RESUMEN
La hepatitis C crónica es un problema importantede salud pública por su posible evolución a cirrosis ysu prevalencia a pesar de su leve expresividad clínicainicial. El objeto de este artículo es revisar losmecanismos de prevención para evitar transmisionesverticales y revisar los tratamientos ya realizadosen niños junto con las terapias propuestas en laactualidad.El factor de riesgo fundamental es la elevada cargaviral materna junto con coinfección materna conVIH y adicción a drogas por vía parenteral. No estáindicada la cesárea electiva ni contraindicada la lactanciamaterna.La primera terapia propuesta en niños fue el InterferonAlfa. En el Hospital Infantil La Fe fueron tratadossiete niños de los cuales tres negativizaron lacarga viral inmediatamente tras tratamiento volviendoa ser positiva seis meses después, concluyendo,al igual que en otras series más amplias, que estaterapia no era eficaz.En la actualidad la única terapia aprobada en niñospor la FDA es el Interferon pegilado asociado aRibavirina, empleado en el contexto de ensayos clínicoso como uso compasivo con resultados esperanzadores
ABSTRACT
Hepatitis C is an important public health problemdue to its long-term consequences although its prevalenceand clinical expression is low in childhood.The objective is to review prevention mechanisms,therapies employed in children and new perspectivesof treatment.High viral load in mothers is the main risk factorfor vertical transmission. HIV co-infection and drugaddiction in mothers are also risk factors. Electivecaesarea is not indicated and breast milk is not contraindicated.Alpha Interpheron was the first treatment employedin children. In Hospital Infantil La Fe, seven childrenwere treated with alpha Interpheron. Three ofthem got negative viral load at the end of treatment,but RNA was detected again in all of them after sixmonths, thus concluded alpha interpheron is not anefficient therapy in agreement with larger studies.Now, pegylated interpheron and ribavirine has beenapproved approved by FDA in paediatric patientsand preliminary results are promising
