RESUMEN

Introducción:

la gastrosquisis es una malformación congénita caracterizada por una herniación visceral a través de un defecto de la pared abdominal. Comúnmente se localiza a la derecha del cordón umbilical con protrusión visceral principalmente de íleon distal, estómago e hígado; y no se encuentra cubierto por una membrana protectora. La prevalencia de gastrosquisis es de 0.5-7 por cada 10,000 recién nacidos vivos, con un promedio de 1/2700 nacimientos a nivel mundial. La mayor prevalencia de casos con gastrosquisis ocurre en madres jóvenes <20 años y un mal estado nutricional. Presentación del caso madre de 18 años, primigesta, con antecedentes gineco-obstétricos de pobre cuidado prenatal. No se realizó ultrasonidos prenatales, nace neonato en el hospital público General Madre Obrera del municipio Llallagua, departamento de Potosí a las 39,1 semanas, con peso de 2,375 gramos, parto por cesárea, con puntuación de Apgar de 1 al minuto y al quinto minuto, inmediatamente se reanima con oxígeno por penlon, intubación endotraqueal y administración de adrenalina endovenosa, no se logra recuperar y fallece a los 10 minutos de vida con diagnóstico de gastrosquisis.

Discusión:

el reconocimiento temprano de esta patología es esencial para prevenir complicaciones mortales. Permite considerar diferentes abordajes terapéutico-quirúrgicos para alcanzar un mayor porcentaje de sobrevida, especialmente en zonas donde la incidencia es alta como en nuestro medio con una media de 17 casos anuales y una sobrevida de apenas 47 por ciento.

Conclusiones:

la gastrosquisis tiene una alta tasa de supervivencia en países donde hay control prenatal apropiado y diagnóstico temprano; en este caso reportado no se realizaron los ultrasonidos como parte del seguimiento a la madre de este neonato(AU)
ABSTRACT

Introduction:

gastroschisis is a congenital malformation characterized by a visceral herniation through a defect in the abdominal wall. It is commonly located to the right of the umbilical cord with visceral protrusion mainly of the distal ileum, stomach and liver; and it is not covered by a protective membrane. The prevalence of gastroschisis is 0.5-7 per 10,000 live newborns, with an average of 1/2700 births worldwide. The highest prevalence of cases with gastroschisis occurs in young mothers <20 years and poor nutritional status. Case description18-year-old mother, with a gyneco-obstetric history of poor prenatal care. Prenatal ultrasound was not performed, a newborn is born in the General Madre Obrera public hospital in the Llallagua municipality, department of Potosí at 39.1 weeks, weighing 2,375 grams, delivery by caesarean section, with Apgar score of 1 minute and at the fifth minute, he is immediately resuscitated with penlon oxygen, endotracheal intubation and administration of intravenous adrenaline, he is unable to recover and dies at 10 minutes of life with a diagnosis of gastroschisis.

Discussion:

early recognition of this pathology is essential to prevent fatal complications. It allows considering different therapeutic-surgical approaches to reach a higher survival rate, especially in areas where the incidence is high as in our environment with an average of 17 annual cases and a survival of only 47 percent.

Conclusions:

gastroschisis has a high survival rate in countries where there is appropriate prenatal control and early diagnosis; in this reported case, ultrasounds were not performed as part of the follow-up to the mother of this neonate(EU)
