RESUMO
Objetivos:
Identificar o escore de risco para úlcera por pressão utilizando a Escala de Braden e avaliar as feridas quanto à localização anatômica, mensuração da ferida e classificação, sendo realizado na clínica médica e cirúrgica de um hospital público, com pacientes que apresentavam imobilidade física.Metodologia:
Estudo descritivo de caráter quantitativo, em que os pacientes foram avaliados utilizando a escala de Braden para a determinação do risco de desenvolvimento de úlceras por pressão e naqueles que já apresentavam a lesão, estas foram categorizadas quanto à localização anatômica, mensuração e classificação.Resultados:
Constatou-se que a maioria dos pacientes que desenvolveram ulcera por pressão estavam classificados na categoria risco elevado e risco muito alto. A maior parte dos pacientes apresentava apenas uma lesão,com predomínio na região sacral, de categoria II e pequena extensão.Conclusão:
O desenvolvimento de ulcera por pressão possui relação direta com a Escala de Braden, pois, quanto menor seu escore, maior a prevalência das lesões.ABSTRACT
Objectives:
To identify the risk score for pressure ulcers using the Braden Scale and assess the wounds on the anatomical location of the wound measurement and classification, being held in medicine and surgery in apublic hospital, with patients with physical immobility.Methods:
A descriptive study of quantitative character, in which patients were evaluated using the Braden scale to determine the risk of developing ulcersby pressure and those who already had the injury, these were categorized as the anatomical location, measurement and classification.Results:
It was found that most patients who developed ulcer pressure were classified as "high risk" and "very high risk". Most patients had only one lesion, predominantly in the sacral region, category II and small extent.Conclusion:
The development of pressure ulcer is directly related to the Braden Scale, the refore, the lower your score, the higher the prevalence of injuries.
