well-being, providing a context for the discussion of measurement issues in child welfare settings. Based on a structured review of the literature, the article discusses instruments that appear to be most appropriate for use in a child welfare setting. Instruments are presented within stages of development, including (1) Infancy and Early Childhood, (2) Middle Childhood, and (3) Adolescence. Implications for the design and use of child well-being instruments in child welfare practice are discussed.

Family assessment in child welfare services: instrument comparisons.

Family assessment instruments can enhance the clinical judgment of child welfare practitioners by structuring decision-making processes and demonstrating the linkages between assessment, service provision, and child and family outcomes. This article describes the concept of family assessment in the child welfare context and provides an overview of the theoretical and disciplinary influences in the family assessment field. Based on a structured review of 85 instruments, the article discusses 21 that appear to the be the most valid and reliable for evaluating four federally-defined domains of family assessment: (1) patterns of social interaction, (2) parenting practices, (3) background and history of the parents or caregivers, and (4) problems in access to basic necessities such as income, employment, and adequate housing. Key measurement criteria as well as practical considerations in the selection and implementation of family assessment instrumentation in child welfare are discussed.

Ouvidoria: o sentido ético da participação / Ombudsman: the ethical sense of participation

A intencao de realizar esta pesquisa sobre o tema "Ouvidoria: o sentido etico da participacao", é contribuir com a qualidade dos servicos prestados à população usuária do ILSL. O setor ouvidoria, desde que implantado, constitui-se como instrumento de transformação considerando importante a opinião e a participação do cliente/ usuário na identificação de pontos que possam ser melhorados, tornando-se democrático. O estudo teve como objetivo geral caracterizar as representações e a participação dos usuários do ILSL na ouvidoria e quantificar essas representações, bem como conhecer as atribuições do ouvidor, que deve agir sempre de acordo com o código de ética profissional. A pesquisa caracterizou-se como quali-quantitativa, descrevendo e correlacionando os dados obtidos. O universo da pesquisa contemplou-se por profissionais que compõem ou fizeram parte da ouvidoria entre os anos de 2000 a 2006. Percebe-se, com a pesquisa, que a procura pelos serviços da ouvidoria tem aumentado e que ela esta sendo gradativamente conhecida, devido a divulgação dos seus serviços.

Use of a single page elder abuse assessment and management tool: a practical clinician's approach to identifying elder mistreatment.

Elder abuse is a growing, alarming public health issue. As health care professionals, our challenge is to balance our duty to protect the safety of the vulnerable elder with the elder's right to self-determination. Clinicians in busy practice settings across the continuum of care as well as community-based social workers, emergency medical system, police, and banking personnel are collaborative partners needing tools that focus on early recognition, assessment, intervention, and management of elder abuse. A simple one-page tool that provides principles of assessment and management, best practice guidelines and screening questions will hopefully serve to raise awareness of this important public health issue and maintain a high index of suspicion for elder abuse, neglect and financial exploitation.

Validation of the Strengths and Stressors Tracking Device with a child welfare population.

The Strengths and Stressors Tracking Device (SSTD) is a rapid assessment measure of family well-being that assesses the particular strengths and needs of families at intake to help guide case planning and evaluate the effectiveness of treatment. The device assesses families from an ecological perspective in the domains of environmental conditions, social support, caregiver skills, and child well-being, and may be used at multiple points during treatment to assess change. SSTD has high internal consistency in all domains, distinguishes between physical abuse and neglect, and is sensitive to specific changes made by families across the duration of treatment.
