RESUMEN

La atresia de duodeno es el sitio más frecuente de obstrucción de vías digestivas, es diagnosticable en la etapa prenatal; en cambio, la variz de la vena umbilical constituye una rara entidad. Ambas anomalías han sido asociadas a otras malformaciones y entidades genéticas. Se presenta el caso de una paciente con embarazo de riesgo genético, remitida de su área de salud por alteraciones en la ecografía fetal correspondiente al segundo trimestre, estomago aumentado de tamaño, asas de intestino delgado ligeramente dilatadas, acompañado de un aumento del peristaltismo intestinal. Se hace diagnóstico simultáneo de atresia duodenal y variz de la vena umbilical. La gestante y su familia deciden continuar con el embarazo. El diagnóstico precoz de malformaciones obstructivas del tubo digestivo, como la atresia duodenal, y de una variz umbilical asociada, cuando se decide evolución hasta el término del embarazo, permite al equipo multidisciplinario adoptar medidas para prevenir posibles complicaciones prenatales y planificar un tratamiento quirúrgico postnatal precoz, para una mejor evolución postoperatoria(AU)

ABSTRACT

Duodenal atresia is the most frequent site of obstruction of the digestive tract and may be diagnosed in the prenatal stage. However, the varix of the umbilical vein is a rare entity. Both anomalies have been associated with other malformations and genetic entities. This study presents the case of a patient with a genetic high-risk pregnancy referred from her health area due to alterations in fetal echography corresponding to the second trimester, enlarged stomach, slightly enlarged small intestine loops, accompanied by an increase in intestinal peristalsis. A simultaneous diagnosis of duodenal atresia and varix of the umbilical vein is made. The pregnant woman and her family decide to keep the pregnancy. Early diagnosis of obstructive malformations of the digestive tract such as duodenal atresia and an associated umbilical varix, when deciding progress until pregnancy reaches full term, allows the multidisciplinary team to take measures to prevent possible prenatal complications and to plan an early postnatal surgical treatment for a better postoperative progress(AU)
