Quimioterapia paliativa nos cuidados de final de vida: revisão integrativa de literatura. (AU)

RESUMO

O câncer é considerado atualmente como um importante problema de saúde pública, que envolve um conjunto de mais de 100 doenças, responsável por mais de seis milhões de óbitos a cada ano, o que representa cerca de 12% de todas as causas de morte no mundo. Para garantir a qualidade de vida das pessoas acometidas, o tratamento oncológico deve contemplar diferentes tipos de tratamentos farmacológicos, como a radioterapia, cirurgia e quimioterapia, e não-farmacológicos. Descrever e analisar as evidências científicas disponíveis na literatura, referentes ao uso da quimioterapia considerada “paliativa”, desde o ponto de vista dos Cuidados Paliativos, enfatizando aspectos psicossociais e espirituais associados à doença e seu tratamento nos cuidados de final de vida. Trata-se de um trabalho de revisão integrativa da literatura. A busca bibliográfica foi realizada, nas bases de dados PubMed e LILACS, referente ao período de 2000 a 2014, artigos publicados em inglês, português e espanhol, com humana maiores de 18 anos. Foram usados os descritores controlados (Decs) antineoplastic agents, drug therapy combination, drug therapy, paliative care, terminal care, hospice care e supportive care, em diferentes combinações. Foram identificados 2.128 artigos que compuseram o material inicial desta revisão. Após a aplicação de critérios de exclusão, foram selecionados 17 artigos, todos internacionais, e que foram categorizados em duas unidades temáticas o contexto da quimioterapia paliativa (N=7) e quimioterapia paliativa nos cuidados finais de vida (N=10). Os estudos indicam que a utilização da quimioterapia paliativa nos cuidados de final de vida é mais agressiva aos pacientes, ocasionando maior frequência e duração das internações, procedimentos de caráter invasivos e efeitos colaterais....

ABSTRACT

Cancer is now considered a major public health problem, which involves a set of more than 100 diseases, accounting for more than six million deaths each year, representing about 12% of all causes of death worldwide . To ensure the quality of life of affected people, the cancer treatment should include different types of pharmacological treatments, such as radiation, surgery and chemotherapy, and non-pharmacological. Describe and analyze the scientific evidence available in the literature regarding the use of chemotherapy considered "palliative", from the point of view of palliative care, emphasizing psychosocial and spiritual aspects associated with the disease and its treatment in end of life care. It is an integrative review of the literature work. The literature search was conducted in PubMed and LILACS, which covers the period 2000 to 2014, articles published in English, Portuguese and Spanish, with higher human 18. We used the controlled descriptors (Decs) antineoplastic agents, combination drug therapy, drug therapy, paliative care, terminal care, hospice care and supportive care, in different combinations. 2,128 articles that formed the starting material of this review were identified. After applying exclusion criteria, we selected 17 articles, all international, and were categorized into two thematic units the context of palliative chemotherapy (N = 7) and palliative chemotherapy in end of life care (N = 10). Studies indicate that the use of palliative chemotherapy in late life care is aggressive patients, resulting in increased frequency and duration of hospitalization, character invasive procedures and side effects. But patients are willing to undergo this treatment, even for small benefits as "control" of cells, inhibition of some complications and the feeling of being is making it possible for the disease, which sometimes delayed the joint action...
