Our goal was to develop a novel technique for inducing Achilles tendinopathy in animal models which more accurately represents the progressive histological and biomechanical characteristic of chronic Achilles tendinopathy in humans. In this animal research study, forty-five rabbits were randomly assigned to three groups and given bilateral Achilles injections. Low dose (LD group) (n = 18) underwent a novel technique with three low-dose (0.1mg) injections of collagenase that were separated by two weeks, the high dose group (HD) (n = 18) underwent traditional single high-dose (0.3mg) injections, and the third group were controls (n = 9). Six rabbits were sacrificed from each experimental group (LD and HD) at 10, 12 and 16 weeks. Control animals were sacrificed after 16 weeks. Histological and biomechanical properties were then compared in all three groups. At 10 weeks, Bonar score and tendon cross sectional area was highest in HD group, with impaired biomechanical properties compared to LD group. At 12 weeks, Bonar score was higher in LD group, with similar biomechanical findings when compared to HD group. After 16 weeks, Bonar score was significantly increased for both LD group (11,8±2,28) and HD group (5,6±2,51), when compared to controls (2±0,76). LD group showed more pronounced histological and biomechanical findings, including cross sectional area of the tendon, Young's modulus, yield stress and ultimate tensile strength. In conclusion, Achilles tendinopathy in animal models that were induced by serial injections of low-dose collagenase showed more pronounced histological and biomechanical findings after 16 weeks than traditional techniques, mimicking better the progressive and chronic characteristic of the tendinopathy in humans.

Tendinopathy is a common disease with a variety of treatments and therapies. Laser therapy appears as an alternative treatment. Here, we investigate the effects of laser irradiation in an experimental model of tendinitis induced by collagenase injection on rats' Achilles tendon, verifying its action in important inflammatory markers. Male Wistar rats were used and divided into five groups: control saline (C), non-treated tendinitis (NT) and tendinitis treated with sodium diclofenac (D) or laser (1 J) and (3 J). The tendinitis was induced by collagenase (100 µg/tendon) on the Achilles tendon, which was removed for further analyses. The gene expression for COX-2; TNF-α; IL-6; and IL-10 (RT-PCR) was measured. The laser irradiation (660 nm, 100 mW, 3 J) used in the treatment of the tendinitis induced by collagenase in Achilles tendon in rats was effective in the reduction of important pro-inflammatory markers such as IL-6 and TNF-α, becoming a promising tool for the treatment of tendon diseases.

A terapia celular vem sendo utilizada com resultados promissores no tratamento da tendinite equina, entretanto ainda existem dúvidas quanto à persistência e ao comportamento dessas células quando implantadas no local da lesão, e quanto à sua migração para outros focos inflamatórios. O objetivo deste estudo foi avaliar a marcação das células-tronco mesenquimais (CTMs) com nanocristal antes e após o implante em lesões tendíneas experimentais do tendão flexor digital superficial (TFDS) de equinos, bem como observar a possibilidade de migração das CTMs marcadas para outro foco de lesão, o membro contralateral do mesmo animal. Para isso, foi realizada a indução de lesão experimental no TFDS em ambos os membros torácicos de cinco equinos e, após sete dias, foram implantadas as CTMs autólogas marcadas com o nanocristal Qtracker 655 em um dos membros dos animais. Após sete dias do implante, foi realizada a biópsia tendínea para posterior avaliação histopatológica, utilizando-se microscopia com fluorescência. Também foi realizado o teste de viabilidade celular antes e após a incubação com o nanocristal. As CTMs marcadas e injetadas no tecido tendíneo mantiveram sua fluorescência sete dias após seu implante, e não ocorreu migração para o membro contralateral. O uso do nanocristal para a marcação das CTMs derivadas da medula óssea equina mostrou-se efetivo pelo fato de essa nanopartícula não ter alterado a viabilidade celular e por ela ter permanecido ativa durante o período implantado...

The aim of this study was to evaluate the labeling of mesenchymal stem cells (MSCs) with nanocrystals before and after implantation in experimental tendinitis of the superficial digital flexor tendon (SDFT) of horses, observing the migration possibility of MSCs marked to another lesion, performed on the contralateral limb of the same animal. An experimental lesion was induced in SDFT in both forelimbs of five horses, and after seven days autologous MSCs labeled with Qtracker(r) 655 were implanted in one member of the animals. Tendon biopsy was performed for subsequent histopathological evaluation using fluorescence microscopy seven days after the implant. Cell viability test was also performed before and after incubation with the cell labeling kit. MSCs labeled and injected into the tendon tissue maintained their fluorescence seven days after their implantation and there was no migration to the contralateral limb. Tendinopathy is a pathology found mainly in the rotator cuff, patellar, Achilles and flexor tendons. Tendinopathy is a significant impediment to performance in athletes and in workers in the labor market. Some studies have indicated that inflammation in adjacent tissues may affect the rotator cuff and Achilles tendon. In this study alterations were verified in the extracellular matrix (ECM) of the deep digital flexor tendon after two periods (12 and 24 hr) of induction inflammation in rat paw. Wistar rats were divided into three groups: those that received injection of 1% carrageenan; those that received 0.9% NaCl; and those that received no application. The tendon was divided into distal (d), proximal (p), and intermediate (i) regions. Biochemical analyses were performed and included non-collagenous proteins (NCP), glycosaminoglycans (GAGs), hydroxyproline (HoPro) and metalloproteinases 2 and 9. Tissue sections were stained with toluidine blue, hematoxylin-eosin, and Ponceau SS and observed under polarization microscopy. Remarkable results were detected that included the presence of MMP-9, degradation of NCP and GAG and the presence of cellular infiltrate closer to digits in d region. The different concentrations of HoPro, as well as alterations in the organization of the collagen fibers showed the collagenous matrix undergoing some alterations. The results indicated that the induced inflammation in rat paw exhibited characteristics similar to the typical acute inflammatory process observed in tendons.

Fluoroquinolone (FQ)-associated tendinopathy and myopathy are uncommon but well recognized complications of the use of this class of antibacterial agents. The case of a 63-year-old previously asymptomatic female patient who developed severe left shoulder tendinopathy after surreptitiously doubling the prescribed dose of levofloxacin for the treatment of community-acquired pneumonia is reported here. Surgical stabilization with suture anchors and subacromial decompression were needed.

A terapia LED de baixa potência possui efeitos analgésico e antiinflamatório. O objetivo desse estudo foi analisar a ação da terapia com LED de baixa potência na reparação tendínea por meio de histologia e histomorfometria. Foram usados 25 ratos Wistar, de 220 a 250 g, divididos em três grupos experimentais avaliados no 7º e 14º dia: A, tendinite induzida sem tratamento; B, tendinite induzida tratada com LED de baixa potência, densidade de energia 4 J/cm² por 120 segundos; e C, sem indução de tendinite. A histomorfometria mostrou-se altamente significativa (p<0,001) na terapia com LED em contrarrelógio de 14 dias, comparada ao grupo saudável (C). Entretanto, não mostrou significância levando em conta o grupo tendinite (A14), em que valor p>0,05. As análises histológica e histomorfométrica demonstraram qualitativo e quantitativo aumento no número de fibroblastos aos sete dias de tratamento, e das fibras colágenas, aos 14 dias, para grupo tratado com LED de baixa potência em relação ao grupo sadio (C). O mesmo não foi percebido quando relacionado ao grupo tendinite (A14).


The low power LED therapy has analgesic and anti-inflammatory effects. The objective of this study was to analyze the action of therapy with low power LED on the tendon repair, using histology and histomorphometry. Were used 25 Wistar rats, with 220 to 250 g, divided into three experimental groups (7 and 14 days): A7 and A14 induced tendonitis without treatment; B7 and B14, induced tendinitis treated with LED low power density energy 4 J /cm² for 120 seconds; and C, without induction of tendinitis. Histomorphometry was highly significant (p<0.001) with LED therapy against the clock for 14 days, compared with the healthy group (C). But not significant compared with the tendinitis group (A14) (p> 0.05). Histology and histomorphometry analysis demonstrated qualitative and quantitative increase in the number of fibroblasts to seven days of treatment, and collagen fibers at 14 days for the group treated with LED low power compared to the healthy group (C). But it is unclear even when related to the tendinitis group (A14).

Low-level laser therapy (LLLT) has been found to produce anti-inflammatory effects in a variety of disorders. Tendinopathies are directly related to unbalance in expression of pro- and anti-inflammatory cytokines which are responsible by degeneration process of tendinocytes. In the current study, we decided to investigate if LLLT could reduce mRNA expression for TNF-α, IL-1ß, IL-6, TGF-ß cytokines, and COX-2 enzyme. Forty-two male Wistar rats were divided randomly in seven groups, and tendinitis was induced with a collagenase intratendinea injection. The mRNA expression was evaluated by real-time PCR in 7th and 14th days after tendinitis. LLLT irradiation with wavelength of 780 nm required for 75 s with a dose of 7.7 J/cm(2) was administered in distinct moments: 12 h and 7 days post tendinitis. At the 12 h after tendinitis, the animals were irradiated once in intercalate days until the 7th or 14th day in and them the animals were killed, respectively. In other series, 7 days after tendinitis, the animals were irradiated once in intercalated days until the 14th day and then the animals were killed. LLLT in both acute and chronic phases decreased IL-6, COX-2, and TGF-ß expression after tendinitis, respectively, when compared to tendinitis groups: IL-6, COX-2, and TGF-ß. The LLLT not altered IL-1ß expression in any time, but reduced the TNF-α expression; however, only at chronic phase. We conclude that LLLT administered with this protocol reduces one of features of tendinopathies that is mRNA expression for pro-inflammatory mediators.

An inflammatory process was induced by intratendinous injection of bacterial collagenase into the superficial digital flexor tendon (SDFT) of the left thoracic limb of 10 horses. One week later, the tendons in five of the horses (group 1) were treated with glycosaminoglycan polysulphate (GAGPS), and the tendons of the other five (group 2) were treated with saline solution. The horses were euthanased 150 days after the collagenase injections, and samples of the SDFTs were frozen at -14 degrees C, sectioned at 5 to 7 mum longitudinally and transversely, and stained by the picrosirius red method. Morphometric analysis was used to quantify the organised and disorganised bundles of collagen in the samples from groups 1 and 2. Significantly more organised bundles of collagen were observed in the tendons treated with GAGPS.

Neste estudo, lesões tendíneas foram induzidas experimentalmente em eqüinos por meio da administração de diferentes doses de colagenase. Foram usados 12 eqüinos adultos, clinicamente sadios e distribuídos aleatoriamente em dois grupos que receberam diferentes doses de colagenase. O grupo 1 (G1) recebeu 1,25mg e o grupo 2 (G2) recebeu 2,5mg de colagenase. Com a agulha posicionada no centro dos tendões flexores digitais superficiais dos membros torácicos, as diferentes doses de colagenase foram aplicadas conforme o grupo experimental. O acompanhamento das lesões tendíneas baseou-se em exames físicos e ultra-sonográficos, realizados a cada sete dias até o período de 28 dias pós-indução. Nas imagens ultra-sonográficas, foram avaliados os parâmetros ecogenicidade, percentual de área lesada, grau de redução e severidade das lesões do tendão flexor digital superficial. O parâmetro mais fiel em resposta às diferentes doses de colagenase administradas foi o percentual de área lesada no interior do tendão. O G2 apresentou uma melhor resposta ultra-sonográfica da tendinite do flexor digital superficial para a realização de estudos experimentais.


In this study, tendinous injuries were induced experimentally in horses, through the administration of different doses of collagenase. Twelve horses, clinically healthy, were distributed into two groups that received different doses of collagenase. Group 1 (G1) received 1.25mg and Group 2 (G2) received 2.5mg of collagenase. With the needle positioned at the center of the superficial digital flexor tendon in the equine forelimbs, different doses of collagenase were applied in agreement the experimental group. The monitoring of colagenase-induced tendonitis was based on physical and ultrasonographic exams, carried out every seven days until the period of 28 days post-induction. In the images of ultrasound were evaluated the echogenicity, the percentage of injured area, degree of reduction and severity of injuries of colagenase-induced tendonitis. The most accurate parameter in response to different doses of collagenase administered was the percentage of area injured in the interior of the tendon. The G2 presented one better ultrasonographic reply of the tendonitis of the superficial digital flexor for the accomplishment of experimental studies.

O objetivo deste trabalho foi estudar o efeito do plasma rico em plaquetas (PRP) no tratamento da tendinite induzida no tendão do músculo flexor digital superficial (TFDS) de eqüinos mediante avaliação ultra-sonográfica. Para isso foram utilizados seis eqüinos hígidos machos castrados, com idade de 8-15 anos (x=12 anos). A tendinite do TFDS foi provocada em ambos os membros torácicos, mediante a administração intratendínea de 2,5mg de colagenase (2,5mg.µL-1), sendo esse procedimento considerado o início da fase experimental. Doze dias após a indução da tendinite, os animais foram submetidos a dois tratamentos: sendo que na lesão efetuada no TFDS direito (grupo tratado, GT), foram administrados 2,5mL de PRP ativado com cloreto de cálcio a 0,0125mol.L-1, contendo concentrações variando de 320.000 a 500.000 plaquetas.µL-1; na tendinite do TFDS esquerdo (grupo controle, GC), foram injetados 2,5mL de solução salina a 0,9 por cento. Após cinco dias, os animais foram submetidos à atividade física controlada e progressiva durante 30 dias. Os exames ultra-sonográficos foram realizados antes e após indução da tendinite (48 horas após e no 7º, 12º, 14º, 21º, 28º, 35º, 42º dias do experimento), sendo avaliado a área transversal do tendão (ATT), área transversal da lesão (ATL), o percentual da ATL, a intensidade e ecogenicidade da lesão, assim como o paralelismo das fibras colágenas. Os resultados revelaram redução (P<0,05) da ATL e do grau de ecogenicidade da lesão em função do tempo, mas com diferença (P<0,05) entre grupos apenas para ATL, com valores menores no GT. O tratamento com PRP proporciona maior redução da área da lesão, mensurada por ultra-sonografia.


The objective of the study was to evaluate the effect of platelet-rich plasma (PRP) in the treatment of induced tendinitis in the superficial digital flexor tendon (SDFT) of horses through ultrasonographic evaluation. Thus, six 8 to15-year-old healthy gelding horses (x=12 years) were used. Tendinitis of the SDFT was provoked in both forelimbs via intratendineous administration of 2.5mg of collagenase (2.5mg.mL-1), what was considered as beginning of the experimental phase. Twelve days after induced tendinitis, the horses were submitted to two treatments: (1) in the lesion caused in the right SDFT (treated group, TG), 2.5mL of PRP activated with calcium chloride at 0.0125mol.L-1, at concentrations from 320,000 to 500,000 platelets.µL-1, were administered; (2) in the tendinitis of the left SDFT (control group, CG), 2.5mL of 0.9 percent saline solution was administered. After 5 days, the animals were submitted to controlled and progressive physical activity during 30 days. Ultrasonographic examinations were carried out before and after tendinitis induction (48 hours after and on experimental days 7, 12, 14, 21 28, 35 and 42), with tendon cross-sectional area (CSA), lesion cross-sectional area (L-CSA), L-CSA percentage, severity and echogenicity of the lesion, as well as collagen fiber alignment being evaluated. The result revealed reduction (P<0.05) in the L-CSA and in the degree of lesion echogenicity in function of time, but with difference (P<0.05) between groups only for L-CSA, with smaller values in TG. The treatment with PRP promotes greater reduction in the area of the lesion measured by ultrasound.

Fluoroquinolone-associated tendinitis are well described in the literature, but these are not frequently observed and usually are related to the oldest fluoroquinolones. Levofloxacin is a recently introduced fluoroquinolone. In this paper we report four cases (three female, aged 39 to 70 years old) of Achilles tendinitis occurring in a period of few months. Two patients on chronic dialysis, one kidney transplant recipient, and one patient with chronic vasculitis, both with normal renal function. All were chronically using corticosteroids. In the four cases, tendinitis had an acute onset with bilateral involvement and was invalidating. In 3 cases the onset of tendinitis was early during levofloxacin treatment and in 1 case, it began 10 days after the end of the treatment. All patients were treated with rest and physiotherapy, two were treated with non-steroidal anti-inflammatory drugs. All patients had a complete recovery alter 3-8 weeks.

RESUMO

Describimos el caso de una paciente de 32 años con lupus eritematoso sistémico (LES), en tratamiento con glucocorticoides, quien luego de iniciar terapia antimicrobiana que incluía ciprofloxacina para el tratamiento de una infección respiratoria, presentó tendinitis aquiliana derecha y luego ruptura tendinosa. Se hace énfasis sobre la posibilidad de tendinopatías con la utilización de quinolonas, describiendo los posibles factores de riesgo, su diagnóstico y la forma de prevenir y tratar esta complicación. Es el primer caso en la literatura médica colombiana de ruptura tendinosa en una paciente con LES que recibe tratamiento con quinolona, planteándose un posible factor predisponente
