RESUMEN

Objetivo:

correlacionar los hallazgos de punción aspiración con aguja fina (PAAF) y el resultado histopatológico de la biopsia definitiva en pacientes con nódulos tiroideos que acudieron al Hospital General del Este "Dr. Domingo Luciani" entre enero de 2007 y abril de 2011.

Métodos:

se revisaron las historias de 60 pacientes quienes fueron intervenidos quirúrgicamente por presentar nódulo tiroideo, a quienes se les realizó PAAF del nódulo en el preoperatorio, corte congelado y biopsia definitiva.

Resultados:

de los 60 pacientes estudiados 53 fueron del sexo femenino, 50% de las PAAF fueron negativos para malignidad, 40% indeterminadas, 6,7% insuficiente 3,3% positivo. El corte congelado demostró 51,7% benignos, en 40% no hubo corte congelado, 5% malignos, 3,3% el resultado fue diferido. La biopsia definitiva reportó benigno en un 86,7% de casos. Se encontró una sensibilidad de 33,3% para la PAAF con 100% de especificidad, mientras que para el corte congelado se evidenció 100% de sensibilidad y especificidad.

Conclusión:

el nódulo tiroideo es una enfermedad que tiene mayor prevalencia en el sexo femenino, sin embargo, representa mayor riesgo a malignidad en el sexo masculino; el estudio del nódulo tiroideo debe incluir la PAAF la cual a pesar de haber presentado 33,3% de sensibilidad en este estudio permite orientar la conducta del médico tratante; no se debe excluir el corte congelado en pacientes con citología negativa para malignidad ni con resultado indeterminado(AU)
ABSTRACT

Objective:

to correlate the findings of fine needle aspiration (FNA) and histopathologic outcome of definitive biopsy in patients with thyroid nodules who were treated at the Hospital General del Este "Dr. Domingo Luciani" between January 2007 and April 2011.

Methods:

the records of 60 patients who underwent surgery for thyroid nodules, which also underwent FNA of the nodule on the preoperative, frozen section and final biopsy, were reviewed.

Results:

53 of 60 studied patients were female, 50% of FNA were negative for malignancy, 40% indeterminate, 6.7% insufficient, 3.3% positive. The frozen section shown 51.7% benign, 40% had no frozen section, 5% malignant, 3.3% the result was deferred. Definitive biopsy reported benign in 86.7% of cases. A sensitivity of 33.3% was found for FNA with 100% specificity, while frozen section shown a 100% sensitivity and specificity.

Conclusion:

the thyroid nodule is a disease mostly prevalent in females, but represents a higher risk of malignancy in males; PAAF most be included in the thyroid nodules study, which despite having 33.3% sensitivity in this study, lets define the treating physician behavior; the frozen section in patients with negative cytology for malignancy should not be excluded nor the ones with indeterminate result(AU)
