RESUMO
Este trabalho, partindo do tema elaborado, levantou o seguinte problema: As propriedades e vantagens estudadas nas membranas de Hidrogel, se faz desse material eficaz no tratamento dos tecidos periodontais na doença periodontal. Sendo assim, o objetivo deste trabalho foi realizar um levantamento bibliográfico sobre as propriedades e vantagens do uso de membranas de Hidrogel na Regeneração Óssea Guiada nos casos da perda óssea e a recessão gengival associada à Doença Periodontal. O uso de membrana para Regeneração Óssea Guiada (ROG) é um componente essencial do tratamento de doenças periodontais e na regeneração óssea. Neste caso, discutiremos as propriedades dos hidrogéis e seus benefícios e limitações nessa área. Apesar dos desafios significativos existentes, a regeneração óssea baseada em hidrogel é uma grande promessa para o futuro tratamento de doenças e defeitos relacionados aos ossos. Com uma compreensão aprofundada os hidrogéis serão, sem dúvida, uma ferramenta poderosa para o tratamento clínico de defeitos ósseos no futuro.
This study, based on the topic elaborated, raised the following problem: The properties and advantages studied in Hydrogel membranes and if this material can be effective in the treatment of periodontal tissues in periodontal disease. The objective of this work was to perform a bibliographic survey on the properties and advantages of using Hydrogel membranes in Guided Bone Regeneration in cases of bone loss and gingival recession associated with Periodontal Disease. The use of Guided Bone Regeneration (GBR) membrane is an essential component of the treatment of periodontal diseases and bone regeneration. In this case, we discussed the properties of hydrogels and their benefits and limitations in this area. Despite significant challenges, the hydrogel-based bone regeneration holds great promise for the future treatment of bone-related diseases and defects. With in-depth understanding, hydrogels will undoubtedly be a powerful tool for clinical treatment of bone defects in the future.

PURPOSE: To examine the association between educational level and the presence of periodontal disease in adults ages ≥ 50 years in Mexico. MATERIALS AND METHODS: A cross-sectional study was conducted on 2098 Mexican adults, using data from the annual reports of the Epidemiological Monitoring System for Oral Pathologies from 2019-2022. Data were collected on sociodemographic characteristics such as gender, age, educational level, oral hygiene, and diabetes. Periodontal status was evaluated using the Community Periodontal Index (CPI) and was classified into: CPI = 0 (healthy); CPI = 1 (bleeding on probing); CPI = 2 (calculus); and CPI = 3 or 4 (pocket depth ≥ 4 mm). A multinomial regression model was used to estimate the odds ratio (OR) and the 95% confidence intervals (CI), using periodontal status as the result. RESULTS: 39.9% of subjects presented periodontal pockets of ≥ 4 mm, 20.8% presented calculus, and 12.8% presented bleeding, while only 26.4% were classified as healthy. A low level of education (≤ 9 years) (OR = 4.84; p 0.001), age ≥ 65 years (OR = 1.33; p = 0.025), poor oral hygiene (OR = 6.86; p 0.001), smoking (OR = 1.51; p = 0.025), and diabetes (OR = 1.73; p 0.001) were statistically significantly associated with the presence of periodontal pockets ≥ 4 mm. CONCLUSIONS: A low level of education is associated with worse periodontal status in adults aged 50 years or more. These findings reiterate the importance of implementing effective strategies and the incorporation of interventions for improving the access to and quality of services targeted at aging communities.

El parto prematuro es un problema médico, social y económico importante, causa gran mortalidad y morbilidad neonatal, tiene un impacto importante en el sistema de salud y afecta la calidad de vida de las familias. El peso de los recién nacidos de madres con enfermedad periodontal es significativamente menor en comparación con los de madres no afectadas por esta enfermedad bucal. Este resultado adverso se considera un problema de salud pública global según los datos epidemiológicos. Objetivo. Determinar la asociación entre la prevalencia de parto prematuro y la enfermedad periodontal en Bolivia, Chile y Colombia entre el 2000 y el 2020. Materiales y métodos. Este estudio ecológico consideró las poblaciones de mujeres de Bolivia, Chile y Colombia, y la prevalencia de partos prematuros y enfermedad periodontal, discriminadas por grupos de edad. El estudio abarcó el período entre el 2000 y el 2020. La estrategia de búsqueda con la herramienta de investigación del Institute for Health Metrics and Evaluation incluyó prevalencia, grupos de edad, años entre 2000 y 2020, causas de parto prematuro y enfermedad periodontal, contexto y ubicaciones, mujeres y tasas. El análisis estadístico incluyó una regresión lineal simple entre parto prematuro y enfermedad periodontal para cada grupo de edad dentro de cada país. Resultados. Las tasas de partos prematuros fueron mayores en el grupo de 15 a 19 años (Bolivia: 697.563, Chile: 844.864, Colombia: 804.126). La prevalencia de la enfermedad periodontal aumentó con la edad, particularmente en el grupo de 45 a 49 años (Bolivia: 22'077.854, Chile: 34'297.901, Colombia: 32'032,830). Según los grupos de edad, la regresión lineal fue estadísticamente significativa (p < 0,001) para todos los grupos evaluados de la población boliviana, en los grupos mayores de 30 años para las colombianas y solo en el grupo de 15 a 19 años para las mujeres chilenas. Conclusión. Se encontró asociación entre el parto prematuro y la enfermedad periodontal en todos los grupos de edad en Bolivia, solo en el grupo de 15 a 19 años en Chile, y de 30 años y más en Colombia en el período evaluado de 20 años.

Periodontal disease, a multifactorial inflammatory condition affecting the supporting structures of the teeth, has been increasingly recognized for its association with various systemic diseases. Understanding the molecular comorbidities of periodontal disease is crucial for elucidating shared pathogenic mechanisms and potential therapeutic targets. In this study, we conducted comprehensive literature and biological database mining by utilizing DisGeNET2R for extracting gene-disease associations, Romin for integrating and modeling molecular interaction networks, and Rentrez R libraries for accessing and retrieving relevant information from NCBI databases. This integrative bioinformatics approach enabled us to systematically identify diseases sharing associated genes, proteins, or molecular pathways with periodontitis. Our analysis revealed significant molecular overlaps between periodontal disease and several systemic conditions, including cardiovascular diseases, diabetes mellitus, rheumatoid arthritis, and inflammatory bowel diseases. Shared molecular mechanisms implicated in the pathogenesis of these diseases and periodontitis encompassed dysregulation of inflammatory mediators, immune response pathways, oxidative stress pathways, and alterations in the extracellular matrix. Furthermore, network analysis unveiled the key hub genes and proteins (such as TNF, IL6, PTGS2, IL10, NOS3, IL1B, VEGFA, BCL2, STAT3, LEP and TP53) that play pivotal roles in the crosstalk between periodontal disease and its comorbidities, offering potential targets for therapeutic intervention. Insights gained from this integrative approach shed light on the intricate interplay between periodontal health and systemic well-being, emphasizing the importance of interdisciplinary collaboration in developing personalized treatment strategies for patients with periodontal disease and associated comorbidities.

To review current literature and synthesize clinical outcomes related to different low-level laser techniques as a complement to basic periodontal therapy (BPT). Electronic searches were conducted in PubMed, Cochrane, and Scopus, and clinical trials published from January 2013 to August 2023 using photobiomodulation as a complement to basic periodontal therapy, with a clear description of the laser technique, were included. The risk of bias was assessed using the Joanna Briggs Institute Critical Assessment Checklist. Estimates of interest were calculated using random effects meta-analyses. A total of 947 references were retrieved, and 22 studies were included for qualitative synthesis. Ten studies used intrasulcular laser techniques, with 89% using infrared wavelength, and 12 studies used transgingival techniques, with 61.5% using red wavelength. The frequency of photobiomodulation after BPT ranged from 1 to 9 sessions, with follow-up periods ranging from 5 days to 12 months. Risk of bias was considered low in 16 studies and moderate in six studies. Meta-analysis of 13 studies showed that BPT reduced probing depth at 4-, 12- and 24-weeks post-treatment, and improved clinical level attachment at 6-, 12- and 24-weeks post-treatment. Studies suggest that photobiomodulation may be a valuable complement in the treatment of periodontitis, especially using transgingival application technique.

This study aimed to assess whether dentists correctly understand the benefit of a dental treatment when it is presented using absolute numbers or relative risk reduction (RRR). This parallel-group randomized controlled trial recruited dentists from 3 postgraduate courses in Rio de Janeiro, Brazil. Participants received, in sequentially numbered sealed opaque envelopes, the description of a hypothetical scenario of the benefit (avoidance of multiple tooth loss) of nonsurgical periodontal treatment without or with antibiotics. Treatment benefit was presented in 2 different formats: absolute numbers or RRR. Dentists were given 10 minutes to read the treatment scenario and answer 5 questions. The final sample for analysis included 101 dentists. When asked to estimate the number of patients out of 100 who would avoid multiple tooth loss without antibiotics, 17 dentists (33%) in the absolute numbers group and 12 (25%) in the RRR group provided the correct response (p = 0.39). Regarding treatment with antibiotics, 26 dentists (50%) in the absolute numbers group and 14 (29%) in the RRR group provided the correct response (p = 0.04). Only 16 dentists (31%) in the absolute numbers group and 12 (25%) in the RRR group gave correct answers for both questions (p = 0.51). Most dentists did not correctly understand the benefit of the treatment, irrespective of the format it was presented. Slightly more dentists correctly understood the benefit of the treatment when it was presented as absolute numbers than as RRR.

AIM: The occurrence of periodontal diseases is still to be determined in large samples of major Brazilian cities. This study aimed to assess the periodontal status of adults from Curitiba, Paraná, Brazil, using periodontitis definitions by the Centers for Disease Control and Prevention and the American Academy of Periodontology (CDC/AAP) and the recently published ACES 2018 Classification Framework. MATERIALS AND METHODS: A multi-stage probability sampling technique was applied to draw individuals aged 18 or older. A total of 566 individuals underwent a full-mouth periodontal examination. Periodontitis cases were defined according to the CDC/AAP and the ACES 2018 Classification Framework. Non-periodontitis cases were classified as healthy or gingivitis. The agreement between the two definitions was calculated. RESULTS: Periodontal health was present in 33.6% and 13.8% of individuals, and gingivitis was found in 11.7% and 7.5%, according to CDC/AAP and ACES, respectively. Mild, moderate and severe periodontitis (CDC/AAP) were present in 2.1%, 33.4% and 19.1% of individuals, respectively. Using ACES, 34.3% had Stages I/II and 43.3% had Stages III/IV. The occurrence of periodontitis was higher when a subgroup of individuals aged 30+ were analysed, ranging from 69.6% (CDC/AAP) to 90.1% (ACES). CDC/AAP and ACES agreement for health, gingivitis and periodontitis accounted for 68.8% of the observations. CONCLUSIONS: Periodontal diseases affect more than 66% of the population aged 18+ years. Classic definition by the CDC/AAP and the recently published ACES Framework yielded moderate agreement.

Objective: The study delved into the epigenetic factors associated with periodontal disease in two lineages of mice, namely C57bl/6 and Balb/c. Its primary objective was to elucidate alterations in the methylome of mice with distinct genetic backgrounds following systemic microbial challenge, employing high-throughput DNA methylation analysis as the investigative tool. Methods: Porphyromonas gingivalis (Pg)was orally administered to induce periodontitis in both Balb/c and C57bl/6 lineage. After euthanasia, genomic DNA from both maxilla and blood were subjected to bisulfite conversion, PCR amplification and genome-wide DNA methylation analysis using the Ovation RRBS Methyl-Seq System coupled with the Illumina Infinium Mouse Methylation BeadChip. Results: Of particular significance was the distinct methylation profile observed within the Pg-induced group of the Balb/c lineage, contrasting with both the control and Pg-induced groups of the C57bl/6 lineage. Utilizing rigorous filtering criteria, we successfully identified a substantial number of differentially methylated regions (DMRs) across various tissues and comparison groups, shedding light on the prevailing hypermethylation in non-induced cohorts and hypomethylation in induced groups. The comparison between blood and maxilla samples underscored the unique methylation patterns specific to the jaw tissue. Our comprehensive methylome analysis further unveiled statistically significant disparities, particularly within promoter regions, in several comparison groups. Conclusion: The differential DNA methylation patterns observed between C57bl/6 and Balb/c mouse lines suggest that epigenetic factors contribute to the variations in disease susceptibility. The identified differentially methylated regions associated with immune regulation and inflammatory response provide potential targets for further investigation. These findings emphasize the importance of considering epigenetic mechanisms in the development and progression of periodontitis.

La gingivitis se ha definido como una inflamación de la encía caracterizada por edema, eritema, cambio de la morfología normal, exudado acuoso y hemorragia de interés mundial. El objetivo fue evaluar el efecto dos pastas dentales en pacientes jóvenes diagnosticados con gingivitis en la ciudad de CuscoPerú en el año 2022. La investigación se realizó en alumnos de la Facultad de Economía de la Universidad Nacional San Antonio Abad del Cusco todos ellos diagnosticados con gingivitis en el mismo año. El estudio se desarrolló bajo un diseño experimental pre-test post-test con dos grupos experimentales cada uno con n=15. Evaluados con el índice de placa de Löe & Silness y el índice gingival de Löe & Silness (IG). Los participantes fueron orientados a cepillar los dientes con el dentífrico tres veces al día, por un periodo de 2 semanas. Se utilizó como instrumento de recolección de datos una matriz de registro para la medición antes y después de aplicado el tratamiento en la cual se plasmaron los índices de placa e índice gingival de Löe & Silness. Resultó que los sujetos del estudio en su gran mayoría presentaron gingivitis moderada y un estado de higiene oral regular. Con lo cual concluimos que la pasta dental aplicada en el Grupo I resultó más efectiva en la recuperación de pacientes con gingivitis. El dentífrico con contenido de Ratania fue bien tolerado por los participantes, no existiendo ningún tipo de alteración de tipo alérgica en los tejidos bucales, por lo tanto, no interrumpió el tratamiento.
Gingivitis has been defined as an inflammation of the gum characterized by edema, erythema, change in normal morphology, watery exudate, and bleeding of global interest. The aim was to evaluate the effect of two toothpastes on young patients diagnosed with gingivitis in the city of Cusco, Peru in 2022. The research was conducted on students from the Faculty of Economics at the National University of San Antonio Abad in Cusco, all diagnosed with gingivitis in the same year. The study was carried out under a pre-test post-test experimental design with two experimental groups, each with n=15. Evaluated using the Löe & Silness plaque index and the Löe & Silness gingival index (GI). Participants were instructed to brush their teeth with the toothpaste three times a day for a period of 2 weeks. A data collection instrument was used, a record matrix for measurement before and after treatment application, where the Löe & Silness plaque and gingival indices were recorded. The study subjects mostly presented moderate gingivitis and a regular oral hygiene status. Therefore, we conclude that the toothpaste applied in Group I was more effective in the recovery of patients with gingivitis. The toothpaste containing Ratania was well tolerated by the participants, with no allergic tissue alterations in the oral tissues, thus not interrupting the treatment.
A gengivite foi definida como uma inflamação da gengiva caracterizada por edema, eritema, alteração na morfologia normal, exsudato aquoso e sangramento de interesse mundial. O objetivo foi avaliar o efeito de duas pastas de dentes em pacientes jovens diagnosticados com gengivite na cidade de Cusco, Peru, em 2022. A pesquisa foi realizada em alunos da Faculdade de Economia da Universidade Nacional de San Antonio Abad em Cusco, todos diagnosticados com gengivite no mesmo ano. O estudo foi realizado sob um desenho experimental pré-teste pós-teste com dois grupos experimentais, cada um com n=15. Avaliados com o índice de placa de Löe & Silness e o índice gengival de Löe & Silness (IG). Os participantes foram orientados a escovar os dentes com a pasta de dentes três vezes ao dia, por um período de 2 semanas. Foi utilizado como instrumento de coleta de dados uma matriz de registro para medição antes e depois da aplicação do tratamento, na qual foram registrados os índices de placa e índice gengival de Löe & Silness. A maioria dos sujeitos do estudo apresentou gengivite moderada e um estado de higiene oral regular. Portanto, concluímos que a pasta de dentes aplicada no Grupo I foi mais eficaz na recuperação de pacientes com gengivite. A pasta de dentes com conteúdo de Ratania foi bem tolerada pelos participantes, não havendo nenhuma alteração alérgica nos tecidos bucais, portanto, não interrompendo o tratamento.

Este artículo tiene como objetivo identificar y analizar los factores determinantes que conducen a la extracción de dientes permanentes en pacientes que asisten a hospitales y/o servicios odontológicos. A través de una revisión exhaustiva de la literatura científica y datos epidemiológicos, se exploran las principales causas de pérdida dental, incluyendo caries, enfermedad periodontal, trauma y factores socioeconómicos. Se discuten las implicaciones clínicas y de salud pública de estos hallazgos, así como estrategias preventivas y terapéuticas para reducir la incidencia de extracciones dentales (AU)
This article aims to identify and analyze the determining factors leading to the extraction of permanent teeth in patients attending hospitals and/or dental services. Through a comprehensive review of scientific literature and epidemiological data, the main causes of tooth loss are explored, including caries, periodontal disease, trauma, and socioeconomic factors. The clinical and public health implications of these findings are discussed, as well as preventive and therapeutic strategies to reduce the incidence of dental extractions (AU)

Background: Gingivitis is an inflammation of the gums that is the initial cause of the development of periodontal disease by the activity of Nuclear Factor-kappa B (NF-κB), Interleukin-1ß (IL-1ß), Interleukin-6 (IL-6), p38, and Tumor Necrosis Factor-α (TNF-α). Unaddressed chronic inflammation can lead to persistent disturbances in other parts of the body. Brazilin is a naturally occurring plant chemical that may have antibacterial and anti-inflammatory effects. Treatment based on the natural plant compound, brazilin, is developed in the form of a topical cream for easy application. Objective: The aim is to develop the natural compound brazilin in the form of a topical cream as an anti-inflammatory agent to reduce NF-κB expression through Imunohistochemistry (IHC) methods, and the expression of pro-inflammatory genes IL-1ß, IL-6, p38, and TNF-α. Methods: Male Sprague-Dawley rats were induced with gingivitis using P. gingivalis bacteria. The observed groups included rats treated with a single application of brazilin cream and rats treated with two applications of brazilin cream. The treatment was administered for 15 days. On days 3, 6, 9, 12, and 15, anatomical wound observations and wound histology using hematoxylin-eosin and Masson's Trichrome staining were performed. NF-κB protein expression was analyzed using the IHC method. Gingival inflammation gene expression of NF-κB, IL-1ß, IL-6, p38, and TNF-α was measured using q-RTPCR. Results: Single and double applications of brazilin cream increased angiogenesis and decreased NF-κB protein expression, in addition to the IL-1ß, IL-6, p38, and TNF-α gene expressions. Conclusion: In a rat gingivitis model, Brazilin cream may function as an anti-inflammatory agent in the gingival tissue.

AIMS: Cerebral palsy (CP) is the term for a set of neurological disorders resulting from brain damage that impairs motor function. The aim of the present study was to perform a systematic review of the literature to determine whether individuals with CP are at a greater risk of negative periodontal health outcomes compared to those without CP. METHODS: This study followed the recommendations of the MOOSE guidelines. Electronic searches were conducted in the PubMed, Web of Science, Scopus, Ovid, Embase, and PsycInfo databases. Observational studies assessing periodontal outcomes in individuals with CP were included. Risk of bias was appraised using the Newcastle-Ottawa scale. Meta-analyses were conducted and the results were presented using standardized mean differences (SMD), odds ratios (OR), and 95% confidence intervals (CI). The strength of the evidence was also assessed. RESULTS: A total of 316 records were retrieved from the electronic databases, 17 of which were included in the qualitative synthesis. Meta-analyses revealed significantly higher scores in individuals with CP compared to those without CP for the oral hygiene index (SMD = 0.47 [95% CI: 0.17-0.78, I2 = 80%), gingival index (SMD = 0.75 [95% CI: 0.39-1.11], I2 = 79%), plaque index (SMD = 0.70 [95% CI: 0.07-1.33], I2 = 93%), and calculus index (SMD = 0.98 [95% CI: 0.76-1.20], I2 = 0%). However, no significant difference was found between groups for the prevalence of gingivitis (OR = 1.27 [95% CI: 0.28-5.66], I2 = 93%). The risk of bias for the outcome assessment and statistical tests was low. The strength of the evidence was deemed very low. CONCLUSION: Individuals with CP may experience more significant negative periodontal health outcomes compared to those without CP.

The hypothesis that physiological changes in women can affect periodontal tissues is the subject of this study, and inflammatory markers such as matrix metalloproteinase-8 can measure susceptibility to inflammation. The study aimed to analyze MMP-8 levels in periodontal sites of postpartum women and women without a history of pregnancy, comparing health parameters and periodontal disease. This is a case-control study with 40 participants, 20 cases (women in the postpartum period) and 20 controls (women without any pregnancy), who underwent clinical periodontal examination and the collection of crevicular gingival fluid. The ELISA test was used to detect MMP-8 levels. Postpartum women had worse periodontal parameters, such as bleeding index on probing, number of sites with CAL ≥ 3, and fewer teeth present. In the group of women without a history of pregnancy, a significantly lower MMP-8 level was observed in healthy sites and a higher one was observed in periodontal pockets (p < 0.01). In contrast, in postpartum women, MMP-8 levels were elevated in both healthy sites and periodontal pockets (p > 0.01). The MMP-8 levels in gingival fluid appear to be related to periodontal clinical parameters and may be a possible marker of enzymatic changes involved in periodontal tissue destruction in postpartum women.

OBJECTIVES: Rumination is a maladaptive emotion regulation strategy. It has been associated with several psychological disorders and physical problems. This cross-sectional study aimed to evaluate whether cognitive rumination is associated with periodontal disease (PD), tooth loss (TL), and oral health-related quality of life (OHRQoL). MATERIALS AND METHODS: A population-based sample from a rural area in southern Brazil was evaluated. Calibrated examiners carried out a complete periodontal examination at six sites-per-tooth. Rumination and Reflection (RRQ) and Oral Health Impact Profile (OHIP-14) questionnaires were administered. Regression modeling was used to assess the prevalence ratio (PR) between rumination and PD and to estimate the rate ratio (RR) between rumination and TL and rumination and OHIP. RESULTS: Severe periodontitis prevalence of 33% was observed in the sample. In the Poisson-adjusted model (n = 587), individuals who ruminate more have 27% more periodontal disease (PR: 1.27, 95%CI:1.02 - 1.60). Regarding TL and OHIP, negative binomial regression (n = 672) showed an association with rumination, but it was not significant (RR 1.14, 95%CI 0.99 - 1.31) and (RR 1.20, 95%CI 0.98 - 1.48), respectively. CONCLUSIONS: Cognitive rumination was independently associated with periodontal disease in individuals living in a rural area. Borderline non-significant estimates were observed regarding TL and OHRQoL. More research using different populations and focusing on individual's responses to psychological stress may confirm these results. CLINICAL RELEVANCE: Emotional regulation is crucial to deal with stress, anxiety, and depression. Since psychopathologies are among the most prevalent diseases in the world, it is critical to understand the role of these issues in dental outcomes.

Background: The aim of this review was to evaluate the effects of periodontal disease (PD) treatment in pregnant women to reduce the risk of preterm birth (PB) and low birth weight (LBW) by conducting an umbrella review. Methods: A comprehensive search for the literature up to April 2024 was conducted across multiple databases including PubMed, Cochrane Library, Scopus, EMBASE, Scielo, Web of Science, Google Scholar, Proquest Dissertations and Theses, and OpenGrey. We specifically targeted systematic reviews (SRs) with or without meta-analyses, irrespective of language or time constraints, focusing on primary studies examining the effect of PD treatment in pregnant women to reduce the risk of PB and LBW. Various types of non-systematic reviews, intervention studies, observational studies, preclinical and basic research, summaries, comments, case reports, protocols, personal opinions, letters, and posters were excluded from consideration. The quality and overall confidence of the included studies were assessed using the AMSTAR-2 tool. Results: After the initial search, 232 articles were identified, of which only 24 met the selection criteria after exclusion. The majority of these studies indicated that periodontal treatment reduces the risk of PB and LBW. Conclusions: According to the findings and conclusions drawn from the SRs with a high overall confidence level, PD treatment in pregnant women reduces the risk of PB and LBW.

RESUMEN Introducción: A lo largo de los años se ha propuesto una notable variedad de protocolos quirúrgicos periodontales, cuyo enfoque pasó de la simple resolución de defectos de tejidos blandos a la realización de procedimientos predecibles y mínimamente invasivos. Inicialmente se usaba el injerto gingival libre. Posteriormente se han probado diversas técnicas, incluidas aquellas con sustitutos de tejidos blandos, como membranas tipo Alloderm. Hoy en día se considera el gold standard el injerto de tejido conectivo subepitelial asociado al colgajo de avance coronal. Por último, se ha examinado la técnica Pinhole, con mayor preservación de tejido y resultado más estético. Objetivos: El objetivo de esta revisión fue la comparación de técnicas actualizadas para el tratamiento de recesiones múltiples en maxilar superior e inferior. El resultado del procedimiento se evaluó en términos de cobertura radicular completa, reducción de la recesión, ganancia en altura y volumen, resultado estético, dolor postoperatorio y morbilidad del lecho donante y receptor del paciente. Material y métodos: Se realizaron búsquedas electrónicas y manuales para recopilar estudios de boca dividida, ensayos clínicos controlados aleatorios, series de casos, estudios piloto, libros de periodoncia, estudios de casos, revisiones sistemáticas y metanálisis, incluyendo los defectos de recesiónes gingivales múltiples, en maxilar y mandibular, de las cuatro clases de Miller, debido a su extensa evidencia. Resultados: Se incluyeron treinta y cuatro publicaciones y se extrajeron datos sobre el resultado de las técnicas quirúrgicas de dieciocho artículos. La evaluación clínica analizó la cantidad de cobertura radicular completa, la reducción de la recesión y la ganancia de altura y volumen, mientras que la perspectiva del paciente se expresó en términos de satisfacción estética y posibles complicaciones postoperatorias. Los procedimientos de los últimos diez años mostraron mejores resultados en todos los factores mencionados anteriormente. Conclusión: La predictibilidad y la estabilidad del tratamiento a largo plazo representan los factores que guían el proceso de elección de la técnica y que añaden valor a los procedimientos más actualizados. Se observaron progresos tanto a nivel estético, al reducir las discrepancias entre el área intervenida y el tejido circundante, como a nivel postoperatorio, al aminorar las molestias del paciente. Los desafíos propios de esta rama pronto podrían encontrar respuesta gracias a su rápida evolución, la cual permite concebir más avances.
ABSTRACT Background: Along the years, a remarkable variety of periodontal surgical protocols has been proposed, the focus of which has shifted from the mere resolution of a soft tissue defect to the performance of predictable and minimally invasive procedures. Initially, the free gingival graft was used. Subsequently, many different techniques were experimented, including those using soft tissue substitutes, such as the Alloderm membrane. Nowadays, the association of the connective tissue graft with the coronally advanced flap is considered the gold standard. Finally, the Pinhole technique, being a more conservative method in terms of tissue preservation and aesthetic outcome, was proposed. Objectives: The aim of this review was the comparison of the updated techniques for the treatment of multiple periodontal recessions, affecting both maxilla and mandible. The procedure outcome was assessed in terms of complete root coverage, recession reduction, gain in height and volume, aesthetic outcome, patient's post-operative pain and morbidity of donor and recipient sites. Material and methods: Electronic and hand searches were performed to collect split- mouth studies, randomized controlled clinical trials, case series, pilot studies, periodontal books, case studies, systematic reviews and meta-analysis, including maxillary and mandibular multiple gingival recession defects of all four Miller's classes, for its extensive evidence. Results: Thirty-four publications were included and data regarding the surgical techniques outcome were extracted from eighteen articles. The clinical evaluation analyzed the amount of complete root coverage, recession reduction and gain in height and volume, while the patient's perspective was expressed in terms of aesthetic satisfaction and possible postoperative complications. Procedures in the last ten years showed better results in all the above-mentioned factors. Conclusion: Procedural predictability and long-lasting treatment stability embody the factors driving the technique election process and adding value to more updated procedures. Progress was observed both at an aesthetic level, by reducing the discrepancias between the surgical region and the surrounding tissue, and at a postoperative level, by reducing patient discomfort. The challenges inherent to this branch could soon find answers thanks to its prompt evolution, which allows for further advances to be conceived.

O objetivo do presente estudo foi revisar a literatura para buscar evidências na associação entre a doença de Alzheimer e a Periodontite. A metodologia usada resultou numa busca às bases de dados PubMed/MEDLINE, Cochrane Library e Web of Science, através dos artigos publicados entre o período de maio de 2000 a maio de 2022. A doença de Alzheimer (DA) é classificada como uma condição neurodegenerativa, um grupo heterogêneo de doenças caracterizadas pela perda lenta e progressiva de uma ou mais funções do sistema nervoso. A doença periodontal (DP) é uma doença infecciosa e inflamatória que causa principalmente destruição óssea alveolar e perda dentária e estima-se que entre 20 e 50% da população geral possa sofrer de DP, dos quais 15-20% apresentam formas graves. A inflamação desempenha um papel crítico no aparecimento e progressão de ambas as doenças. A conclusão desta revisão é que a literatura estudada mostra que os patógenos periodontais e as citocinas pró-inflamatórias contribuíram para a progressão do processo neurodegenerativo da doença de Alzheimer. Porém, são necessários mais estudos clínicos controlados randomizados para a confirmação da relação causal desta associação.
The aim of this study was to review the literature to look for evidence in the association between Alzheimer's disease and Periodontitis. The methodology used resulted in a search of the PubMed/MEDLINE, Cochrane Library and Web of Science databases, through the articles published between May 2000 and May 2022. Alzheimer's disease (AD) is classified as a neurodegenerative condition, a heterogeneous group of diseases characterized by the slow and progressive loss of one or more functions of the nervous system. Periodontal disease (PD) is an infectious and inflammatory disease that mainly causes alveolar bone destruction and tooth loss and it is estimated that between 20 and 50% of the general population may suffer from PD, of which 15-20% present severe forms. Inflammation plays a critical role in the onset and progression of both diseases. The conclusion of this review is that the literature studied shows that periodontal pathogens and pro-inflammatory cytokines contributed to the progression of the neurodegenerative process of Alzheimer's disease. However, more randomized controlled clinical trials are needed to confirm the causal relationship of this association.

Periodontal disease is a highly prevalent chronic inflammatory disease that affects the tissues that support the teeth, while leukemia is a type of malignous cancer that affects the production of blood cells. Recent studies suggest that immune response and microbial disbiosis related to periodontal disease may be associated with an increased risk of developing leukemia and may affect its prognosis, as well as leukemia type and treatment may also have effects on the periodontium, demanding a interdiscipinary approach of these patients. The aim of this study was to conduct a literature review to assess the association between periodontal disease and leukemia in adult patients. An electronic database serch using the descriptors was performed. Clinical studies with periodontal examination in adult individuals with leukemia were selected. After literature search, 9 studies were reviewed. Gingival bleeding and periodontal pockets were frequent findings. Periodontitis prevalence varied among studies, ranging from 29% to 82,4% in patients diagnosed with leukemia. The relationship between periodontal disease and leukemia is complex and multifaceted and there are few studies available in adults, with heterogeneous exam protocols. Still, the high prevalence of gingivitis and periodontitis found in the studies suggest that periodontal diagnosis and treatment could be a helpful tool to prevent further complications in leukemia treatment.
A doença periodontal é uma doença inflamatória crônica altamente prevalente e que afeta os tecidos que sustentam os dentes, enquanto a leucemia é um tipo de câncer maligno que afeta a produção de células sanguíneas. Estudos recentes sugerem que a resposta imune e a disbiose microbiana relacionada a doença periodontal podem estar associadas a um risco aumentado de desenvolver leucemia e pode afetar o prognóstico da doença, assim como o tipo de leucemia e o tratamento também podem ter efeitos no periodonto, exigindo uma abordagem interdisciplinar desses pacientes. O objetivo deste estudo foi realizar uma revisão de literatura para avaliar a associação entre doença periodontal e leucemia em pacientes adultos. Foi realizada uma busca eletrônica em bancos de dados utilizando os descritores. Foram selecionados estudos clínicos com exame periodontal em indivíduos adultos com leucemia. Após busca na literatura, 9 estudos foram revisados. Sangramento gengival e bolsas periodontais foram achados frequentes. A prevalência da periodontite variou entre os estudos, sendo de 29% a 82,4% em pacientes diagnosticados com leucemia. A relação entre doença periodontal e leucemia é complexa e multifacetada e existem poucos estudos disponíveis em adultos, com protocolos de exames heterogêneos. Ainda assim, a alta prevalência de gengivite e periodontite encontrada nos estudos sugere que o diagnóstico e o tratamento periodontal podem ser uma ferramenta útil para prevenir maiores complicações no tratamento da leucemia.
