RESUMEN
La sepsis es una enfermedad en extremo frecuente y grave. En Cuba, relevante su morbilidad crítica pediátrica, destacándose la adquirida en la comunidad, seguida de nosocomial y neonatal; elevando gastos hospitalarios, estadía y mortalidad. En adultos tercera parte fallecen por sepsis. El objetivo fue incrementar competencia y desempeño de médicos que trabajan en áreas de terapias intensivas municipales. Pinar del Río, 2011. Se us'o un estudio analítico, ambispectivo, de cohorte, longitudinal. Se diseño curso de superación, así como test evaluativos. Intervención educativa sobre diagnóstico, fisiopatología y manejo de sepsis grave. Se evaluaron conocimientos de médicos, áreas intensivas municipales antes y después de la intervención educativa, así como cambio en indicadores de calidad (atención al paciente con sepsis), posterior a la capacitación. Los médicos tenían más de 5 años de especialista (44,5 por ciento), 16,7 por ciento había recibido capacitación sobre sepsis. El examen inicial 33,3 por ciento regular y 38,9 mal, final ninguno mal, 50 por ciento bien y 33,3 por ciento excelente. Mejoría en uso apropiado de antibióticos e inicio temprano de terapéutica antibiótica. Mejoraron indicadores de calidad de atención brindada a pacientes con sepsis. Los médicos (>50 por ciento) que laboran en áreas de urgencias tienen poca experiencia y no han cursado postgrados de sepsis. Conclusiones:
Se obtuvo elevación de conocimientos sobre sepsis posterior al curso. Se evidencia la utilidad de la intervención educativa en mejorar las condiciones en pacientes con sepsis grave en emergencias provinciales y se mejoraron los indicadores de calidad en la atención médica en sepsis en las áreas intensivas municipales(AU)ABSTRACT
Sepsis is an extremely frequent and severe entity. In Cuba, pediatric morbidity is significant, where community-acquired sepsis prevail, followed by nosocomial and neonatal sepsis; increasing hospital costs, stay and mortality. In adults the third part dies as a cause of sepsis. The objective was to improve competence and performance of medical doctors working at municipal Intensive Care Units, Pinar del Rio in 2011. An analytical cohort and longitudinal study was used, with both retrospective plus prospective components, . The training course was designed together with the evaluative tests. An educational intervention about diagnosis, physiopathology and management of severe sepsis. The assessment of knowledge of the medical doctors in the intensive care areas before and after the implementation of the educational intervention, as well as the modification of quality indicators (medical care of patients with sepsis), after the training course. Medical doctors; 44,5 per cent had more than 5 years experience of specialist, 16,7 per cent had received training courses about sepsis management. Initial examination 33,3 per cent fair and 38,9 bad. Final examination 50 per cent good and 33,3 per cent excellent. Improvements on the appropriated use of antibiotics and early beginning of antibiotic therapy were observed. Conclusion:
Doctors having not much experience in emergency care areas who had not received postgraduate studies in sepsis management (>50 per cent), an increase of knowledge about sepsis management after the training course was observed. The efficacy of the educational intervention to improve the conditions of patients suffering from severe sepsis in provincial emergency services increased the indicators of quality in the medical management of sepsis in municipal intensive care units(AU)
