Manejo del climaterio y la menopausia, luego de los acontecimientos en el Women’s Health Initiative / Climateric and menopause. After the Women’s Health Initiative

RESUMEN

En el inicio del año 2003, el promedio de vida alcanza alrededor de 70 años en América Latina y llega a los 80 años en países industrializados, como Japón; el porcentaje de gente mayor de 60 años para el año 2000 fue en el Perú 7,2 por ciento, para el año 2005 se calcula el porcentaje en 12,6 por ciento y para el año 2050 se triplicará a 22,4 por ciento. Por ello, la importancia de reflexionar sobre el manejo del climaterio y la menopausia, especialmente después de los acontecimientos ocurridos con el Women’s Health Initiative (WHI). El presente trabajo revisa los acontecimientos inmediatamente previos y los que siguieron al WHI con el manejo hormonal en mujeres menopáusicas, cuando, en julio del año 2002, JAMA publicaba los hallazgos que habían obligado a los investigadores a tomar la determinación que es de conocimientos de todos –incluyendo de nuestras pacientes menopáusicas–. El autor hace una breve reflexión sobre los pros y contras del tratamiento hormonal en la menopausia, a quién tratar y las posibilidades terapéuticas para la sintomatología asociada a esta época de la vida.

ABSTRACT

At the beginning of 2003, life averaged 70 years in Latin America and reached 80 years in industrialized countries, like Japan. For the year 2000, the percentage of Peruvian people above 60 year-old was 7,2 per cent for year 2025 it is estimated in 12,6 per cent; and, for year 2050 it will be three times up to 22,4 per cent. It follows the importance of continuing considering climacteric and menopause treatment, especially following the events of the Women’s Health Initiative (WHI). This paper reviews the immediately previous events and those that followed WHI with regards to hormonal therapy in menopausal women. In July 2002, JAMA published the findings that obliged to take the determination that is known to all of us –including our menopausal patients–. The author summarizes the pros and cons of menopause hormonal treatment, which women to treat, and therapeutic possibilities for the symptoms associated to this of life.
