El canino maxilar incluido es una alteración frecuente en la práctica ortodóncica y se define así, cuando completada su formación apical no ha erupcionado, o pasaron, al menos, 6 meses de la erupció de su homólogo contralateral. El objetivo de este trabajo es valorar el diagnóstico y tratamiento tempranos del canino maxilar incluido palatalmente. Con motivo de este artículo se revisó la literatura entre 1988 y 2014 y se han reportado prevalencia, etiología, factores de predicción, para orientar el diagnóstico temprano y pronosticar potenciales retenciones, pues son susceptibles de pasar inadvertidas durante la dentición mixta, a causa de la variabilidad del patrón y momentos eruptivos. Ante un diagnóstico de canino desplazado a palatino, se sugiere considerar tratamientos interceptivos, como la exodoncia oportuna del canino temporario, aislada o en combinación con el manejo ortodóntico del espacio sagital y transversal, con la finalidad de reducir un abordaje de exposición quirúrgica con tracción ortodóntica y minimizar el riesgo de reabsorción radicular de los dientes adyacentes.
ABSTRACT
Maxillary canine impaction is a frequent anomaly in orthodontic practice, and it is defined as follow, when completed its root development, it has not erupted or at least, six months passed since its contralateral counterpart eruption. The aim of this study is to assess the early diagnosis and treatment of the palatal impacted canine. By the article the literature was reviewed between 1988 and 2014 and prevalence, etiology, predictive factors were reported to guide the early diagnosis and predict potential impactions as they are likely to go unnoticed during the mixed dentition stage because of the variability of the eruptive pattern and time. Faced with a diagnosis of palatally displaced canine, it is suggested to consider interceptive treatment, such as the timely deciduos canine extraction, isolated or in combination with orthodontic management of the transverse and saggital space, in order to reduce an approach of surgical exposure plus orthodontic traction and minimize the risk of root resorption of the adjacent teeth.
