RESUMEN
Introducción:
el linfoma difuso de células B es el tipo más común de linfoma no Hodgkin en adultos y se caracteriza por su agresividad y amplia localización. Por lo general, la primera señal de su presencia es una masa que crece rápidamente, con algunos nódulos linfáticos. También pueden aparecer fiebre, pérdida de peso y profusa sudoración nocturna.Objetivo:
se presenta un caso de linfoma de células B grandes difuso que, por su forma de presentación atípica (linfoma de partes blandas con toma de órganos internos), es de interés para los especialistas.Presentación del caso paciente de sexo femenino, 65 años de edad, color de piel mestizo y con antecedentes de hipertensión arterial. Acudió a consulta por presentar una masa tumoral dolorosa en la región izquierda de la espalda y pérdida de peso. No presentó tos, disnea u otros síntomas respiratorios. Durante el examen físico, en la pared posterolateral izquierda torácica se palpó una masa dura, no dolorosa e inamovible, de aproximadamente 12 x 12 cm. En el examen inmunohistoquímico se comprobó la presencia de un linfoma de células B grandes difuso (estadio IV).Discusión:
pese a la presentación atípica de la enfermedad, el diagnóstico inmunohistoquímico y la afectación de otros órganos (costillas y pleura) son datos consistentes con lo reportado en la literatura.Conclusiones:
ante la presencia de una masa tumoral dolorosa y pérdida de peso sin otros síntomas, se debe tener en cuenta como posible diagnóstico el linfoma de células B grandes difuso con presentación atípica(AU)ABSTRACT
Introduction:
diffuse B-cell lymphoma is the most common type of non-Hodgkin lymphoma in adults. It is characterized by its aggressiveness and wide location. Usually, the first sign of its presence is a rapidly growing mass, with some lymph nodes. Fever, weight loss and profuse night sweats may also occur.Objective:
a case of diffuse large B-cell lymphoma is presented which by its atypical presentation form (soft-tissue lymphoma with internal organs), is of interest to the specialists. Case presentation female patient, 65 years old, dark skin color and a history of arterial hypertension. She came to the clinic for presenting a painful tumor mass in the left region of the back and weight loss. Did not present cough, dyspnea or other respiratory symptoms. During the physical examination, a hard, non-painful and immovable mass of approximately 12 x 12 cm was palpated on the left posterolateral thoracic wall. In the immunohistochemical examination the presence of diffuse large B-cell lymphoma (stage IV) was confirmed.Discussion:
despite the atypical presentation of the disease, the immunohistochemical diagnosis and the involvement of other organs (ribs and pleura) are data consistent with what has been reported in the literature.Conclusions:
in the presence of a painful tumor mass and weight loss without other symptoms, diffuse large B-cell lymphoma with atypical presentation should be considered as possible diagnosis(AU)
