Infección por micobacterias en paciente sometido a trasplante de progenitores hematopoyéticos / Mycobacterium infection in patients subjected to hematopoietic stem cell transplant

RESUMEN

Se presenta un paciente diagnosticado de leucemia aguda linfoblástica de alto riesgo que, tras el tratamiento quimioterápico inicial fue sometido a un trasplante alogénico de progenitores hemopoyéticos de un donante familiar HLA idéntico. En la evolución postrasplante presentó en el día +47 un cuadro de fiebre y gran afectación del estado general. La TAC pulmonar evidenció la existencia de un patrón micronodular difuso bilateral. La fibroscopia y el lavado broncoalveolar no permitieron un diagnóstico, por lo que se procedió a una biopsia pulmonar que mostró la existencia de bacilos ácido-alcohol resistentes. El tratamiento tuberculostático permitió la resolución del caso

ABSTRACT

A patient diagnosed of high risk acute lymphoblastic leukemia is presented. After initial chomotherapy, he underwent an allogenic transplant of hemopoietic stem cells from an HLA identical family donor. During the post-transplant course, on day +47, he had a picture of fever and great involvement of the general condition. The pulmonary CT scan showed the existence of a bilateral diffuse micronodular pattern. The fibroscopy and bronchoalveolar lavage did not allow for a diagnosis, so that a pulmonary biopsy was done that showed the existence of acid-alcohol resistant bacilli.
