Identificação de interações medicamentosas e eventos hemorrágicos em idosos em uso de varfarina / Identifying drug interactions and bleeding events in warfarin use in the elderly

RESUMO

Objetivos: Identificar o uso de medicamentos, interações com a varfarina, e a ocorrência de eventos hemorrágicos em um grupo de pacientes participantes da oficina educacional em um ambulatório de anticoagulação. Métodos: Trata-se de um estudo observacional descritivo, realizado no período entre junho a setembro de 2014, em um ambulatório multiprofissional de atendimento a pacientes em uso de anticoagulante oral do Sistema Único de Saúde (SUS) localizado em Belo Horizonte, Minas Gerais. Na primeira etapa, foram conduzidas oficinas educacionais. A segunda foi consulta dos prontuários eletrônicos de cada paciente participante das oficinas e a identificação das interações medicamentosas, com o auxílio da base de dados, Micromedex. Na terceira etapa, os dados foram registrados e compilados em planilha do programa Microsoft Excel para análise estatística. Resultados: Analisaram-se os prontuários de 64 pacientes que utilizavam algum medicamento, além da varfarina. De um total de 184 interações medicamentosas, identificaram-se 131 decorrentes da possibilidade do aumento de sangramento. Como esperado, a varfarina, anticoagulante, prevaleceu entre os itens prescritos. Os eventos hemorrágicos foram observados em prontuários de apenas 4 pacientes, sendo eles hematúria, equimose, melena e hematêmese. Conclusões: Este estudo confirma a importância do acompanhamento ambulatorial de pacientes que fazem uso de anticoagulante oral e as oficinas educacionais ofertadas para prevenir o efeito resultante das potenciais interações medicamentosas.

ABSTRACT

Objectives: To identify the use of drugs, interactions with Warfarin, and the occurrence of bleeding events in a group of patients participating in the educational workshop offered at an anticoagulation clinic. Methods: This is a descriptive observational study conducted between June and September of 2014 in a multiprofessional outpatient clinic for patients using oral anticoagulant, within the Unified Health System (SUS) in Belo Horizonte, Minas Gerais. In the first stage, educational workshops were conducted. The second stage was to consult the electronic medical records of each patient participating in the workshops and identify possible drug interactions with the aid of the database, Micromedex. And in the third stage, data were recorded and compiled in a Microsoft Excel spreadsheet, for statistical analysis. Results: We analyzed the charts of 64 patients who used some medication, in addition to Warfarin. From a total of 184 drug interactions, a total of 131 were identified stemming from the possibility of increased bleeding. As expected, Warfarin, an anticoagulant, prevailed among the prescribed items. Bleeding events were observed in these records for only 4 patients, including hematuria, bruising, melena, and hematemesis. Conclusions: This study demonstrates that Warfarin interacts with a range of different medicines, so there is obvious importance for ambulatory monitoring of patients taking oral anticoagulants and for educational workshops offered to prevent the resulting effect of potential drug interactions.
