Evaluación de habilidades clínicas en estudiantes del Nuevo Programa de Formación de Médicos / Evaluation of clinical abilities in students included in the new program of medical formation

RESUMEN

Introducción: las habilidades y los hábitos son complejos niveles de dominio de la persona sobre la actividad que realiza, las primeras se forman como acciones sistematizadas y los hábitos son resultantes de un complejo proceso de automatización de las operaciones. Objetivo: evaluar el desarrollo de habilidades clínicas por los estudiantes en la práctica docente de la asignatura Clínica IV durante la rotación por el Hospital Universitario Dr Miguel Enríquez durante el curso escolar 2008-2009. Métodos: el universo estuvo conformado por los 393 estudiantes que cursaron la asignatura Clínica IV del Policlínico Facultad Ángel Alberto Galañena LLevat, del Nuevo Programa de Formación de Médicos. Se trabajo con una muestra, constituida por 84 estudiantes seleccionados por el método aleatorio simple. La recolección de la información se realizó mediante la aplicación de una guía de observación a las unidades de análisis. Resultados: el mayor porcentaje de los estudiantes obtuvo la calificación de regular en 4 de las 6 habilidades clínicas evaluadas Razonamiento clínico 73,8 por ciento, interrogatorio 63,1 por ciento, examen físico 53,6 por ciento y presentación oral 51,2 por ciento. Un 52,4 por ciento fue evaluado de mal en el punto de selección de la conducta terapéutica integral. Conclusiones: los estudiantes demostraron desarrollar las habilidades clínicas en la asignatura Clínica IV, durante la rotación por el hospital. Con dificultades en la conducción del interrogatorio, la realización examen físico del sistema nervioso, realización del diagnóstico diferencial y selección de la conducta terapéutica integral(AU)

ABSTRACT

Introduction: abilities and habits are complex levels of mastery of an activity by a person; the former are created in the form of systematized actions and the latter are the result of a complex process of automation of operations. Objective: to evaluate the development of clinical abilities by the students in the teaching practice of subject Clinic IV during rotation at Dr Miguel Enríquez hospital in the 2008-2009 academic year. Methods: the universe was composed by 393 students who studied the subject Clinic IV at Angel Alberto Galañena Llevat teaching polyclinics within the New Program of Medical Formation. The sample was 84 students who were selected by the simple randomized method. The information collection was based on an observational guide applied to analysis units. Results: The highest percentage of students was qualified as regular in 4 out of the 6 tested clinical abilities clinical reasoning, 73,8 percent; questioning, 63,1 percent; physical examination, 53,6 percent and oral presentation, 51,2 percent. The selection of the integral therapeutic behaviour was evaluated as being wrong in 52,4 percent of students. Conclusions: the students developed clinical abilities derived from the subject Clinic IV during rotation at the hospital; however, difficulties were found in the questioning of patients, the physical exam of the nervous system, the differential diagnosis and the selection of the integral therapeutical behaviour to be followed(AU)
