Reinserção de freio labial superior ­ relato de caso / Upper labial frenum reinsertion ­ case report

RESUMO

O freio labial superior possui a função de promover estabilidade na linha média do lábio e impedir a excessiva exposição da gengiva. A desinserção do freio pode levar a complicações, tais como acúmulo de restos alimentares, irritação local, dificuldade de fonação e comprometimento estético. Paciente leucoderma, sexo feminino, 31 anos compareceu à Clínica Escola da Faculdade de Odontologia ULBRA-Torres, relatando incômodo e dor. A paciente descreveu sofrer de labirintite e, que em uma das crises, sofreu uma queda e lesionou a face. A queda causou um descolamento do freio labial superior juntamente com a desinserção da papila gengival. Para a solução do caso, optou-se por uma intervenção cirúrgica. O procedimento foi realizado por meio de uma incisão intrasulcular, tanto em vestibular como em palatino, nos Incisivos Centrais Superiores e posterior curetagem de tecido de granulação e debridamento da região. O freio labial foi reposicionado através de sutura com pontos simples. O resultado obtido após dez (10) dias mostrou total reinserção do freio labial, promovendo a estabilidade na linha média do lábio e a cessação da sintomatologia apresentada inicialmente pela paciente, mas com presença de espaço/buraco negro em região interpapilar. A evolução do quadro ao longo de 3 meses de acompanhamento mostrou a total maturação do tecido e o gradual desaparecimento do espaço/buraco negro (AU).

ABSTRACT

The primary function of the upper labial frenum is to promote stability in the lip middle line and to prevent overexposure of the gum. Frenum detachment may lead to complications such as accumulation of food debris, local irritation, phonation difficulty and aesthetic commitment. A 31-year-old leucoderma female patient came to the Clinical School of ULBRA-Torres School of Dentistry reporting discomfort and pain. The patient described that she suffered from labyrinthitis and that during a crisis she had fallen down and injured her face. The fall caused a tear in the upper labial frenum and the detachment of the gingival papilla. Surgery was the option chosen to treat the lesion. The procedure included an intrasulcular incision on both buccal and palatal aspects of the maxillary central incisors and a subsequent curettage of granulation tissue and debridement of the region. The labial frenum was repositioned and lsutured with simple stitches. The result obtained after ten (10) days showed total reinsertion of the labial frenum, thus promoting stability in the lip middle line and interruption of the symptoms initially presented by the patient, but there was the presence of a space/black hole in the interpapillary region. The evolution of the condition over 3-month follow-up period showed full tissue maturation and gradual disappearance of the space/black hole (AU).
