RESUMEN
Fundamento es importante identificar los factores que incrementan el riesgo de complicaciones gastrointestinales serias, así como, determinar la conducta a seguir para reducirlas.

Objetivo:

identificar los factores de riesgo asociados con las gastropatías en pacientes atendidos en consulta de gastroenterología.

Métodos:

se diseñó un estudio de casos y testigos los cuales fueron seleccionados a partir de los pacientes que asistieron a la consulta de gastroenterología de un policlínico de Trinidad, Sancti Spíritus, durante el año 2008. Se incluyeron 104 casos de gastropatías diagnosticadas por endoscopia y 208 testigos. Se analizaron como variables edad, sexo, las exposiciones al hábito de fumar, la ingestión de café, bebidas alcohólicas y medicamentos antiinflamatorios no esteroideos. Se utilizó la regresión logística dicotómica para controlar el sesgo de confusión, después de la cual se calculó el Riesgo atribuible poblacional porcentual y la curva de dosis respuesta.

Resultados:

se identificaron como factores de riesgo independientes para las gastropatías la ingestión de antiinflamatorios no esteroideos con una curva de dosis respuesta significativa para la exposición en tabletas diarias y meses de consumo; el hábito de fumar , con una curva de dosis respuesta significativa para la exposición en cigarrillos diarios y en años de consumo. Los antiinflamatorios no esteroideos identificados como riesgos independientes fueron la indometacina, el ibuprofeno y la aspirina. El incremento de 1 año de edad también representó un riesgo en esta serie.

Conclusiones:

el hábito de fumar y la ingestión de indometacina, ibuprofeno y aspirina representaron riesgos independientes para contraer una gastropatía. Los médicos deben tener en cuenta los efectos de estos medicamentos para no prescribirlos en altas dosis y por tiempos prolongados(AU)
ABSTRACT

Background:

it is important to identify factors increasing the risk of serious gastrointestinal complications and to determine the actions to take in order to reduce them.

Objective:

to identify risk factors associated with gastropathies in patients attending the gastroenterology consultation.

Methods:

a study including cases and witness selected out of patients attending the gastroenterology consultation was conducted in a polyclinic in Trinidad, Sancti Spiritus in the year 2008. 104 cases of gastropathies diagnosed by endoscopy and 208 witnesses were studied. The variables analyzed were age, sex, smoking habit, coffee intake, alcohol consumption and non-steroidal anti-inflammatory drugs. Dichotomous logistic regression was used to control de confounding bias, and subsequently the population attributable risk percent and the dose-response curve were calculated.

Results:

the independent risk factors for gastropathies identified were ingestion of non-steroidal anti-inflammatory drugs, with a significant dose-response curve for daily tablets and months of consumption; smoking habit with a significant dose-response curve for daily cigarettes and years of consumption. Non-steroidal anti-inflammatory drugs identified as independent risks were indomethacin, ibuprofen and aspirin. The increase in 1 year of age also represented a risk in this series.

Conclusions:

smoking habit in addition to the ingestion of indomethacin, ibuprofen and aspirin were independent risks for gastropathy. Physicians should take into account the effects of these medications to avoid prescribing high doses of them for long periods(AU)
