Factores que al ingreso influyen en la evolución de pacientes con diagnóstico de pancreatitis aguda / Factors that influence in the evolution of patients with acute pancreatitis

RESUMEN

Se realizó un estudio longitudinal, de cohorte, en enfermos atendidos con el diagnóstico de pancreatitis aguda, en el servicio de Urgencias de Cirugía general del Hospital General Universitario Carlos Manuel de Céspedes de Bayamo, Granma, en el período comprendido desde el primero de enero del 2005 hasta el 31 de diciembre del 2010, con el objetivo de identificar y cuantificar la influencia de distintos factores clínicos e imagenológicos en la evolución de pacientes con pancreatitis aguda. Se incluyeron 198 pacientes con dicho diagnóstico. Como variables independientes se tomaron los factores pronósticos hipotéticos obtenidos en urgencias, y como variable respuesta la muerte durante la hospitalización. En el análisis univariado de las variables cuantitativas se encontró la influencia de la edad (RR 1,6 IC 95 por ciento 1,06-2,42), las cifras de glucemia (RR 1,57 IC 1,09-2,26) y el tiempo de evolución (RR 2,3 IC 95 por ciento 1,00-5,34) en el desarrollo de la muerte. Entre las cualitativas sobresalió la forma clínica severa de pancreatitis aguda (RR 2,4 IC 95 por ciento 1,5-3,9). En el modelo de regresión logística multivariado quedaron incluidas como variables independientes asociadas al pronóstico la edad, la forma clínica y las cifras de glucemia(AU)

ABSTRACT

A longitudinal cohort study was performed in patients with the diagnosis of acute pancreatitis in the Emergency Service of General Surgery at Carlos Manuel de Céspedes University General Hospital in Bayamo, Granma, in the period from January 1rst, 2005 to December 31rst, 2010, with the objective of identifying and quantifying the influence of various clinical and imagenological factors in the evolution of patients with acute pancreatitis. There were included 198 patients with this diagnosis. As independent variables there were taken the hypothetical prognosis factors obtained in emergencies, and as a response variable, death during hospitalization. The influence of the age was found in the univariate analysis of quantitative variables (RR 1.6; 95 percent CI 1,06-2,42), the figures of glucose (RR 1.57 IC 1,09-2,26) and the time of evolution (RR 2.3; 95 percent CI 1,00-5,34) in the development of the death. Among the qualitative variables it prevailed the severe clinical form of acute pancreatitis (95 percent CI 2.4 RR 1,5-3,9). In the multivariate logistic regression model there were included as independent variables associated with the prognosis, the age, the clinical form and blood glucose numbers(EU)
