Carcinoma tubular bilateral sincrónico. Correlación radiopatológica / Synchronous bilateral tubular carcinoma: radiopathologic correlation

RESUMEN
El carcinoma tubular es una forma bien diferenciada de carcinoma ductal infiltrante, que se define por la presencia de al menos un 75% de elementos tubulares, suponiendo en el caso de las formas puras más del 90%. Presentamos un caso de carcinoma tubular puro bilateral sincrónico, hallazgo muy poco frecuente. La mamografía mostró una pequeña masa irregular de márgenes mal definidos en la mama izquierda con microcalcificaciones asociadas (hallazgo poco frecuente) y una lesión espiculada en la mama derecha. Histológicamente, se correspondían con formas puras, aunque en el lado derecho se asociaba a carcinoma intraductal micropapilar. Correlacionamos los hallazgos histológicos de las muestras obtenidas mediante biopsia con aguja gruesa (BAG) y piezas quirúrgicas de cuadrantectomía, con los hallazgos de imagen y comentamos la alternativa terapéutica elegida
ABSTRACT
Tubular carcinoma is a well-differentiated type of invasive ductal carcinoma defined by the presence of at least 75% of tubular elements; the purest forms of tubular carcinoma contain more than 90% tubular elements. We present a very rare case of synchronous bilateral pure tubular carcinoma. Mammography showed a small, irregular mass with ill-defined margins and associated microcalcifications (a rare finding) in the left breast and a spiculated lesion in the right breast. At histologic examination, both lesions were found to be pure-type tubular carcinoma, although the lesion from the right breast was associated to micropapillary intraductal carcinoma. We correlate the histologic findings for the samples obtained by core needle biopsy and surgical specimens obtained by quadrantectomy with the imaging findings and comment on the therapeutic alternative chosen
