RESUMO

Tinnitus is a common condition characterised by a subjective, and less commonly objective, noise sensation that is often described as ringing heard in one or both ears by the individual. The exact cause of tinnitus is not known, but it is related to physiological changes in the auditory system and is closely linked to psychological well-being. Tinnitus can have a significant effect on the lives of people who experience it on a continuous basis. Although there is no cure for tinnitus, various strategies are available to support individuals to manage the condition on a daily basis; these management strategies are discussed in this article.

RESUMO

A number of ear, nose and throat symptoms are often associated with pregnancy, which although transient (most will disappear after birth), can have a significant impact on the woman's quality of life. It is essential therefore that midwives are aware of how such symptoms might present during pregnancy, in order to appropriately inform women and safely manage those symptoms. Tinnitus, the most common ear symptom experienced by women during pregnancy, for some will be a distressing experience and for others may indicate more serious conditions such as pre eclampsia, requiring careful monitoring and confident management.

Tinnitus is a common, yet poorly understood problem. This symptom has many causes, both benign as well as serious. Patients can experience significant changes in quality-of-life related to symptom severity and duration. This article explores causes of tinnitus, evaluation in a primary care setting, and management strategies.

About 10% of the population experiences tinnitus, a common and distressing symptom characterized by the perception of sound in the absence of external stimuli. There is, however, marked heterogeneity in etiology, perception, and extent of distress among those who experience tinnitus. Reactions to tinnitus vary from simple awareness to severe irritation; some people have difficulty in hearing because of the loudness of the noise. Severe tinnitus causes many, often psychological, symptoms (e.g., tension, frustration, impaired concentration, disrupted sleep). For some, tinnitus is temporary, for others it is longstanding. Although many people adjust successfully, others are disabled by tinnitus; approximately 5% experience persistent and severe symptoms affecting their lifestyle and significantly reducing their quality of life. Because tinnitus is poorly understood and no single therapeutic approach is effective for all patients, many patients are told that ''nothing can be done'' and they must ''learn to live with it.'' In spite of these challenges, there is remarkably little relevant nursing literature on tinnitus. This literature review was conducted to explore current knowledge of tinnitus, including prevalence, causes and diagnosis, and assessment. Its psychosocial effects and impact on individuals are considered. Implications for practice are discussed-demonstrating that understanding the full impact of the condition and identifying patients' needs are essential to effective care.

CONCLUSION: This study shows that cochlear implantation is relatively safe surgery with few major complications and within acceptable limits. However, close follow-up observation and effective medical and nursing intervention could alleviate further complications and thus become key elements for promoting recovery of patients undergoing such surgery. OBJECTIVES: Cochlear implantation has become an effective method for curing patients disabled by profound hearing loss in China. However, full exploration of the associated complications remains to be completed. The objective of this study was thus to analyse the postoperative complications in patients with cochlear implants (CIs) in order to design improved measures for clinical and nursing interventions. METHODS: A retrospective study of 262 patients receiving CIs at the Department of Otorhinolaryngology/Head and Neck Surgery, Chinese People's Liberation Army General Hospital, Beijing, China from March 1997 to December 2006 was conducted. RESULTS: Among 262 patients, 4 cases (1.5%) had 1 or more major complications requiring substantial medical or nursing interventions, including 1 case of cerebrospinal fluid (CSF) otorrhoea accompanied by meningitis, 2 cases of facial nerve paresis and 1 case of perforation of tympanic membrane. Forty cases (15.3%) had some form of minor complication that settled spontaneously or easily with conventional treatments and nursing, of which dizziness and vomiting was the most frequent (4.2%), followed by CSF gusher without otorrhoea and/or induced meningitis (2.7%), tinnitus (1.9%) and facial nerve partially exposed without paralysis (1.5%). Eleven cases (4.2%) had some symptoms associated with installation of the cochlear device. Except for one patient who had no response after implantation because his auditory nerves were underdeveloped, all the patients who received appropriate treatment and nursing intervention had a favourable prognosis.

BACKGROUND: Counselling is a basic psychological intervention for chronic tinnitus the effectiveness of which has not yet been evaluated. SCIENTIFIC QUESTION: The therapeutic effect of counselling was compared to that of a self-help manual. Outcome was analysed for tinnitus disability, tendency to become chronic, and accompanying psychiatric disorders. METHODS: A total of 75 tinnitus outpatients were randomly assigned to group counselling (n=35) and self-help (n=40). Tinnitus disability, general psychological disturbances, depression, anxiety, coping and illness beliefs were measured using questionnaires (TQ, SCL-90-R, BDI, BAI, FKV, KKG) administered before and after treatment and at a 6-month follow-up. Psychiatric disorders (DSM-IV) were assessed using the CIDI. RESULTS: Counselling and the self-help manual had a significant effect on tinnitus disability, showing most profit for participants with a high level of tinnitus distress. The significant reduction in tinnitus distress was maintained at the 6-month follow-up. There was, however, no difference between the two treatment-groups. Effect-sizes for patients with DSM-IV-diagnoses were smaller. CONCLUSION: For tinnitus outpatients without psychiatric comorbidity, self-help manuals can be an effective first treatment.

It is no longer necessary for people with tinnitus to suffer, as self-help strategies are available. Nurses are in a position to provide information about tinnitus and help people to manage their condition. This article gives guidance for nurses produced by the British Tinnitus Association.
